PUSHED PROPS/POWER UP, LEFT ENGINE REMINED ROLLED BACK (11%) N1. SHUTDOWN ENGINE CUTOFF, FEATHERED, FIREWALL, RAQN THROUGH QRH FIRE/FAILURE ABOVE 1500. NO FOD/FIRE/OIL LEAK, RAN QRH FOR AIR START RESTARTED NORMAL RAN WITH AND WITHOUT STANDBY PUPMP FOR REST OF FLIGHT. LANDED WITH L STANBY PUMP ON AS PRECAUTIN, TEMPS PRESSURES NORMAL.
MAINTENANCE REPORTED THAT THE AIRSTAIR DOOR LOWER FORWARD HANDRAIL CABLE WAS FRAYED. REMOVED AND REPLACED THE HANDRAIL CABLE IAW BMM 52-10-02.
PILOTS REPORTED THAT THERE WAS A LINGERING, FOUL, MOLDY STENCH IN THE COCKPIT AND CABIN. THOUGHT TO BE COMING FROM THE BLOWER VENTS. THE ODOR WAS NOTED AS VERY NAUSEATING AND FATIGUING FOR THE CREW TO FLY IN. MAINTENANCE CLEANED AREAS FORWARD AND AFT OF THE MAIN SPAR UNDER FLOOR. REMOVED BROWN SMELLY SUBSTANCE WITH ALCOHOL AND A SCRUBBER PAD. APPLIED CPC TO CLEANED AREAS.
MAINTENANCE DISCOVERED THAT THE PASSENGER DOOR AFT CABLES WERE LOOSE AND REQUIRE ADJUSTMENT. TIGHTENED PASSENGER DOOR AFT CABLE IAW AMM 52-10-02.
FOUND THE LT INBD LE INSIDE SKIN HAS SEVERAL AREAS OF CHAFING FROM THE CONTROL CABLES RUBBING & HOLES, A .5" SHALLOW HOLE & CHAFE MARKS, A .375", .125" HOLES IN SKIN ONLY, SHALLOW HOLE, SMALL .25" WEAR HOLE, .75" TEAR IN THE EDGE OF THE SKIN & A CRACK, .25" IN LENGTH. REPAIRED IAW EO 15-1900-57-002.

DURING A SCHEDULED INSPECTION, FOUND A CRACK IN THE FRAME AT FS 107 BY STRINGER 8L, PROTRUDING OUT OF RIVET ON EITHER SIDE, .25" IN LENGTH. REPAIRED IAW EO 15-1900-53-007. MAJOR REPAIR.
MAJOR REPAIR, DURING A SCHEDULED INSPECTION FOUND A CRACK IN THE FRAME AT FS 509.5, AFT SIDE OF CARGO DOOR FRAME AT MID-CARGO FLOOR LEVEL, .7" IN LENGTH. REPAIRED THE AFFECTED AREA. PERFORMED ALL REQUIRED RII INSPECTIONS IAW EO 15-1900-53-006.
PILOT REPORTED THAT ON CLIMB OUT AFX DISABLE LIGHT ILLUMINATED, N1 ABOVE 91% ON THE RT SIDE. REPLACED PCB A123, PN 129-364091-9, SN ON 993-F-94, SN OFF 799-C-93 IAW MI 34-01, SYS OPERATIONS CHECK GOOD.
MAJOR REPAIR DURING A SCHEDULED INSPECTION FOUND THE LT WING SPAR AFT CAP AT WS 12325 FASTENER HOLE MISSED DRILLED THROUGH THE WING SKIN & WING SPAR CAP. REPAIRED THE AFFECTED AREA & PERFORMED ALL REQUIRED RII INSPECTIONS IAW EO 14-1900-57-003. MATERIALS UTILIZED ARE INCLUDED IN THE ENGINEERING ORDER.

DURING A SCHEDULED ROUTINE INSPECTION FOUND A CRACK IN THE FRAME AT FS 164 AND STRINGER 10R, 0.375" IN LENGTH. REPAIRED IAW EO 13-1900-53-028. MAJOR REPAIR.

PILOT REPORTED THAT ON LANDING, LOWERED THE GEAR, INTRANSIT LIGHT CAME ON AND WENT OUT, GOT GREEN ON MAINS, NOT ON NOSE, MANUALLY EXTENDED GEAR AND STILL NO GREEN ON NOSE. MX REPLACED THE NOSE UPLOCK SWITCH, OPS CK GOOD.
DURING A SCHEDULED ROUTINE INSPECTION, FOUND 2 CRACKS ON A LT ELEVATOR ASSY IN THE T/E, APPROX 23.1875" OTBD OF TRIM TAB OTBD CUTOUT, ON BOTH SIDES OF THE ELEVATOR SKIN, (TOP AND BOTTOM SIDES), 1" IN LENGTH. REPAIRED IAW EO 13-1900-27-001. MAJOR REPAIR.
DURING A SCHEDULED ROUTINE INSPECTION, FOUND A CRACK IN THE LT FWD SPAR CAP ANGLE AT THE VERT STAB AT VSS 84.10 AND THE 20 PERCENT AT THE SECOND FASTENER, CRACK RADIATES DOWNWARD FROM FASTENER, .375" IN LENGTH. REPAIRED IAW FR-CO-02530, REV 1, STEP 1. MAJOR REPAIR.
DURING A SCHEDULED ROUTINE INSPECTION, FOUND 5 TO 7 CRACKS RADIATING FROM THE CENTER LIGHTNING HOLE ON THE STAB REAR SPAR CENTER WEB, 2.5 INCHES IN LENGTH. REPAIRED THE AFFECTED AREA AND PERFORMED ALL REQUIRED RII INSPECTIONS IAW EO 12-1900-55-009. MATERIALS UTILIZED ARE INCLUDED IN THE EO. MAJOR REPAIR

DURING A SCHEDULED ROUTINE INSPECTION FOUND THE AFT FUSELAGE FRAME AT FS 456 CRACKED AT THE FWD GAS STRUT MOUNT UPPER INBD NUT PLATE, BETWEEN STRINGER 4L AND 5L, 2 CRACKS 1" AND .2" IN LENGTH. MX REPAIRED THE AFFECTED AREA AND PERFORMED ALL REQUIRED RII INSPECTIONS IAW EO 11-1900-53-067. MATERIALS UTILIZED ARE INCLUDED IN THE EO. MAJOR REPAIR.
ALL THE INSPECTION LIGHTS IN THE AIRSTAIR DOOR SYS ARE INOPERATIVE. MX FOUND A BROKEN TERMINAL WIRE ON THE DOOR, MX REPLACED THE TERMINAL AND OPS CHECKED GOOD.
PILOT REPORTED THAT AT 80 KNOTS IAS THE STALL WARNING HORN SOUNDED AND THE TAKEOFF WAS ABORTED. MX TROUBLESHOT THE SYS, COULD NOT DUPLICATE, CHECKED CONNECTORS ON THE SWITCH HARNESSES, ATTACHED A BREAKOUT BOX, NO DEFECTS FOUND, RESECURED SQUAT SWITCH HARNESS, IAW BMM 27-34-00, SYS OK FOR SERVICE.
PILOT REPORTED THAT THE AIRSTAIR CABLE HOUSING IS RIPPED, UPPER AFT. MX REPLACED THE UPPER AFT CABLE IAW BMM 52-10-02, PN: 1440-10GLA.
PILOT REPORTED THAT ON DECENT PASSENGER OXYGEN MASKS DEPLAYED INADVERTENTLY. MAINTENANCE REPACKED ALL PAX O2 MASKS IN CONTAINERS IAW BMM 35-20-01.
EMERGENCY LIGHT BULB DID NOT TEST. MAINTENANCE REPLACED THE BULB PN 1864, REF MM 31-10-00, OPS CHECK GOOD.
MAJOR REPAIR - DURING A SCHEDULED ROUNTINE INSP, A DENT ON RT SIDE OF FUSELAGE AT FS 531 BETWEEN STRINGER 11R AND 10R WAS FOUND. MAINTENANCE REPAIRED THE AFFECTED AREA AND PERFORMED ALL REQUIRED RII INSPECTIONS IAW EO 10-1900-53-049. MATERIALS UTILIZED ARE INCLUDED IN THE ENGINEERING ORDER.
ABORTED TAKEOFF- AIRCRAFT 247GL DID AN ABORTED TAKEOFF IN HVR FLT 5186 FOR RT BLEED AIR FAIL ANNUNCIATOR ILLUMINATING. CREW RETURNED TO GATE AND CALLED MX. THE CREW AND MX AGREED THAT IT COULD BE MEL'D UNDER 21-3-2 CAT B. THE CREW BEFORE DOING THE MEL RAN THE AIRCRAFT FOR THE "O" PROCEDURE OF THE MEL AND CONFIRMED AIRCRAFT WAS IN OPERATING ORDER TO DO THE MEL. RT BLEED WAS MEL'D. PILOTS REPORTED THAT THEY OBSERVED A RIGHT HAND BLEED AIR FAIL ANNUNCIATOR ILLUMINATED ON TAKEOFF. PILOTS PERFORMED MEMORY ITEMS AND QRH ITEMS AND THE LIGHT REMAINED ILLUMINATED. MAINTENANCE ORIGINALLY PLACED THE ITEM ON MEL 21-3-2 CATEGORY B. MAINTENANCE ULTIMATELY REPAIRED THE RIGHT HAND LEADING EDGE EVA TUBING IAW 26-11-00.
ACFT WOULD NOT PRESSURIZE DURING CLIMB OUT. MX SECURED THE RT OUTFLOW VALVE THAT WAS FOUND LOOSE IAW MM 21-30-01-201. AN OPS TEST FLIGHT WAS CONDUCTED AND FOUND TO BE SATISFACTORY.
AT 400 FT, TURNED BLEEDS ON AN LT ENVIRO FAIL ANNUN ILLUMINATED, TRIED TO RESET SEVERAL TIMES IAW THE CHECKLIST, DID NOT RESET, RETURNED TO DEPARTURE AND TRANSFERED THE ISSUE TO DML NR 35433 PER MEL 21-14 CAT C ALL O PROCEDURES COMPLIED WITH. MX REMOVED AND REPLACED THE LT PRSOV PN 114-380059-5 SN ON 0154B OFF 0456B IAW BMM 21-11-03 OPS CHECKS WERE GOOD. MX CLEARED THE DML AND THE PLACARD WAS REMOVED.
DURING SCHEDULED STRUCTURAL INSP MX DISCOVERED CRACKS AROUND THE CARGO DOOR UPPER FRAME AT FWD CORNER AREA AT FS 456 AND UPPER FRAME AFT CORNER AT FS 509. APPROXIMATE CRACK LENGTHS 5 INCHES AND 2.75 INCHES RESPECTIVELY. MX REPAIRED THE AFFECTED AREAS IAW EO-09-1900-53-002. MATERIALS UTILIZED INCLUDE PN'S 129-440043-1, MS20470AD5, AC240 B1/2 SEALANT, 9309-3NA, NAS1097-4,NAS1097-5, CR3212-5, CR3212-4,CR3214-4, MS20470D-5 AND NAS1330C3KB116.
MAJOR REPAIR - SKIN CRACKS ON TOP OF FUSELAGE AT F.S. 150.6 AT STRINGER 1 WERE FOUND DURING SCHEDULED STRUCTURAL CHECK. RIVET HEAD WAS SHEARED WITH CRACKS STARTING AT SAID RIVET AND ANOTHER CRACK AT A RIVET SAME LOCATION OTHER SIDE OF STRINGER 1. MAINTENANCE REPAIRED THE AFFECTED AREA IAW EO-09-1900-53-001. RII INSPECTION COMPLIED WITH PER TASKCARD 4.3.7.
SIGNIFICANT EVENT - ENROUTE FROM PGA TO FMN THE PILOTS REPORTED THAT THE GEAR RELAY CIRCUIT BREAKER POPPED TWICE AND THEY HAD TO MANUALLY EXTEND THE GEAR. MX REMOVED AND REPLACED THE LANDING GEAR RELAY IAW MFG MX 32-30. OPS CHECKS WERE GOOD.
EES - FLIGHT 5041 (DEN) PILOTS REPORTED THE MAIN CABIN DOOR COULD NOT BE OPENED FROM THE INSIDE. MAINTENANCE CLEANED THE PUSH BUTTON SWITCH AND OPERATIONALLY CHECKED THE DOOR. OPS GOOD IAW MM 52-10-00.
STRINGER 10R CRACKED AT FS 451 IAW CRACK IS ABOUT .5 IN LONG AT STRINGER RELIEF CUTOUT. CRACKED AT STRINGER FLANGE. REPAIRED STRINGER FLANGE AT FS 451 IAW. 1900D SRM 51-70. REPAIR DIMENSIONS APPROXIMATELY 9.375 IN BY 1 IN. 2024 T-3 .050. RII COMPLIED WITH IAW TASKCARD 8.2.1.
FRAME AT FS 451.0 CRACKED AT STRINGER 10R RELIEF CUTOUT CRACK IS .6250 IN LONG. MAINTENANCE REPAIRED FRAME FLANGE AT FS 451.0 IAW 1900D SRM 51-70. REPAIR APPROXIMATELY 6 INCH BY 2 INCH 2024 T3 .063. RII COMPLIED WITH IAW TASKCARD 8.2.1.
LT HEAVY ANGLE CRACKED AT FS 288. CRACK LENGTH APPROXIMATELY 1.75 INCH. MAINTENANCE REPAIRED LT HEAVY ANGLE AT FS 288 IAW 1900D SRM 53-90. REPAIR DIMENSIONS APPROXIMATELY 13.5 INCHES. RII COMPLIED WITH IAW TASKCARD 8.2.1.
HOLE IN FUSELAGE ON THE RT SIDE FORWARD OF FS 423.25 AND 2 INCHES ABOVE STRINGER 12R. APPROXIMATELY 1 INCH V SHAPE HOLE/CUT. MAINTENANCE REPAIRED AFFECTED AREA WITH A FLUSH REPAIR IAW MFG SRM 51-70. RII COMPLIED WITH IAW TASKCARD 8.2.1. HOLE CUTOUT SIZE 1.3 INCH X 1.3 INCH.
FLIGHT 5128, ENROUTE, PILOTS REPORTED THAT WHILE INFLIGHT MAIN GEAR EXTENDED AND THE LANDING GEAR RELAY CIRCUIT BREAKER POPPED. CREW MANUALLY EXTENDED GEAR TO ACHIEVE A (3) GREEN CONFIGURATION. MAINTENANCE REMOVED AND REPLACED A FLEXIBLE HYDRAULIC LINE IN THE RT GEAR AREA AND PERFORMED AN OPERATIONAL CHECK IAW MM 32-30-10. OPERATIONAL AND LEAK CHECK GOOD.
LEFT WINDSHEILD OUTER PLY SHATTERED. REPLACED WINDSHEILD.
DURING ROUTINE STRUCTURAL INSPECTION, ALL THREE EMERGENCY EXIT WINDOW PANS WERE FOUND TO HAVE CRACKS AROUND THE RADIUS. WINDOW PANS WERE REPLACED, AIRCRAFT RETURNED TO SERVICE.
