AT CLT, CREW REPORTED THE EMERGENCY ESCAPE PATH FLOOR LIGHTING INOPERATIVE AT ROWS 2 AND 3. MAINTENANCE INSPECTED THE FLOOR TRACK EMERGENCY LIGHTS, REMOVED AND REPLACED A CONNECTOR AND PERFORMED CHECKS USING THE AIRCRAFT MAINTENANCE MANUALS. AIRCRAFT WAS RELEASED FOR SERVICE.
DURING INSPECTION CRACKS WERE FOUND ON THE FS 559.00 CAP ANGLE PRESSURE BULKHEAD. REPAIRS WERE MADE USING MAINTENANCE MANUALS AND AIRCRAFT RELEASED FOR SERVICE.
CREW REPORTED A LIGHTNING STRIKE TO THE NOSE OF THE AIRCRAFT ON APPROACH TO CLT. AIRCRAFT LANDED IN CLT WITHOUT INCIDENT. MAINTENANCE INSPECTED THE AIRCRAFT AND REMOVED AND REPLACED THE AFT LOWER NAV LIGHT LENS AND RETAINER. MAINTENANCE REPAIRED BURN MARKS TO THE LEFT ELEVATOR, TAIL CONE AND RIGHT HORIZONTAL STABILIZER TIP FAIRING AND REMOVED AND REPLACED STATIC DISCHARGERS. CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE. ALL WORK WAS ACCOMPLISHED USING THE AIRCRAFT MAINTENANCE MANUALS AND REOâS.
DURING MAINTENANCE A DENT WAS FOUND AT FS 259.0 STR 26R TO STR 27R. REPAIRS WERE MADE PER MANUFACTURERS MANUALS AND AIRCRAFT RELEASED FOR SERVICE.
DURING INSPECTION A CRACK WAS FOUND ON THE REAR PRESSURE BULKHEAD WEB THAT SURROUNDS THE LINTEL FITTING AT FS 621.15. AREA WAS REPAIRED USING REO 601R-53-61-1230 REV. AIRCRAFT WAS RELEASED FOR SERVICE.
GEAR DISAGREE DURING FLIGHT NOSE GEAR SHOWED RED WHITE GEAR DISAGREE CAS MESSAGE. AIRCRAFT DIVERTED TO DAY AND LANDED WITHOUT INCIDENT. MAINTENANCE TROUBLESHOT, REMOVED AND REPLACED DOWNLOCK PROXIMITY SENSOR PER THE AIRCRAFT MAINTENANCE MANUAL. AIRCRAFT WAS RELEASED FOR SERVICE.
CREW REPORTED THE STANDBY ATTITUDE INDICATOR FAILED IN FLIGHT. AIRCRAFT DIVERTED TO CLT AND LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE STANDBY ATTITUDE INDICATOR. CHECKS WERE PERFORMED PER THE AIRCRAFT MAINTENANCE MANUAL WITH NO DEFECTS. AIRCRAFT WAS RELEASED FOR SERVICE.
F/O SIDE WINDOW OUTER PLY SHATTERED AT 11,000 FEET. MAINTENANCE REMOVED AND REPLACED THE WINDOW PER THE AIRCRAFT MAINTENANCE MANUAL. AIRCRAFT WAS RELEASED FOR SERVICE.
CREW REPORTED THE RT WINDSHIELD SHATTERED AT 19,000 FT. ACFT LANDED WITHOUT INCIDENT. MX REMOVED AND REPLACED THE WINDSHIELD IAW THE ACFT MM. ACFT WAS RETURNED TO SERVICE.
CREW REPORTED THE LT WINDSHIELD CRACKED. ACFT RETURNED TO DEPARTURE AND LANDED WITHOUT INCIDENT. MX REMOVED AND REPLACED THE WINDSHIELD IAW THE ACFT MM. ACFT WAS RETURNED TO SERVICE.
CREW REPORTED A GEAR DISAGREE WARNING MESSAGE AFTER TAKEOFF. CREW ELECTED TO RETURN TO DAY AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED AND FOUND THE NOSE GEAR DOOR SWITCH NOT IN FLT/NORMAL POSITION. SWITCH WAS REPOSITIONED AND SYSTEM TESTED. NO OTHER DEFECTS NOTED. ACFT WAS RELEASED FOR SERVICE. (S)

MAINTENANCE REPAIRED ALL DAMAGE PER REO 601R-53-00-115 AND IAW THE ACFT MAINTENANCE MANUAL. ACFT WAS RELEASED FOR SERVICE. (S)
CREW REPORTED THE LANDING GEAR DID NOT EXTEND WITH THE LANDING GEAR HANDLE. CREW MANUALLY EXTENDED THE GEAR. THE CREW DECLARED AN EMERGENCY AND LANDED IN CAK WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE LANDING GEAR CONTROL HANDLE IN ACCORDANCE WITH THE ACFT MAINTENANCE MANUAL. OPERATIONAL CHECKS WERE GOOD AT THIS TIME. ACFT WAS RELEASED FOR SERVICE. (S)
CREW REPORTED A STRONG CHEMICAL ODOR COMING FROM VENTILATION SYSTEM. CREW ELECTED TO RETURN TO DAY AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED AND FOUND THE RIGHT ENGINE WASH FROM THE PREVIOUS NIGHTS MAINTENANCE CAUSING THE SMELL. MAINTENANCE PERFORMED ENGINE GROUND RUNS TO DISSIPATE THE SOLUTION IN ACCORDANCE WITH THE AIRCRAFT MAINTENANCE MANUAL. NO CHEMICAL SMELLS AFTER GROUND RUNS AT THIS TIME. AIRCRAFT WAS RELEASED FOR SERVICE.
CREW REPORTED A LOSS OF DIRECTIONAL CONTROL ON TAKEOFF. THE ACFT PULLED LEFT AND WAS UNCONTROLLABLE WITH THE RUDDER. CREW ELECTED TO ABORT THE TAKEOFF AND RETURNED TO THE GATE WITHOUT INCIDENT. MAINTENANCE INSPECTED THE NOSE LANDING GEAR AND FOUND NO DEFECTS. MAINTENANCE ALSO CHECKED THE RIGGING AND FOUND TO BE WITHIN LIMITS AT THIS TIME IN ACCORANCE WITH THE ACFT MAINTENANCE MANUAL. MAINTENANCE AND THE CREW COMPLIED WITH A HIGH SPEED TAXI CHECK AND RESULTS WERE SATISFACTORY. ACFT WAS RELEASED FOR SERVICE. (S)

MAINTENANCE INSPECTED AND DEFERRED THE RIGHT HAND THRUST REVERSER PER MEL 78-30-1-1. MAINTENANCE REMOVED AND REPLACED THE POWER DRIVE UNIT IN ACCORDANCE WITH THE AIRCRAFT MAINTENANCE MANUAL. OPS CHECKS WERE GOOD. AIRCRAFT WAS RELEASED FOR SERVICE.
CREW REPORTED AT AROUND 100 KTS ON TAKEOFF ROLL THAT THE RIGHT NOSE GEAR TIRE SHREDDED AND THE LEFT TIRE WENT FLAT. CREW ABORTED TAKEOFF AND RETURNED TO THE GATE WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED BOTH NOSE TIRES IN ACCORDANCE WITH THE AIRCRAFT MAINTENANCE MANUAL 32-42-01-400-801. MAINTENANCE PERFORMED A STRUCTURAL INSPECTION IN ACCORDANCE WITH THE AIRCRAFT MAINTENANCE MANUAL 05-21-00-220-801. MAINTENANCE ALSO PERFORMED AN INSPECTION CHECK FOR TIRE BURST DAMAGE IN ACCORDANCE WITH THE AIRCRAFT MAINTNENACE MANUAL 05-51-29-210-801. AIRCRAFT WAS RELEASED FOR SERVICE.
THE CREW REPORTED A LIGHTNING STRIKE DURING DESCENT INTO CLT. THE FLIGHT LANDED WITHOUT INCIDENT. MAINTENANCE PERFORMED EN ROUTE AND DETAILED LIGHTNING STRIKE INSPECTIONS. DAMAGE WAS FOUND ON THE LT HORIZONTAL STABILIZER TIP FAIRING AND TWO STATIC WICKS. ALL DAMAGE WAS REPAIRED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
DURING CLIMBOUT, WITH THE LT PACK MEL'D, THE RT PACK HIGH PRESS MESSAGE ILLUMINATED. THE CREW FOLLOWED THE QRH AND RETURNED TO DCA. MAINTENANCE OPERATIONALLY CHECKED THE RT PACK IAW CRJ200 AMM 21-51-00-710-801 AND WAS UNABLE TO DUPLICATE THE DISCREPANCY. NO DEFECTS WERE FOUND. ALL OPS CHECKS WERE GOOD. THE AIRCRAFT WAS RELEASED FOR SERVICE.
DURING CRUISE THE STAB TRIM CAUTION MESSAGE ILLUMINATED. THE CREW SUCCESSFULLY RE-ENGAGED THE SYSTEM AND CONTINUED TO DTW. MAINTENANCE REMOVED & REPLACED THE CAPTAIN'S PITCH TRIM DISCONNECT SWITCH IAW CRJ200 AMM 27-41-30-000(400)-802. OPERATIONAL CHECKS WERE GOOD. A SATISFACTORY OPERATIONAL CHECK FLIGHT WAS PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
