The Cirrus pilot’s failure to maintain the final approach course for the assigned runway, which resulted in a collision with the Swearingen which was on final approach to the parallel runway. Contributing to the accident was the failure of the controller to issue a traffic advisory to the Swearingen pilot regarding the location of Cirrus, and the Cirrus pilot’s decision to fly higher than recommended approach speed which resulted in a larger turn radius and contributed to his overshoot of the final approach course.

The probable cause of the incident was the Key Lime Air flight 4216 pilot's inadvertant entry onto the active runway. A contributing factor to the incident was the failure of the Denver tower ground and local controllers to detect the aircraft on the airport movement area safety system (ASDE) display and issue a go-around instruction to the arrival flight crew.

INVESTIGATION REVEALED THAT OTHER EMB 145 OPERATORS HAVE MODIFIED THIS DOOR THROUGH THEIR ENGINEERING PROCESS TO ADD ADDITIONAL FASTENERS DUE TO LOSS OF THE DOORS DURING FLT WHEN SUCH LOSSES CANNOT BE ATTRIBUTED TO MAINT ERROR. THE DOOR AND LATCH DESIGN DO NOT APPEAR TO BE TOLERANT TO NORMAL MECHANICAL & HUMAN VARIANCE. IN PARTICULAR, THE UPPER MOST FORWARD LATCH APPEARS TO BE MORE CRITICAL THAN THE OTHER LATCHES AND IS OF A DESIGN WHICH IS NOT TOLERANT OF NORMAL WEAR.

FOLLOWED BY OUR LEFT MAIN MAKING CONTACT ALMOST SIMULTANEOUSLY AS OUR NOSE WHEEL. IT FELT AND LOOKED AS IF OUR LEFT MAIN GEAR FAILED UPON MAKING CONTACT WITH THE RUNWAY. WE IMMEDIATELY STARTED TO VEER OF THE RUNWAY TO THE LEFT, AND AFTER TALKING TO PILOT NO CONTROL INPUTS HAD ANY EFFECT ON OUR TRAJECTORY.
