WITH AUTOFEATHER ARMED AND POWERED AND PASSING APPROX 93-94 PERCENT THE AFX ANNUNCIATOR ILLUMINATES. AFTER TROUBLESHOOTING, REMOVED DEFECTIVE RT HIGH OIL PRESSURE SWITCH, INSTALLED NEW SWITCH IAW MM CHAPTER 61-21-01. GROUND RUN UP OPERATED WITHIN SPECIFICATIONS.
MAJOR REPAIR - DURING A SCHEDULED ROUTINE INSP, MX FOUND A CRACK IN THE BULKHEAD WHERE THE HYD POWERPACK ATTACHES, LT INBD WING, 3.6" IN LENGTH. MX REPAIRED THE AFFECTED AREA AND PERFORMED ALL REQUIRED RII INSPECTIONS IAW EO 11-1900-57-004. MATERIALS UTILIZED ARE INCLUDED IN THE EO.
MAJOR REPAIR - DURING A SCHEDULED ROUTINE INSPECTION MX FOUND THE AILERON SCRAPED 32" OF OTBD, TOP LWR AND FWD SKIN DAMAGED AND OTBD 4 RIBS AND DOUBLERS. MAINTENANCE REPAIRED THE AFFECTED AREA AND PERFORMED ALL REQUIRED RII INSPECTIONS I.A.W. EO 11-1900-27-003. MATERIALS UTILIZED ARE INCLUDED IN THE ENGINEERING ORDER.
SIG EVENT - PILOT REPORTED THAT THE LT AFX ANNUNCIATOR AND REPEATER FLICKERED AND THEN WENT OUT, CAME BACK ON FOR A WHILE AND WENT OUT AGAIN. MX REPLACED PCB 129.
MAJOR REPAIR - DURING A SCHEDULED ROUNTINE INSPECTION MAINTENANCE FOUND THE RT STABILON OB TIP DAMAGED WHILE PARKING IN HANGER. MAINTENANCE REPAIRED THE AFFECTED AREA AND PERFORMED ALL REQUIRED RII INSPECTIONS IAW EO 11-1900-55-005. MATERIALS UTILIZED ARE INCLUDED IN THE ENGINEERING ORDER.

MAJOR REPAIR - DURING A SCHEDULED ROUNTINE INSP MX FOUND 4 CRACKS AROUND THE VENT HOLE IN THE VERT STAB BEHIND THE BULLET CTR WEB, ALL 4 CRACKS ARE .5" IN LENGTH. SUPLEMENTAL INFORMATION WILL BE SUBMITTED UPON COMPLETION OF THE REPAIR AND RETENTION OF THE PAPERWORK.
MAJOR REPAIR - DURING ROUTINE SCHEDULED MX A CRACK WAS FOUND IN A CUTOUT AT STRINGER 9AR AND F.S. 177.3. CRACK IS APPROX .8" IN LENGTH. MX REPAIRED THE AFFECTED AREA AND PERFORMED ALL REQUIRED RII INSPECTIONS IAW EO 10-1900-53-019. MATERIALS UTILIZED ARE INCLUDED IN THE EO.
DURING ROUTINE SCHEDULED MX, A CRACK WAS FOUND IN A CUTOUT AT STRINGER 9AR AND FS 163.9. CRACK IS APPROX .5" IN LENGTH. MX REPAIRED THE AFFECTED AREA AND PERFORMED ALL REQUIRED RII INSPECTIONS IAW EO 10-1900-53-018. MATERIALS UTILIZED ARE INCLUDED IN THE EO.
MAJOR REPAIR - PILOTS REPORTED THAT THE FUSELAGE WAS PUNCTURED IN THE LT FUSELAGE JUST FWD OF FS 273.25 ON THE UPPER SKIN. DAMAGED AREA APPROX 2.1 INCH BY 3.5 INCH. (2) CRACKS PRESENT. (1) CRACK APPROX 2.3 INCHES AND THE OTHER 1.1 INCH IN LENGTH. MX REPAIRED THE AFFECTED AREA IAW EO 09-1900-53-020. RII INSPECTION C/W IAW EO 09-1900-53-020.
MAJOR REPAIR - PILOTS REPORTED A SMALL DENT IN THE LT FUSELAGE JUST FWD OF FS 258.25 ON THE UPPER SKIN. DAMAGED AREA APPROX .250 INCH BY .750 INCH. MX REPAIRED THE AFFECTED AREA IAW EO 09-1900-53-019. RII INSP C/W IAW GLA EO 09-1900-53-019.
DURING ROUTINE SCHEDULED MX TWO CRACKS ON TOP OF FUSELAGE AT FS 150.6 WERE DISCOVERED. TOP CENTERLINE 1 RIVET HEAD SHEARED INBD OF CRACK 1.250 INCHES LONG AND A .250 INCH CRACK LT OF CENTERLINE. MX REPAIRED AFFECTED AREA IAW EO 09-1900-53-009. REPAIR AREA APPROX 7.750 INCHES BY 4 INCHES. RII INSP COMPLIED IAW EO INSTRUCTIONS.
FLIGHT 5145 - THE PILOTS REPORTED THAT DURING CRUISE THE VACUUM PRESSURE WAS READING AS LOW AS 3.5 AT FLIGHT LEVEL 220. MAINTENANCE REMOVED AND REPLACED THE VACUUM REGULATOR VALVE IAW B1900D MM 37-00-01. THE OPERATIONAL CHECK WAS GOOD.
PILOTS REPORTED THAT THE NR 2 NAV RADIO WAS INOPERATIVE AND RETURNED TO DEN. MAINTENANCE REMOVED AND REPLACED THE VOR-ILS RECEIVER IAW MM 34-20-00. OPERATIONAL CHECK WAS GOOD.
PILOTS REPORTED THAT TAKEOFF ABORTED DUE TO 2 PROP RPM AT 1500. MAINTENANCE FOUND THE GOVERNOR CONTROL CABLE LOOSE FROM FORWARD MOUNT. RE-ATTACHED & SAFETIED. OPERATIONAL CHECKS GOOD I.A..W 61-20-00.
ABORTED TAKEOFF BECAUSE THE RT PROPELLER RPM ONLY CAME UP TO 1600 AND THEY THEN ABORTED THE TAKEOFF. MAINTENANCE TROUBLESHOT THE SYSTEM AND FOUND THE RIGHT PROPELLER GOVERNOR OUT RIG. MAINTENANCE RIGGED THE ENGINEBY TURNING THE PROP RPM SCREW OUT AND TESTED THE SYSTEM I.A.W. MM 71-00-00. RII INSPECTION COMPLIED WITH IAW TASKCARD 4.3.13. INSPECTION WAS SATISFACTORY.
ON FINAL APPROACH THE PILOTS REPORTED THAT THE NR 2 ENGINE BEGAN TO AUTO-FEATHER AT 500 LBS OF TORQUE. MX ADJUSTED THE LT AND RT AUTOFEATHER ARMING SWITCHES IAW B1900D MM 61-21. OPS CHECK WAS GOOD.
DURING SCHEDULED ROUTINE MAINTENANCE DISCOVERED RT OB ELEVATOR HINGE BRACKET EXFOLIATED. REPLACED RT OB ELEVATOR HINGE BRACKET.
DURING SCHEDULED ROUTINE MAINTENANCE DISCOVERED RT OB ELEVATOR HINGE BRACKET EXFOLIATED. REPLACED RT OB ELEVATOR HINGE BRACKET.
FLIGHT: 9269, DURING FLIGHT, RT FIRE DISCHARGE ANUNCIATOR ILLUMINATED. REPLACED TERMINAL ON RT FIRE EXTINGUISHER.
FLIGHT: 9200, DURING APPROACH LANDING GEAR UNSAFE LIGHTS DID NOT ILLUMINATE (3) GREEN INDICATION DID NOT ILLUMINATE WHEN GEAR HANDLE PLACED IN THE DOWN POSITION. RETURNED TO DEPARTURE, LANDED WITHOUT INCIDENT. REPLACED CIRCUIT BREAKER 16 AND RESECURED LT MAIN LANDING GEAR DOWNLOCK SWITCH.
FLIGHT: 9267 EYW/MIA DURING APPROACH LANDING GEAR HORN AND LIGHT ANNUNCIATED AT ABOUT 75 PERCENT N1 OR ABOUT 500LBS OF TORQUE. SYSTEM WORKS BUT AT A LOWER VALUE THAN NORMAL. ADJUSTED PEDESTAL SWITCHES OPERATIONAL CHECK NORMAL. TEST FLIGHT SATISFACTORY.
FLIGHT: 9247 FLL/FLL - AFTER TAKEOFF LANDING, GEAR DID NOT RETRACT. RETURNED TO FLL LANDED WITHOUT INCIDENT. REPLACED THE LANDING GEAR CONTROL CIRCUIT BREAKER. OPERATIONAL CHECK NORMAL.
DURING FLIGHT, STRONG LUBRICATION SMELL PRESENT IN THE COCKPIT AND CABIN. INSPECTED UNDER FLOORBOARDS IN CABIN AND COCKPIT CLEANED HYDRAULIC FLUID DUE TO OVERSERVICE NO LUBRICATION SMELL NOTED AT THIS TIME.
FLIGHT:9280 EYW/TPA AFTER TAKEOFF WHEN LANDING GEAR SELECTED UP THREE GREEN ANNUNCIATOR ILLUMINATED, MOTOR RUNS INTERMITTENT, BREAKER POPS. CONTINUED TO TPA LANDED WITHOUT INCIDENT. MAINTENANCE FERRY TO FLL. REPLACED LEFT MAIN LANDING GEAR ACTUATOR. TEST FLIGHT SATISFACTORY.
FLIGHT: 9235 JAX/TPA DURING APPROACH LANDING GEAR RELAY CIRCUIT BREAKER TRIPPED. RESET CIRCUIT BREAKER AND LOWERED LANDING GEAR NORMALLY. REPLACED LANDING GEAR SERVICE VALVE.
FLIGHT:9266 MHH/PBI AFTER TAKEOFF LANDING GEAR CIRCUIT BREAKER TRIPPED LANDING GEAR WOULD NOT RETRACT. DIVERTED TO FLL LANDED WITHOUT INCIDENT. TROUBLESHOT LANDING GEAR SYSTEM. CHECKED POWER PACK FLUID. LANDING GEAR OPERATIONAL CHECKS NORMAL NO DEFECTS NOTED.
FLIGHT: 9246, AFTER TAKEOFF LANDING GEAR CIRCUIT BREAKER POPPED COULD NOT RETRACT THE LANDING GEAR. DIVERTED, LANDED WITHOUT INCIDENT. REPLACED TIME DELAY PC BOARD.
FLIGHT: 9287, DURING APPROACH, LANDING GEAR RELAY CIRCUIT BREAKER POPPED.CLEANED LT AND RT NOSE LANDING GEAR ACTUATOR CONNECTOR PLUGS FOR DOWN LOCK SWITCHES. OPERATIONAL CHECK NORMAL, NO DEFECTS NOTED.
EYW/FLL DURING APPROACH LANDING GEAR WARNING SOUNDED. TIGHTENED LANDING GEAR ANNUNCIATOR HOUSING.
DURING CRUISE, OIL PRESSURE LOW ANNUNCIATOR ILLUMINATED, OIL PRESSURE GAUGE INDICATED 50 AND SLOWLY DROPPING, CREW SECURED LT ENGINE AND DECLARED AND EMERGENCY AND LANDED. REMOVED AND REPLACED LT GAS GENERATOR, POWER SECTION AND PROPELLER, RIGGED ENGINE, PERFORMED GROUND RUN NO LEAKS NOTED. P/N PT6A67DGG S/N 114371.
RT BRAKE WILL NOT RELEASE. RESECURED BRAKE PARKING KNOB, LUBED ACT.
SHORTLY AFTER DEPARTURE FROM PIT CREW NOTICED CAPT'S GYROSCOPIC DIRECTIONAL COMPASS SYSTEM WASN'T WORKING CORRECTLY. SO DECEIDED TO RETURN TO PIT. AIRCRAFT LANDED SAFELY WITH NO FURTHER INCIDENT. T/S REPAIRED PINS 5 AND 7 OF CONNECTOR J203 AT NR 1 FLUX DETECTOR. OPS CK GOOD. P/N FREESTOCK
WHILE PERFORMING AN INSPECTION IN THE VERTICAL STAB, THE RIB FLANGE AT CANTED STAB STA 39.289 WAS FOUND CRACKED IN ONE PLACE ON THE RIGHT AND ONE PLACE ON THE LEFT SIDE NEAR THE CENTER STRINGER INTERSECTION. REPAIRED CRACK TO RIB FLANGE IN ACCORDANCE TO BMM P/N 114-590021-9, 51-70-03.
WHILE PERFORMING AN INSPECTION IN THE VERTICAL STAB, THE RIB FLANGE AT CANTED STAB STA 69.184 WAS FOUND CRACKED IN ONE PLACE ON THE RIGHT SIDE NEAR AFT STRINGERS INTERSECTION REPAIRED CRACK TO RIB FLANGE IN ACCORDANCE TO BMM P/N 114-590021-9, 51-70-03.
WHILE PERFORMING AN INSPECTION IN THE VERTICAL STAB, THE RIB FLANGE AT CANTED STAB STA 54.026 WAS FOUND CRACKED IN THREE PLACES ON THE RIGHT SIDE NEAR THE FORWARD, CENTER AND AFT STRINGERS AND TWO PLACES ON THE LEFT. REPAIRED CRACKS TO RIB FLANGE IN ACCORDANCE WITH BMM P/N 114-590021-9, 51-70-03.
WHILE DOING AN INSPECTION, MAINTENANCE DISCOVERED THE LT INBOARD MAIN LANDING GEAR KEEL WAS GOUGED. MAINTENANCE REPAIRED THE KEEL. NO TEARDOWN REPORT WILL BE REQUIRED.
UPON A MAINTENANCE CHECK IT WAS DISCOVERED THAT BOTH THE RIGHT AND LEFT SPAR CAP WAS CHAFING THE WHEEL WELL. BLENDED THE CHAFED AREA IN ACCORDANCE TO RAYTHEON FIELD REPAIR. NO TEARDOWN REPORT WILL BE REQUIRED.
UPON APPROACH FOR LANDING ON FLIGHT 5441 IN CHARLOTTE NORTH CAROLINA, THE LEFT ENGINE TORQUE FELL TO BETWEEN 0-500LBS. ALL THE OTHER ENGINE INDICATIONS WERE NORMAL. THE CREW THEN SHUT DOWN THE ENGINE. THE CREW ALSO DID THE EMERGENCY CHECKLIST AND DECLARED AN EMERGENCY. THE AIRCRAFT LANDED SAFELY WITH NO FURTHER INCIDENTS. THE CORRECTIVE ACTION WAS THAT MAINTENANCE REMOVED AND REPLACED LEFT HAND PNUEMATIC TUBE ASSEMBLY P3 AIR FRONT TUBE. NO TEARDOWN WILL BE REQUIRED.
FLT 5499 - JAX-MCO - ON APPROACH, THE CREW NOTICED THE RT PROP PITCH CHANGING UNCOMMANDED. THEY THEN NOTICED THE RT TOP COWLING WAS COMING LOOSE, AND WAS RESTING ON THE PROP GOVERNOR AND BETA VALVE. THE CREW DECLARED AN EMERGENCY, SHUT DOWN THE RT ENGINE, AND LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE RESECURED THE COWLING AND PERFORMED AN MAINTENANCE RUN ON THE AIRCRAFT AND SYSTEMS. NO DEFECTS WERE NOTED. THE AIRCRAFT WAS RETURNED TO SERVICE. (M)
FLT 5451 - PFN-MCO - ON SHORT FINAL APPROACH TO MCO, CREW RECEIVED A 'TOO LOW GEAR' AURAL WARNING. INITIATED A GO AROUND AND REQUESTED THE RESCUE TRUCKS STANDBY. ALL OTHER GEAR INDICATIONS WERE NORMAL AND THE GPWS DID NOT ACTIVATE ON THE SECOND APPROACH. THE AIRCRAFT LANDED WITHOUT INCIDENT. MAINTENANCE DEFERRED THE GPWS PER MEL 34-18. IN SUBSEQUENT MAINTENANCE, THE GPWS COMPUTER WAS REMOVED AND REPLACED. (M)
