THE CREW REPORTED SMOKE IN THE FLIGHT DECK DURING INITIAL CLIMB THAT SMELLED LIKE ELECTRICAL. THE SMOKE CLEARED & THEN RETURNED DURING LANDING PHASE AND THE FLIGHT DECK WAS HOT. THE AIRCAFT RETURNED AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED THE AIRCRAFT AND REMOVED AND REPLACED THE NR 1 ACM, OPERATIONALLY TESTED WITH NO DEFECTS NOTED, ANDTHE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
CREW REPORTED UPON ENTERING ICING CONDITIONS THE COCKPIT FILLED WITH SMOKE AND ACFT PERFORMED AIR RETURN BACK TO EWR. MX INSPECTED AND RAN A/C SYSTEMS NOTED NR 1 PACK ACM SEIZED. REMOVED AND REPLACED NR 1 ACM AND HEAT EXCHANGER WITH OPERATIONAL CHECKS GOOD. A/C WAS APPROVED FOR RETURN TO SERVICE.
THE CREW REPORTED A STRONG SMOKE ODOR AT THE END OF TAKEOFF ROLL AND IN FLIGHT. THE AIRCRAFT RETURNED TO DEPARTURE AND LANDED WITHOUT INDCIDENT. MAINTENANCE INSPECTED AND FOUND THE NR 1 ACM FROZEN.

CONTRACT MX CW SMOKE IN THE CABIN FIM, FOUND THAT PACK 1 FROZE UP, MOVED TO DMIL OPERATE PER MEL 21-51-00-3 CAT C, EXPIRATION DATE 11/11/1. MX REMOVED AND REPLACED NR 1 PACK AND AIR CYCLE MACHINE. A/C WAS APPROVED FOR RETURN TO SERVICE.
NR 1 ENGINE SMOKES ON SHUTDOWN AND PUDDLES OF OIL IN RTBS. REMOVED AND REPLACED THE ENGINE, OPS AND LEAK CHECKED GOOD. AIRCRAFT RELEASED AND RETURNED TO SERVICE.

DURING C-CHECK THE CARGO DOOR CUTOUT LOWER BEAM WAS FOUND TO BE CORRODED. MAINTENANCE R & R THE BEAM IAW SRM 51-40-02. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
THE CREW REPORTED BURNING ODOR IN CABIN WITH PACKS OPERATING ON APU BLEED. MAINTENANCE INSPECTED AND FOUND ODOR COMING FROM PASSENGER-CABIN SIDEWALL LIGHT FIXTURE BRACKET. THEY REPLACED THE PASSENGER CABIN LIGHT IAW 145 33-21-06. OPS CHECKS WERE GOOD.
DURING MAINTENANCE INSPECTION, THE AFT OVERWING EMERGENCY EXIT LOCATOR SIGN WAS FOUND TO BE INOPERATIVE. MAINTENANCE R & R THE NR 4 EMERGENCY LIGHT INVERTER. OPS TESTED WITH NO DEFECTS, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING MAINTENANCE INSPECTION, THE COCKPIT FLOOD EMERGENCY LIGHT, FIRST AISLE PATH CEILING EMERGENCY LIGHT AND LT FORWARD FUSELAGE EXTERNAL EMERGENCY LIGHTS WERE FOUND TO BE INOPERATIVE. MAINTENENCE R & R THE NR 1 EMERGENCY LIGHT POWER SUPPLY, OPS TESTED WITH NO DEFECTS, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.

DENT IN FUSELAGE SKIN, FRAME 80 STRINGER 5R. CUTOUT DAMAGED AREA, FABRICATED FILLER, DOUBLER AND INSTALLED.
DURING C-CHECK INSPECTION, FOUND THE LT AND RT SIDE HORIZONTAL STAB HINGE BUSHING MIGRATED, MAINTENANCE REWORKED HOUSING AND INSTALLED THE 1ST OVERSIZED BUSHIONS IAW SRM 55-10-02.
THE CREW REPORTED AT APPROX 500FT AGL STRONG ODOR OF SMOKE AND VISUAL SMOKE COMING INTO COCKPIT THROUGH PACK DUCTWORK. IMMEDIATELY TURNED OFF PACKS AND RECIRCULATING FAN. SMOKE SUBSIDED AND THE CREW PERFORMED EMERGENCY AIR RETURN AND LANDED WITHOUT FURTHER INCIDENT. MX INSPECTED AND FOUND EVIDENCE OF DEICING FLUID IN APU AIR INTAKE DUCT. CLEANED AIR INTAKE DUCT RAN ENGINES WITH PACKS AND BLEEDS ON AT VARIOUS SETTINGS NO SMOKE WAS NOTED. THE ACFT WAS RELEASED FOR SERVICE.
THE CREW REPORTED SMOKE IN COCKPIT AND CABIN. MX INSPECTED AND FOUND TO BE CAUSED BY DEICE FLUID IN BLEED DUCTS. RAN APU AND ENGINES WITH BLEEDS ON. CLEARED DEICE FLUID FROM BLEED DUCTS AND OPS CHECKS WERE GOOD.
SUPPLEMENTAL: ON FINAL APPROACH, APPROXIMATELY 1800FT CREW EXPERIENCED ENGINE 2 VIBE FOLLOWED BY ENGINE 2 LOW PRESSURE EICAS MESSAGE FOLLOWED BY SMOKE IN THE CABIN AND EMERGENCY LANDING. THE ACFT LANDED WHERE MX R & R THE NR 2 ENGINE, OPS TESTED WITH NO DEFECTS AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING ROUTING SCHEDULED MX, THE AFT LEFT WING EMERGENCY EXIT LIGHT WAS FOUND INOPERATIVE. MX R & R THE AFT LT WING EMERGENCY LIGHT BULB IAW AMM 33-50-10, OPS CHECKED SUCCESSFULLY. ACFT WAS APPROVED FOR RETURN TO SERVICE.
UNCOMMANDED STEERING ON TOUCHDOWN AIRCRAFT DEPARTED RUNWAY. AIRCRAFT SUSTAINED DAMAGE ON LOWER FUSELAGE IN SEVERAL AREAS. AIRCRAFT REPAIRED WITH THE FOLLOWING EC5300-01023, ETD2011-145-104349 AND ETD2011-145-104407. AIRCRAFT WAS RELEASED TO SERVICE 9/12/11.
MLI - FLT 5821 THE CREW REPORTED UNCOMMANDED NOSE WHEEL STEERING ON TOUCHDOWN. THE AIRCRAFT DEPARTED THE RUNAWAY. SUPPLEMENT TO FOLLOW:
TYS - A/C WAS DEPARTING CAE WHEN THE CREW GOT A MESSAGE ON THE EICAS OF LANDING GEAR AIR/GND FAILURE ON T/O. A/C RETURNED TO CAE AND WAS FERRIED TO TYS WITH RESTRICTIONS. UPON A SAFE ARRIVAL IN TYS, MAINTENANCE FOUND THE LEFT MLG REAR WOW PROX SWITCH OUT OF LIMITS. ADJUSTED THE GAP IAW EMB 145XR AMM 32-63-05; COMPLIED WITH THE GEAR EXTENSIONS AND RETRACTIONS TEST IAW EMB 145XR AMM 32-30-00. NO DISCREPANCIES NOTED AND WAS RELEASED FOR A TEST FLIGHT TO FURTHER EVALUATE. TEST FLIGHT WAS CANCELED DUE TO WEATHER SO THEREFORE A/C WAS REPO TO RIC TO VERIFY ANY ISSUES. NONE NOTED AND WAS RELEASED TO SERVICE.
IAH - FLIGHT 2390, THE CREW REPORTED AN AIR/GROUND FAIL AFTER T/O. THE AIRCRAFT RETURNED TO IAH WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE LANDING GEAR ELECTRONIC CONTROL UNIT, (LGEU), OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH - FLIGHT 2435, DURING TAKEOFF THE CREW REPORTED AN AIR GROUND FAIL MESSAGE. THE AIRCRAFT RETURNED TO IAH WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE ADJUSTED THE NLG WEIGHT ON WHEELS PROXIMITY SENSOR, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
FLIGHT 2610 - EWR - FLIGHT CREW REPORTED ELECTRICAL PROBLEM WITH NUMEROUS MESSAGES ON EICAS (LG AIR. GRND FAIL, SPS ADV, SPS1 INOP, SPOILER FAIL). EWR MX REMOVED AND REPLACED LGEU AND PERFORMED OPERATIONAL CHECK SUCCESSFULLY. ACFT WAS APPROVED TO RETURN TO SERVICE.
IAH - DURING AIRCRAFT MOVE TWO DENTS ON THE LEFT FUSELAGE 1 CTR SKIN WAS REPORTED BETWEEN FRAMES 19 & 20, STRINGERS 11L AND 12L. MAINTENANCE PERFORMED A DOUBLER REPAIR ON THE AFFECTED SKIN, LEAK CHECKED WITH NO DEFECTS. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
ABQ - FLIGHT 2728 - DURING PREFLIGHT INSPECTION THE CREW REPORTED THE RIGHT SIDE EMERGENCY EXIT HATCH EMERGENCY LIGHTS WERE FOUND TO BE INOPERATIVE. THE AIRCRAFT WAS FERRIED TO IAH WHERE MAINTENANCE REMOVED AND REPLACED THE AFFECTED INVERTER, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
SHV - EMERGENCY LIGHT INVERTER NR 6 IS INOP. REMOVED AND REPLACED NR 6 INVERTER IAW EMB 145XR AMM 33-50-11. AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
TYS FOR IAH - THE NOSE WHEEL DEFLECTED MOMENTARILY DURING ROLLOUT UNCOMMANDED. MAINTENANCE PERFORMED FUNCTIONAL TEST OF THE STEERING MANIFOLD IAW EMB 145XR AMM 32-50-04. FUNCTIONAL TEST GOOD. PERFORMED AN ADJUSTMENT TEST OF THE LOAD POTENTIOMETER IAW EMB 145XR AMM 32-50-05. TESTED WITHIN LIMITS, NO OTHER FAULTS FOUND AND WAS REL TO SERVICE.
TYS FOR IAH - THE RECIRCULATING FAN IN FWD AVIONICS BAY BECAME INOP. AIRCRAFT RETURN BACK TO IAH. MAINTENANCE RELEASED A/C IAW EL 21-24-01/2 CAT (C).
FLIGHT CREW REPORTED LAV SMOKE WARNING ON EICAS DURING CLIMBOUT WITH ASSOCIATED GALLEY WARNING BELL. MX REMOVED AND REPLACED ENGINE NR1 AND PERFORMED OPS CHECK SUCCESSFULLY. ACFT WAS APPROVED TO RETURN TO SERVICE.
