FLIGHT ATTENDANTS CLAIM THEY SMELLED AN ELETRIC SMELL LANDING RE PEAT WU SMOKE AND ODOR QUESTIONAIRE ATTACHED INVESTIGATED CABIN, TURNED ON EVERYTHING ELECTRICAL INSIDE AND DID FULL SWEEP OF CABIN, NO ODOR DETECTED. STARTED APU AND OPERATED 1 PACK AT A TIME, OVERWHELMING SMELL OF BURNED OIL IN COCKPIT AND WHOLE CABIN. RAN BOTH ENGINES ONE AT A TIME WITH APU BLEED OFF ISOLATING SYSTEMS AND PACKS, BURNED OIL SMELL STILL PRESENT FOR BOTH ENGINES WHEN PACKS RUNNING. SHUT ENGINES OFF AND TURNED APU BLEED BACK ON, ODOR IS MORE POWERFUL AND OVEWHELMING WITH APU BLEED ON. FOUND PUDDLE OF OIL ACCUMULATING AT APU TURBINE FROM LOOKING AT EXHAUST FROM THE OUTSIDE WITH FLASHLIGHT AND BLACKLIGHT. ABNORMAL AMOUNT OF SMOKE ALSO COMING OUT OF EXHAUST WHERE PUDDLE WAS FOUND AND DIDNT DISSIPATE LIKE NORMAL. BURNED OIL SMELL DETECTED AT #1 PACK EXHAUST VENT WHILE PACKS RUNNING. SUSPECT APU AND BOTH PACK SYSTEMS NEED TO BE REPLACED DUE TO EVIDENCE AND CONTAMINATION. REF AMM 05-51-58. REPLACED CLAMP BLOCK AND COUPLING O-RINGS. AFTER WET MOTOR AND IDLE ENG RUN, NO LEAKS FOUND. ACP'D NR 2 ENG MAIN FUEL FEED LINE INSP IN PYLON. NO LEAKS FOUND. ACFT OK FOR SERVICE.
BATTERY CHARGER MSG LWR EICAS. REPLACED MAIN AC BATTERY. EICAS MSG CLEARED UP & BATTERY CHARGING OKAY.
NR 1 ENG OIL PRESS & QTY ZERO, ENG SHUTDOWN. ENGINE SUFFERED A T-2 BLADE FAILURE. RESULTING UNBALANCE BROKE THE OIL TUBES LEADING TO THE OIL LOSS. ENGINE REPLACED. ACCOMPLISHED TWO CROSS COUNTRY FLIGHTS WITH NO DISCREPANCIES REPORTED.
RIGHT IDG LEAKING 25 DROPS PER MIN RUNNING. ON SHUTDOWN THERE IS A STEADY STREAM OF OIL UNTIL SHAFT CAVITY EMPTIES. REMOVED RT IDG. CIRCUIT BREAKER 'R IDG DISC' PULLED AT LOCATION 'J2' ON P210 PANEL. REPLACED THE NR 2 ENGINE IDG. ALSO REPLACED THE IDG DRIVE SEAL. SYSTEM CKS NORMAL AFTER ENGINE RUN.
FUELER REPORTED, WING FUEL GAUGES BOUNCE AROUND DIFFERS FROM COCKPIT. MUST USE ALTERNATE FUELING PROCEDURES TO VERIFY FUEL. REPLACED FUEL PROCESSOR.
NR 2 ENGINE WON'T SHUT OFF. NR 2 ENG AFTER SHUTDOWN, NOTED FUEL FLOW STUCK AT 1400. TRIED TO SHAKE CUTOFF SWITCH - NO HELP. CYCLED TO ON AND BACK TO CUTOFF, AFTER MORE THAN 30 SECONDS OFF WENT TO ZERO. THEREAFTER, SMELLED STRONG FUEL ODOR. ALSO RAMP SVC REPORTED TAIL PIPE FLAMED AND HEAVY SMOKE. ALSO NOTED, STATUS MSG 'RIGHT FUEL VLV'. REPLACED NR 2 ENG FUEL SHUTOFF SWITCH. SUSPECT NR 2 FUEL CUTOFF SW INTERMITTENT. REPLACED NR 2 ENG FUEL SHUTOFF SWITCH. STARTED ENG 2XS WITH POSITIVE SHUT DOWN.

LOST ENTIRE CENTER HYDRAULIC SYSTEM AT 1000 FEET ON FINAL APPROACH.
FUEL QUANTITY PROCESSOR HAS AN INTERNAL FAULT, CMC MSG 28-20050, ACTIVE. FOUND CHANNEL-L INOP, REPLACED FUEL QUANTITY PROCESSOR UNIT PER MM28-41-01-401, OPS NORMAL. AC OK FOR SERVICE.

DUAL WEATHER RADAR FAILURE. EMERGENCY DECLARED. TROUBLE SHOOTING REVEALED THAT RADAR SCANNER WAS FAULTY. REPLACED SCANNER. SYSTEMS OP CHECKED OK. SUSPECT CONNECTOR PROBLEM WITHIN SCANNER, OR CONNECTION TO RADAR BOXES.

NR 1 ENGINE FAILED. ACFT DIVERTED TO SLC DUE TO LT ENG SURGE, EGT EXCEEDED TEMPERATURE LIMITS. METAL IN TAIL PIPE. TEARDOWN REVEALED EXTREME BURNING OF HPT STAGE-1 NOZZLE GUIDE VANES (NGVS). SEVERAL NGVS BURNED IN HALF, & REMAINDER SHOWED SIGNIFICANT OVERLOAD DAMAGE. DOWNSTREAM DAMAGE SIGNIFICANT. ALL HPT STAGE-1 BLADES FRACTURED AT PLATFORM. REMAINING HPT & LPT AIRFOILS (BLADES & VANES) SCRAPPED DUE TO HEAVY IMPACT DAMAGE. ALTHOUGH NO FUEL SYS CONTAMINATION FOUND, 1 FUEL NOZZLE HAD 2 OF ITS HEATSHIELDS DISPLACED APPROX .080" INWARD FROM NORMAL POSITION. THIS IS A FIRST-OF-KIND INDUSTRY FAILURE, & PWA IS INVESTIGATING THE POSSIBILITY OF FUEL NOZZLE TORCHING.
COFFEEMAKER ELECTRICAL SPARKING. REPEATED ARCING NEAR MID GALLEY COFFEEMAKER PWRD OFF GALLEY & LTS. CONVERSATION WITH PURSER DESCRIBES 2 INSTANCES OF FLASH FROM BELOW COUNTERTOP AFTER POWERING OFF GALLEY. EXAMINATION OF AIRCRAFT GALLEY WIRING & COFFEEMAKERS REVEALS NO SIGNS OF ARCING. ALL DEVICES WERE OPERATED IN EXCESS OF 30 MINUTES WITH NO ANOMALIES NOTED.

ONE MINUTE AFTER TAKEOFF, CREW RECEIVED AFT CARGO FIRE INDICATION. FIRE BOTTLES DISCHARGED AND FIRE INDICATION WARNING DID NOT GO AWAY IMMEDIATELY. CREW DECLARED EMERGENCY AND RETURNED TO HNL. OVERWEIGHT LANDING. UPON LANDING FIRE CREW CHECKED FOR A HEAT SOURCE IN AFT PIT AND FOUND NONE. ONLY LUGGAGE WAS IN THE AFT CARGO PIT. SUBSEQUENT CARGO SMOKE DETECTION TEST AND FIRE WARNING TEST PASSED. NO MAINTENANCE MESSAGE FAULTS WERE RECORDED THAT DIRECTED TO A SYSTEM FAULT. INLET LINES TO DETECTOR WERE INSPECTED FOR MOISTURE AND NONE WAS FOUND. A/C WAS FLOWN TO SFO UNEVENTFULLY WITH EMPTY PITS. DURING EXTINGUISHING SYSTEM REFURBISHMENT, NO ABNORMALITIES WERE NOTED.
