IN THE COURSE OF A PRACTICE AUTOROTATION, THE ENGINE FLAMED OUT WHEN THE THROTTLE WAS ROLLED TO THE IDLE DETENT POSITION.
GOVERNOR ALLOWS ENGINE TO OVERSPEED. FAILED SHOP TEST. DIASSEMBLED AND FOUND UNIT IMPROPERLY ASSEMBLED. SPOOL BUSHING ANTI-ROTATION TANG WAS ENGAGED IN LEVER SLOT.
IN CRUISE FLIGHT THE PILOT REPORTED HIGH FREQUENCY VIBRATION AND WHINING NOISE FROM THE ENGINE COMPARTMENT. THIS WAS FOLLOWED BY AN ENGINE CHIP LIGHT INDICATION AND NOISE AT WHICH POINT THE ENGINE LOST POWER AND PROCEDED TO SHUT DOWN.
CRACKED AFT UPPER LT LONGERON AT TAILBOOM ATTACH POINT.
JUST AFTER TAKEOFF, THE LOW ROTOR CAUTION LIGHT ILLUMINATED. 2 OR 3 SECONDS LATER, THE LOW ROTOR RPM HORN SOUNDED. ROTOR TACH INDICATIONS WERE NORMAL, PILOT RETURNED TO BASE. UPON LANDING, THE MECHANIC REMOVED AND REPLACED BOTH THE LOW ROTOR RPM AND ENGINE OUT SENSORS DUE TO CORROSION AND AC WAS OPS CHECKED AND RETURNED TO SERVICE. (K)

FOUND MAIN ROTOR TRAILING EDGE EXTRUSION EXFOLIATED APPROXIMATELY 2.5.
DURING CRUISE FLIGHT FROM OFFSHORE, PILOT NOTICED N2 DROPPING. LANDING WAS MADE - NO DAMAGE TO AIRCRAFT. DURING TROUBLESHOOTING, FOUND PC LINE FROM ACCUMULATOR TO FUEL CONTROL CRACKED AT FLARE ON POWER CONTROL SIDE CAUSING ENGINE TO SPOOL DOWN. REPLACED LINE AND TEST FLEW AIRCRAFT. OPS CHECK GOOD. TROUBLESHOOTING DID SHOW .9 VIB IN ENG AREA - FOUND AFT ADAPTER ON DRIVESHAFT FROM ENGINE TO OIL COOLER BLOWER WORN. REPLACED ADAPTER AND VIB LEVEL DROPPED TO .02. CHECK OF ADAPTER SHOWS PER MANUAL, IS STILL WITHIN LIMITS. SUBMITTER STATED THEY ARE INSPECTING FLEET FOR QUESTIONABLE LINES AND ADAPTERS.
UPON TEAR-DOWN OF TRANSMISSION FOR INSPECTION, FOUND SCREEN ASSY FULL ON PLASTIC OR NYLON PIECES. INSPECTED PLANETARY ASSY FOUND 3 EACH GEAR ASSYS WITH NO RETAINERS REMAINING, ONE EACH RETAINER WAS IN PLACE. ONCE REMOVED, IT FELL APART. UPGRADE RETAINERS TO NEW P/N 214-040-515-001.
INSTALLED OVERHAULED T/R HUB AND BLADES ON AIRCRAFT. NOTICED AFTER RIGGING T/R HORNS, P/N 206-011-809-107, THEY WERE CONTACTING T/R HUB, P/N 206-011-810-139. REMOVED AND REPLACED ASSY. INSPECTION OF REMOVED UNITS AND OF UNITS IN STOCK SHOWED THAT MACHINING OF HUBS AND HORNS IS NOT CONSISTANT. SUGGEST TRAIL AND ERROR METHOD OF MATCHING UP HORNS TO HUBS OR CHANGE TO USING 206-011-809-109 HORNS.
EXCESSIVE PLAY AT TWO PLACES. SEE BELL MM L-3 FIG 62-21. ON TEAR-DOWN, FOUND BEARINGS AND SPACERS WORN BEYOND LIMITS. TOTAL TIME OF PARTS LESS THAN 1,000 HOURS/1 YEAR SINCE REPLACEMENT.
THIS IS NR 6 AND NR 7 DUPLEX BEARING CHANGE FOR BRINELLING ON RACES PARTICULARLY THE LOWER INNER RACE. ALL HAVE BEEN CHANGED WITH LESS THAN 100-HOUR TT SINCE NEW.
T/R BLADES REMOVED DUE TO FEATHERING BEARINGS WORN AND EXCESSIVE AXIAL MOVEMENT OF THE BLADES. INSPECTED BLADE FEATHERING BEARINGS PER BELL 206L MM, CH 65-30-00, PG 65-49 PARA 65-62 ITEM 6 AXIAL LOOSENESS EXCEEDED .190 INCHES AT BLADE TIP. BEARINGS REPLACED. TOTAL TIME OF OLD BEARINGS 537.7.
T/R BLADES REMOVED DUE TO FEATHERING BEARINGS WORN AND EXCESSIVE AXIAL MOVEMENT OF THE BLADES. INSPECTED BLADE FEATHERING BEARINGS PER BELL 206L MM, CH 65-30-00, PG 65-49 PARA 65-62 ITEM 6 AXIAL LOOSENESS EXCEEDED .190 INCHES AT BLADE TIP. BEARINGS REPLACED. TOTAL TIME OF OLD BEARINGS 537.7.
ON DAILY AIRWORTHY INSPECTION, FOUND STROBE LIGHT BULB BURNED OUT. HAD REPLACED BULB LAST WEEK. CAUSE OF PROBLEM UNKNOWN AT THIS TIME.
DURING ROUTINE MAINTENANCE, FOUND TAIL ROTOR BLADE INBOARD BEARING CRACKED ON BLADE, SN 452, AND SMALL INDICATION ON INBOARD BEARING, SN 446.
