LEFT FDC AIR FILTER BYPASS DOOR WILL NOT OPEN. UPON FURTHER INVESTIGATION, DISCOVERED LT FDC BYPASS DOOR ACTUATOR ATTACHMENT POINT WORN THROUGH. REMOVED & REPLACED WITH A SERVICEABLE UNIT . WORK PERFORMED . OPS CHECKED GOOD.
PILOT REPORTED FLICKERS AND INTERMITTENT SOLID, ROTOR RPM LIGHT ON CWP. PILOT VERIFIED NR ON TRIPLE TACH AND VEMD FOR N2 MATCHED AND WERE STABLE. ABORTED CALL & RETURNED TO BASE TO INITIATE DISCREPANCY, CONTACT MECHANIC, MCAS. UPON ARRIVAL, INSPECTED MGB COMPARTMENT COMPONENTS OF SYSTEM, SAW NO OBVIOUS DEFECTS. ALL HEAT SHRINK WERE CLEAN & DRY IN APPEARANCE. CUT AWAY HEAT SHRINK AT NEW CONNECTION PROVIDED BY MEANS OF SB-EC-135-63-008-2C1; DISCOVERED IMPROPERLY CRIMPED CONNECTIONS BY APPLYING LIGHT TENSION TO CONNECTORS. REMOVED FAULTY CRIMPS AND INSTALLED NEW CRIMPED CONTACTS IAW SB LISTED ABOVE. RESTORED ACFT COWLINGS TO FLIGHT CONFIGURATION AND PERFORMED MAINTENANCE CHECK FLIGHT, NO DEFECTS NOTED.
PILOT REPORTED, HOPPING VIBRATION, IN-FLIGHT WHILE EN-ROUTE TO HOSPITAL. DURING POST FLIGHT CHECK, DISCOVERED THE RED MAIN ROTOR BLADE PITCH LINK UPPER ROD END TO HAVE LATERAL PLAY.
HIGH ENG OIL TEMP ON BOTH ENGS. TROUBLE SHOT BY INSPECTION OF OIL COOLERS, BYPASS VLVS & ENG OIL INDICATING TEMP PROBES. REMOVED OIL COOLERS FOR CLEANING PROCEDURE IAW AMM 79-22-00. WHEN THE BLOWER BAN WAS REMOVED FROM NR 2 SIDE IT WAS DISCOVERED THE BLOWER SHAFT WAS SHEARED. REMOVED & INSPECTED NR 1 BLOWER FAN & DISCOVERED THIS SHAFT HAD SHEARED AS WELL. INSPECTED TRANSMISSION OIL FILTER FOR DEBRIS, NO DEFECTS NOTED. INSPECTED CHIP DETECTOR, NO DEFECTS NOTED. FAN PNS LT L792M3005105 2143+38 TIS RT L972M3004105 2143+38 TIS.
APPROXIMATELY 10 MINUTES INTO FLIGHT ENROUTE, NO PATIENT ON BOARD, THE NR 2 GEN OVHT CAUTION LIGHT ILLUMINATED. IMMEDIATELY RETURNED TO THERMAL BASE. EXECUTED EMER CHECKLIST & SHUTDOWN THE NR 2 GEN. LANDED AT BASE UNEVENTFUL. AFTER VISUAL INSPECTION OF THE GEN THERE WAS NO EVIDENCE OF ANY DAMAGE. UPON VISUAL INSPECTION, FOUND DETERIORATED CONNECTOR AT TERMINAL D, REPLACED AS REQUIRED. SWAPPED NR 1 & NR 2 ENG GEN OVHT SWITCHES FOR TROUBLESHOOTING & PERFORMED MX CHECK FLIGHT. AT APPROX 40 MINS INTO MX FLIGHT, ENG NR 1 GEN OVHT CAUTION ILLUMINATED & PROPER GENERATOR SHUTDOWN & LOAD SHEDDING PROCEDURES WERE PERFORMED BY PILOT. NEW GEN SWITCH ORDERED AOG & REPLACED AS REQUIRED. GND RUN PERFORMED, NO DEFECTS NOTED.
