SMOKE IN COCKPIT / NWS / PITCH TRIM. CLEANED BOTH WATERSOCKS FOR SMOKE IN THE COCKPIT, ALL PANELS HAVE BEEN CLOSED, IAW SA227 MM 21-70-10.
SMOKE IN COCKPIT FROM BLEEDS. TEMP CONTROLLER WAS IN AUTO AND MOSTLY TOWARDS HOT. CLEANED L & R WATERSOCKS GRD RUN'S OPS CHECK GOOD NO SMOKE IN CABIN IAW MM 21-40-00 RTS.
NO E1 TORQUE READING, TAKE OFF ABORTED. CLEANED CANNON PLUG AT TORQUE RING. PERFORMED ENGINE GROUND RUNS PER SA227 MM CH 71-00-30 GROUND OPS CHECK GOOD.
E2 WILL NOT START, NO FUEL FLOW TO ENGINE, RIGHT FUEL FLOW INDICATOR SHOWS NO FUEL FLOW.......MX REMOVED AND REPLACED E-2 FUEL SHUT OFF VALVE WITH OVERHAULED IAW GARRETT TPA321-11 MAINTENANCE CH 73-10-06. MX ALSO INSTALLED OVERHAULED INDICATOR AND OPS CHECK NORMAL. PERFORMED ENGINE GROUND RUNS AND OPS CHECKS, LEAK CHECK GOOD.
FLIGHT TOOK OFF FROM DEN AND THE CREW REPORTED THE GEAR WOULDN'T RETRACT. CREW RETURNED TO DEN AND FOUND HYDRAULIC LEAK IN RT NACELLE. MX HAD REPLACED A FAILED LINE PRIOR TO THE FLIGHT. THE NEW LINE WAS FOUND TO BE LEAKING DUE TO A FITTING WHICH REQUIRED TIGHTENING.
PILOT REPORTED RH BRAKE LOSES PRESSURE DURING TAXI. MX BLED BRAKES.
FLIGHT CREW REPORTED THAT THE NOSE GEAR UNSAFE LIGHT REMAINED ILLUMINATED WITH THE NOSE GEAR RETRACTED. MX ADJUSTED ACTUATOR LINKAGE, CLEANED AND LUBED ALL SWITCHES, THE PROBLEM REOCURRED. THE ROOT DEFECT WAS FOUND TO BE FAULTY WIRING BETWEEN THE UNSAFE INDICATOR AND A TERMINAL STRIP FORWARD OF THE INSTRUMENT PANEL. THE DEFECTIVE WIRING WAS REPLACED.
FLIGHT CREW REPORTED NOSE GEAR FAILED TO RETRACT AFTER TAKEOFF. MX FOUND SHIMMED THE NLG STRUT TO ENSURE PROPER CLEARENCE OF THE RT ACTUATOR TO SIDE OF WHEEL WELL.
E2 SUFFERED UNCONTAINED ENG FAILURE ON CLIMB OUT. 2ND STAGE TURBINE DISK FAILED & WAS THROWN FROM THE ENG THROUGH THE LT & RT SIDES OF THE ENG. APPROX 1/4 OF THE 2ND STAGE TURBINE DISK PENETRATED THE CABIN & WAS FOUND ON THE CARGO FLOOR.
PILOT REPORTED ACFT NOT PRESSURIZING IN FLIGHT. THE LOWER FWD DOOR CORNER FRAME WAS FOUND TO BE CRACKED ALONG FWD END OF DOOR HINGE ATTACH RIVETS. THIS WAS THE THIRD FAILURE OF THIS THIS TYPE FOUND IN OUR FLEET. THE DOOR FRAME CORNER AND INTERCOSTAL WERE REPLACED TO CORRECT THE PROBLEM. M7 PROVIDES A LIGHT AND HEAVY VERSION OF THE DOOR CORNERS, THE PART REMOVED WAS THE LIGHT VERSION. IT IS BELIEVED THAT THE ADDED STRESS OF THE DOOR SNUBBER ATTACHED AT THIS LOCATION TO BE THE CAUSE OF THE FAILURE.
SJU-POS - DURING WET T/O, THE RT ENG PRODUCED 80 PERCENT TQ AND THE T/O WAS ABORTED. WHEN ENG WAS RAN DRY, IT PRODUCED CHARTED TQ AND THE CAWI SYSTEM WAS DEFERRED. ON THE ENSUING DRY TAKEOFF, THE RT ENG PRODUCED 74 PERCENT TQ AND T/O WAS ABORTED. MX ADJUSTED SCREW 'X', PERFORMED ENG GROUND RUNS, RUNS WERE SATISFACTORY, AND A/C WAS RETURNED TO SERVICE. IN FLT FROM SJU TO POS, DURING CLIMB, THE RT ENG TQ DROPPED TO 98 PERCENT AND STARTED BY-PASSING. THE A/C RETURNED TO SJU AND MX ADJUSTED SCREW 'X', PERFORMED ENG GROUND RUNS, RUNS WERE SATISFACTORY. A FCF WAS FLOWN BUT WAS UNSATISFACTORY. MX DRAINED THE WATER/METHANOL, SERVICED THE SYSTEM WITH A NEW WATER/METHANOL MIXTURE, AND PERFORMED AN OP CHECK
FLIGHT 361, RDU TO CVG, 35 NAUTICAL MILES SOUTHWEST OF CVG WHILE DESCENDING OUT OF 8,000 FEET FOR 4,000 FEET, A LARGE BIRD STRUCK THE AIRCRAFT ABOVE THE CENTER WINDSHIELD. AIRCRAFT LANDED AT CVG WITHOUT FURTHER INCIDENT. AIRCRAFT FERRIED TO RDU FOR REPAIR. MAINTENANCE REPAIRED CRACKED SKIN ABOVE WINDSHIELD IN ACCORDANCE WITH THE FAIRCHILD AIRCRAFT STRUCTURAL REPAIR MANUAL 51-90-04. PRESSURIZATION CHECK WAS PERFORMED, NO DEFECTS NOTED. AIRCRAFT RETURNED TO SERVICE.
