RING FRAME CHAFING DAMAGE AT APPROXIMATELY STA 4918, WL 1759, AND BL 320L MEASURING 0.92â LONG X 0.08â WIDE AND IS 0.008â AT ITS DEEPEST
DENT AND CHAFING WAS DISCOVERED ON THE ENGINE DECK (AIRBUS PN 350A21-1239.21) JUST NEAR THE SCUPPER DRAIN HOLE. THE DENT WAS MEASURED TO BE 0.20â X 0.10â AND 0.040â DEEP, AND THE CHAFE WAS MEASURED TO BE 1.0â X 0.40â AND 0.005â DEEP
AT 8 MINUTES IN TO FLIGHT FLI FAILED WITH ELECTRICAL SMELL IN COCKPIT. ON 6/30/2021 I WAS CALLED OUT FOR A VEMD FLI FAILURE. UPON ARRIVAL I INSPECTED PANEL AND ENGINE HARNESS FOR SIGNS OF OVERHEATING AND VISUAL CHECK WITH ELECTRICAL POWER APPLIED TO SEE IF ANY DEFECTS COULD NE NOTED. AFTER INSPECTION WITH NO DEFECTS WE PERFORMED A GROUND RUN WHERE NG DIGITAL READOUT WAS PERFORMING TO MANUFACTURERS SPECS. (DURING OPERATION BELOW 90% NO NEEDLE MOVEMENT. AFTER 90% NEEDLE WOULD START TO MOVE) AFTER GETTING AIRCRAFT RTS FROM MCAS, PERFORMED A 15 MINUTE CHECK FLIGHT WITH NO DEFECTS NOTED.
15 MINS INTO FLIGHT, CREW NOTIFIED THEY SMELLED BURNING ELECTRICAL IN THE CABIN, LANDED WITH NO OTHER ISSUES. MECHANIC AND PILOT RAN UP AIRCRAFT CHECKED SYSTEMS AND COULD NOT DUPLICATE. AIRCRAFT WAS VISUALLY CHECKED AND NO DISCREPANCIES WE NOTED.
A DENT WAS FOUND ON THE RT SIDE OF TAILBOOM JUST ABOVE HORIZONTAL STAB APPROXIMATELY 5 INCHES IN LENGTH.
PILOT BEGAN EXPLAINING THE PROBLEM HE ENCOUNTERED. REMOVING THE PANELS FOR FURTHER INSPECTION OF THE PEDAL ASSEMBLIES. THE PILOT AND MYSELF NOTICED THE CLEAR CIRCUIT BREAKER COVER WASN'T SECURED IN PLACE AND WAS INTERFERING WITH THE PEDAL TRAVEL. IT WAS DETERMINED AFTER FURTHER INSPECTION OF THE AIRCRAFT THAT THE COVER PLATE NOT BEING SECURE WAS THE ISSUE. REINSTALLED COVER PLATE AND INSTALLED TWO MISSING PUSH FASTENERS, OPS CHECKED MOVEMENT OF PEDALS.
DURING PRESTART CHECK, FIRE LIGHT WAS ILLUMINATED. PRESTART CHECKLIST WAS ABORTED AND MX WAS CALLED. MX TROUBLESHOT THE SWITCH AND FOUND NO DISCREPANCIES. THEY THEN REPLACED ALL 3 FIRE DETECTORS. THE FIRE LIGHT WENT OUT. NO FURTHER ISSUES OR DISCREPANCIES WERE NOTED.
