FLIGHT 2381 - AVL - FLIGHT CREW REPORTED AUTO PILOT FAIL IN FLIGHT. CREW DIVERTED FLIGHT AND LANDED UNEVENTFULLY. EWR MX REMOVED AND REPLACED HORIZONTAL STABILIZER CONTROL UNIT (HSCU) AND HORIZONTAL STABILIZER ACTUATOR (HSA) AND PERFORMED OPERATIONAL TEST SUCCESSFULLY. ACFT WAS APPROVED TO RETURN TO SERVICE. SDR NUMBER C2XA10RI049 FOR THE HSCU AND SDR NUMBER C2XA10RI050 FOR HSA.
IAH FLT 2836 THE CREW REPORTED THE PITCH TRIM INDICATIONS WOULD BLANK OUT IN FLIGHT CAUSING THE AUTO-PILOT TO FAIL. THE PRIMARY AND STANDBY TRIM STILL WORKED CORRECTLY. THE AIRCRAFT DIVERTED INTO CLE AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED AND REPLACED THE NR 2 IC600 COMPUTER. OPERATIONAL CHECKS WERE GOOD.
FLIGHT 2055, THE CREW REPORTED A BLEED TWO LEAK. THE AIRCRAFT RETURNED WHERE IT LANDED WITHOUT INCIDENT. MX INSPECTED THE AIRCRAFT AND PERFORMED HIGH POWER RUNS WITH NO LEAKS OR DEFECTS NOTED, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
SHV - DURING C-CHECK RT OMEGA BEAM DOUBLER AT FRAME 20 HAS CORROSION (LEVEL 2 LOCAL). MAINTENANCE REPAIRED RT OMEGA BEAM DOUBLER AT FRAME 20 IAW EMB-145 CPM 51-20-00. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
CLE - FLT 2332 - THE CREW REPORTED A LANDING GEAR AIR/GROUND FAIL MESSAGE ON THE EICAS IN FLIGHT. MAINTENANCE INSPECTED AND REPLACED THE LGEU AND RELAYS KO824, KO025, KO352, KO453.
FLIGHT 2997 - MSP - FLIGHT CREW REPORTED STRONG SMELL IN COCKPIT AND FWD CABIN. MX FOUND RT AVIONICS EXHAUST FAN INOPERATIVE. MX REMOVED AND REPLACED FAN AND PERFORMED OPERATIONAL CHECK SUCCESSFULLY. ACFT WAS APPROVED TO RETURN TO SERVICE.
FLIGHT 2291 - EWR - FLIGHT CREW REPORTED ELECTRICAL BURNING SMELL IN CABIN AROUND SEAT 1A. EWR MX REMOVED AND REPLACED SEAT 1A BALLAST AND PERFORMED OPERATIONAL SUCCESSFULLY. AIRCRAFT WAS APPROVED TO RETURN TO SERVICE.

SHV - DURING C-CHECK IT WAS FOUND THAT THE CARGO DOOR INNER SKIN HAD CORROSION AROUND SEVERAL SCREW HOLES. MAINTENANCE REMOVED THE CORROSION AND PERFORMED A DOUBLER REPAIR IAW CONTINENTAL EXPRESS EC5230-01173. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
SHV - DURING C-CHECK IT WAS FOUND THAT THE RT AILERON PCA BRACKET WAS WORN AND WOULD NOT CONTAIN THE HAT BUSHINGS. MAINTENANCE REMOVED AND REPLACED THE AFFECTED BRACKET IAW CONTINENTAL EXPRESS EC5750-01177. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH - FLIGHT 2185, THE CREW REPORTED THAT THE LEFT MAIN LANDING GEAR DISAGREE LIGHT WHEN THE GEAR WAS EXTENDED. THE AIRCRAFT LANDED AT IAH WITHOUT INCIDENT WHERE MAINTENANCE REMOVED AND REPLACED THE LEFT MAIN GEAR DOWNLOCK PROXIMITY SWITCH, OPERATIONALLY CHECKED WITH NO DEFECTS. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH - FLIGHT 2365, THE CREW REPORTED A LANDING GEAR LEVER DISAGREE WARNING ILLUMINATED INFLIGHT WHILE THE LANDING GEAR WAS IN UP AND LOCKED POSITION. NORMAL INDICATIONS ACHIEVED WHEN GEAR WAS LOWERED. THE AIRCRAFT LANDED AT IAH WITHOUT INCIDENT WHERE MAINTENANCE REMOVED AND REPLACED THE LANDING GEAR LEVER, OPERATIONALLY CHECKED WITH NO DEFECTS NOTED. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH - FLIGHT 2785, THE CREW REPORTED THAT THE MAIN CABIN DOOR COULD NOT BE OPENED FROM THE INSIDE. MAINTENANCE TIGHTENED THE INTERNAL HANDLE ON THE MAIN DOOR, OPERATIONALLY CHECKED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
FLT 2596 - EWR - CREW REPORTED EICAS LAV SMOKE MESSAGE, NO CABIN FIRE OR SMOKE NOTICED. EWR MX PERFORMED OPERATIONAL CHECK IAW EMB 145 AMM SUCCESSFULLY AND COULD NOT DUPLICATE THE PROBLEM. ACFT WAS APPROVED TO RETURN TO SERVICE.
FLT 2994 - EWR - CREW REPORTED FALSE LAV SMOKE ALARM IN FLIGHT. EWR MX REMOVED AND REPLACED LAV SMOKE SENSOR IAW EMB145 AMM 26-14-01 AND PERFORMED OPERATIONAL TEST SUCCESSFULLY. ACFT WAS APPROVED TO RETURN TO SERVICE.
CLE FLIGHT 2451 THE CREW REPORTED UPON ARRIVAL IN MKE THE F/A WAS UNABLE TO OPEN THE MAIN CABIN DOOR. MAINTENANCE INSPECTED AND REPLACED THE MAIN CABIN DOOR SEAL. OPERATIONAL CHECKS WERE GOOD. THE AIRCRAFT WAS RELEASED FOR SERVICE.
TYS DURING A CURRENT 2C INSPECTION THE CENTER LAV OMEGA BEAM TOP CAP WAS FOUND CORRODED. MAINTENANCE REMOVED AND REPLACED THE THE CORRODED AREA IAW EMB 145XR SRM 51-40-02 AND 51-20-02-06. ALL WORK WAS ACCOMPLISHED AND WILL BE RELEASED UPON COMPLETION OF THE 2C INSPECTION.

TYS DURING A 2C INSPECTION BOTH GUSSETS ON THE RIGHT OMEGA BEAM IN THE GALLEY HAD SISNS OF CORROSION. MAINTENANCE REMOVED AND REPLACED THE RIGHT OMEGA BEAM AND BOTH GUSSETS IAW 145XR SRM 51-40-02 AND 51-20-03. ALL WORK WAS ACCOMPLISHED AND WILL BE RELEASED WHEN THE C-CHECK INSPECTION IS COMPLIED WITH.
CREW REPORTED, "STRONG ODOR OF SMOKE IN CABIN AND COCKPIT". MX PERFORMED GENERAL VISUAL INSP OF BLEED SYSTEM DUCTS NR 1 AND NR 2 IAW EMB145XR AMM NR 36-11-09, NO DEFECTS NOTED. RAN BOTH ENGINES ON 2 DIFFERENT OCCASIONS IAW EMB145XR AMM NR 71-00-01 AT LOW AND HIGH POWER SETTINGS, CHECKED BOTH AIRBLEED SYSTEMS IAW EMB145XR AMM NR 36-00-00-700-803-A. NO SMOKE ODOR FOUND IN CABIN OR COCKPIT, SYSTEMS OPS CHECKED GOOD.
TYS, DURING A SCHEDULED INSPECTION THE RIGHT AILERON TO PCA ATTACH FITTING HOUSING WAS GOUGED AND ELONGATED. MAINTENANCE REMOVED AND REPLACED THE FITTING IAW EMB-145XR-AMM 27-11-00 AND EC5750-01177.
THE CREW REPORTED THAT ON DESCENT INTO IAH THE NUMBER TWO ENGINE FAILED (UNCOMMANDED SHUT DOWN). THE AIRCRAFT LANDED AT IAH WITHOUT INCIDENT WHERE MAINTENANCE REMOVED AND REPLACED THE NUMBER TWO ENGINE, OPERATIONAL CHECKED GOOD, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
DURING CLIMBOUT THROUGH 27,000 FT THE NR 2 ENGINE FAILED. ATTEMPTS WERE MADE TO RESTART THE ENGINE WITHOUT SUCCESS. THE AIRCRAFT RETURNED TO DEN AND LANDED WITHOUT INCIDENT WHERE THE AFFECTED ENGINE WAS REPLACED.
IAH- FLIGHT 2719, THE CREW NOTED SMOKE IN THE CABIN DURING CLIMB, DID AN AIR RETURN, AND, LANDED WITHOUT INCIDENT. IAH- MAINTENANCE INSPECTED THE AIRCRAFT, AND, PERFORMED A OPERATIONAL CHECK OF THE AIR CONDITIONING SYSTEM, NO DEFECTS NOTED, AND, THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
