AIRCRAFT WAS APPROACHING STELLAR AIRPORT IN RENO, NV FOR FUEL WHEN THE ENGINE CHIP LIGHT CAME ON. PILOT LANDED AT THE AIRPORT AND CALLED MECHANICS. MECHANICS PULLED CHIP DETECTORS AND FOUND BOTH TO BE HEAVILY CONTAMINATED INCLUDING OIL SAMPLE.
XMSN CHIP LIGHT CAME ON DURING CRUISE FLIGHT.
ENGINE CHIP LIGHT CAUTION ILLUMINATED DURING FLIGHT. PILOT MADE A PRECAUTIONARY LANDING ON THE SIDE OF INTERSTATE 15. MECHANIC INSPECTED THE ENGINE CHIP DETECTORS. ON THE FWD R/H CHIP DETECTOR, A VERY SMALL METALLIC SLIVER WAS FOUND. CLEANED CHIP DETECTOR AND HAD THE PILOT PERFORM A 10-MINUTE PENALTY GROUND RUN. UPON SUCCESSFUL GROUND RUN THE AIRCRAFT WAS RETURNED TO SERVICE.
CREW SUSPECTED FUEL SMELL IN REAR SEAT. *** NOTE THIS AIRCRAFT STILL REMAINS OUT OF SERVICE AT THIS TIME FOR TROUBLE SHOOTING, NO PARTS HAVE YET BEEN FOUND AT THIS TIME TO BE FAILING AND THE FUEL SMELL HAS YET TO BE DUPLICATED.***
DURING FLIGHT THE MEDICAL CREW STATED THAT THERE WAS A STRONG SMELL OF FUEL FUMES IN THE CABIN. PILOT DISCUSSED WITH THE CREW AND THEY DECIDED TO RETURN THE AIRCRAFT TO BASE. AFTER EXTENSIVE TROUBLE SHOOTING THE MECHANINC FOUND A FAULTY FUEL FILLER CAP.
AFTER TAKE OFF CREW HAD A FUEL ODOR INSIDE COCKPIT. AIRCRAFT TURN AROUND AND FOUND FUEL CAP NOT INSTALLED PROPERLY BY FBO. IAW GOM 9.4, AIRCRAFT PREFLIGHT WALK-AROUND, COWLS, CAPS AND FASTENERS SHOULD BE CHECKED FOR PROPER INSTALLATION BEFORE FLIGHT
DURING ENG RUN UP FOR TAKEOFF, WHEN ENG REACHED 100% RPM THE ACFT EXPERIENCED AN INCREASED MAIN ROTOR VIBRATION. AFTER TROUBLESHOOTING, DETERMINED MAIN ROTOR HUB ASSEMBLY BEARINGS WORN. THIS ACFT REMAINS OUT OF SERVICE AT THIS TIME AWAITING A REPLACEMENT MAIN ROTOR HEAD ASSEMBLY.
WHILE INFLIGHT, THE FADEC DEGRADE & RESTART LIGHT ILLUMINATED. THE PILOT RETURNED TO BASE WITHOUT INCIDENT. DOWNLOADED FAULT CODE & DETERMINED HMU PLA POTENTIOMETER ISSUE. CYCLED THE THROTTLE SEVERAL TIMES FROM CUT OFF THROUGH 100% TO CLEAN ANY DIRT OR GRIT FORM POTENTIOMETER. CLEARED OLD FAULT CODES & HAD NO NEW FAULT CODES. GND RUN ACFT & NO NEW DISCREPANCIES OCCURRED. ACFT WAS RETURNED TO SERVICE.
TRANSMISSION CHIP LIGHT ILLUMINATED IN FLIGHT WHILE EN-ROUTE TO HOSPITAL. THE PILOT LOCATED A SAFE SPOT TO LAND, & LANDED WITHOUT INCIDENT. INSPECTED THE CHIP PLUGS & FOUND A SMALL METALLIC SLIVER ON THE BOTTOM CHIP PLUG. REMOVED & PERFORMED THE REQUIRED MX TO INCLUDE A 30 MINUTE PENALTY RUN IAW THE MM WITH NO DEFECTS NOTED. THE ACFT WAS RETURNED TO SERVICE AT THAT TIME.
UPON DEPARTING, THE TRANSMISSION CHIP LIGHT ILLUMINATED. PILOT LANDED & CALLED MAINTENANCE TECHNICIAN. A VERY SMALL SLIVER WAS FOUND ON THE TRANSMISSION TOP PLUG. THE LOWER PLUG WAS CLEAN & THE FREEWHEELING UNIT PLUG WAS CLEAN. CLEANED THE TOP CHIP PLUG & REINSTALLED. PERFORMED GND RUN & RETURNED TO SERVICE.
DURING NORMAL SHUTDOWN A FADEC DEGRADE LIGHT AS THE THROTTLE WAS PLACED TO OFF. THE MECHANIC ARRIVED & VERIFIED OVERSPEED TEST 4 & 5 FAIL & AN OVERSPEED FAIL. VERIFIED ALL CONNECTORS TO BE CLEAN & SECURE IAW RR 250 C47B MM CHPT 73-25-01 FAULT ISOLATION MANUAL. THE FAULTS WERE THEN CLEARED. PERFORMED A GND RUN WITH NO DISCREPANCIES NOTED. THE ACFT WAS RETURNED TO SERVICE AT THAT TIME.
PILOT REPORTED ENGINE CHIP LIGHT ILLUMINATED. REMOVED UPPER AND LOWER CHIP PLUGS AND REMOVED A SMALL AMOUNT OF FLAKES. CLEANED THE PLUGS AND REINSTALLED. THE ENGINE WAS RUN AT 100 PERCENT FOR 30 MINUTES WITH NO ADDITIONAL WARNING LIGHTS. THE ACFT WAS RETURNED FOR SERVICE.
ENGINE CHIP LIGHT ILLUMINATED DURING FLIGHT. PERFORMED A PRECAUTIONARY LANDING. MECHANIC ARRIVED, PULLED AND INSPECTED ALL ENGINE CHIP DETECTORS AND FOUND CHIPS/FLAKES ON THE LOWER ENGINE CHIP PLUG. CLEANED CHIP PLUG, PERFORMED 30 MINUTE PENALTY RUN. NO LIGHT INDICATED AFTER 30 MINS. FOR PRECAUTIONARY MEASURES, MECHAINC DRAINED, FLUSHED AND SERVICED ENGINE AS WELL AS REPLACED ENGINE OIL FILTER. THIS ACFT HAS RETURNED TO SERVICE.
DURING FLIGHT, ENGINE CHIP LIGHT ILLUMINATED. DRAINED, FLUSHED, CHANGED FILTER, GROUND RUN COMPLETE, NO CHIPS DETECTED.
