STANDARD TRAINING FLIGHT AND FINAL LANDING FOR THE DAY. VFR RULES THE WHOLE DAY. FLIGHT INSTRUCTOR TOOK T HE CONTROLS AND HEADED BACK TO KORK AIRPORT TO LAND. WE CONTINUED AS NORMAL MAKING A RIGHT DOWNWIND FOR RUNWAY 23, BEING FULLY CONFIGURED FOR LANDING ON FINAL BACKING THIS UP WITH OUR CHECKLISTS. WE LANDED ON THE NUMBERS WITH FULL FLAPS, AND GEAR DOWN AND LOCKED VERIFIED BY THREE GREEN LIGHTS. WE TOUCHED DOWN WITH OUR RIGHT MAIN MAKING CONTACT FIRST, VERIFIED BY TIRE MARKS ON THE RUNWAY. FOLLOWED BY OUR LEFT MAIN MAKING CONTACT ALMOST SIMULTANEOUSLY AS OUR NOSE WHEEL. IT FELT AND LOOKED AS IF OUR LEFT MAIN GEAR FAILED UPON MAKING CONTACT WITH THE RUNWAY. WE IMMEDIATELY STARTED TO VEER OF THE RUNWAY TO THE LEFT, AND AFTER TALKING TO PILOT NO CONTROL INPUTS HAD ANY EFFECT ON OUR TRAJECTORY. ROUGHLY 800 FEET AFTER TOUCHDOWN, JUST PRIOR TO THE 1000 FOOT MARKINGS, OUR LEFT WING/ENGINE/TIRE SEEMS TO HAVE MADE CONTACT WITH THE DIRT AND PULLED US FURTHER MORE TO THE LEFT, AT THIS TIME PILOT STOP AND FEATHERED THE ENGINES. WE CONTINUED TO VEER LEFT WHERE WE EVENTUALLY LANDED IN AN EMBANKMENT ROUGHLY 1400 FEET FROM THE RUNWAY MARKING NUMBER 23 IN A STRAIGHT LINE DISTANCE
LEFT GEAR NEEDED 5 CYCLES TO GET DOWN INDICATION. REMOVED AND REPLACED LH MLG INBOARD GEAR DOOR STOP W/ NEW PER SA227 MM CH 52-80-10. GEAR SWINGS GOOD.
ON LANDING AIRCRAFT PULLED RIGHT ON RUNWAY. MAINTENANCE REMOVED AND INSTALLED #4 BRAKE AND WHEEL ASSEMBLY, BLED BRAKE AND INSPECTED AIRCRAFT. NO DAMAGED WAS FOUND.
RIGHT OVERHEAT WING LIGHT LIT UP, RESET BLEEDS & R GEN AND LIGHT REILLUMINATED. MX INSPECTED AND TESTED AND GROUND RUN NORMAL. *REG# NOT FOUND IN AIRFLEETS
PIC REPORTED ENGINE "ROLLED BACK" (I.E. FLAMED OUT) ON 3 MILE FINAL INTO RIL. PIC REPORTED HE DID NOT FEATHER/SECURE THE ENGINE DUE TO STAGE OF FLIGHT AND LANDED WITH THE ENGINE WINDMILLING. AIRCRAFT LANDED AND TAXIED IN WITHOUT INCIDENT. PIC REPORTED ENGINE WAS FREE TURNING AND ON THE START LOCKS WHEN HE CONTACTED MX. MX DIRECTED PIC TO RUN THE ENGINE TO ATTEMPT TROUBLESHOOTING. ENGINE RUN WAS SATISFACTORY. MX PERSONNEL WERE FLOWN TO RIL AND THE ENGINE WAS INSPECTED AND ADDITIONAL GROUND RUNS INCLUDING POWER ASSURANCE RUNS WERE CONDUCTED WITH NO ANOMOLIES. THE AIRCRAFT WAS FERRIED TO APA WITH NO ISSUES. THE FUEL SOV WAS REPLACED AS A PRECAUTIONARY MEASURE ALTHOUGH NO FAILURE WAS EVER OBSERVED. AIRCRAFT WAS TEST FLOWN AND AN NTS SHUTDOWN WAS CONDUCTED SATISFACTORILY WITH THE EXCEPTION OF AN INOP. AUTO IGNITION LIGHT. THE LIGHT WAS REPAIRED AND THE AIRCRAFT WAS RETURNED TO SERVICE WITH NO DETERMINATION MADE AS TO THE CAUSE OF THE FAULIRE. AS OF THIS REPORT THE AIRCRAFT HAS OPERATED SEVERAL FLIGHTS WITH NO ISSUES.
PILOT LANDED SAFELY IN DEN AND GOT OUT TO DO A POST FLIGHT AND CHOCK THE AIRCRAFT WHEN HE NOTICED THE FRONT RIGHT WHEEL WAS MISSING FROM THE AIRCRAFT AFTER TAXIING IN AND PARKING ON THE RAMP. MORE INFORMATION TO COME ONCE MX IS ABLE TO ACCESS THE NOSE LANDING GEAR.
PILOT REPORTED RH WING OVERHEAT ILLUMINATED N FLIGHT (STEADY), FOLLOWED CHECKLIST DEACTIVEATED RT BLEED AND CONTIMUIED TO DESTINATION WITH NO FURTHER ISSUES. MX REMOVED AND REPLACED RT MIXING VALVE WITH OVERHAULED, GROUND OPS CHECK GOOD.
PILOT REPORTED LOUD BANG IN FLIGHT FOLLOWED BY ACTIVATION OF NTS SYSTEM; PILOT SECURED ENGINE AND LANDED. ENGINE REMOVED/REPLACED IN THE FIELD AND TRANSPORTED TO MAIN MX BASE FOR TEARDOWN. MX PERSONNEL FOUND DURING TEARDOWN THAT SEVERAL BLADES ON THE 1ST HPT WHEEL ASSY FAILED AND BROKE AWAY FROM THE DISK AT THE FIR TREE ATTACHMENT. ENGINEERING/METALLURGICAL ANALYSIS HAS BEEN REQUESTED.
AIRCRAFT DEPARTED RAMP WITH NOSE WHEEL STEERING DEFERRED IAW MEL. RAMP & TAXIWAY SURFACES ICY. PILOT REPORTED THAT THE ACFT WAS DIFFICULT TO TURN WITH DIFFERENTIAL BRAKING. BRAKES OVERHEATED, THE PILOT IN THE ACFT BEHIND N2728G CALLED ON THE RADIO TO INFORM PILOT OF N2728G THAT THE BRAKES APPEARED TO BE SMOKING & ON FIRE. FOUND ALL FOUR BRAKES BURNT & DAMAGE TO TIRES DUE TO HEAT & FIRE.
PILOT REPORTED THAT ON CLIMB OUT THE ENG LOST OIL PRESS. PILOT SECURED THE ENG & RETURNED TO THE DEPARTURE AIRPORT & LANDED. MX FOUND SIGNIFICANT DAMAGE TO THE 3RD STAGE TURBINE & DISASSEMBLED ENG. AFT TURBINE BEARING WAS FOUND DESTROYED, FAILED. 1ST & 2ND TURBINES SHOWED NO IMPACT DAMAGE BUT SHOWED SIGNIFICANT DISTRESS & WERE COATED IN ALUMINUM, FROM COMPRESSOR HOUSING. FURTHER DISASSEMBLY STILL REQUIRED. SDR WILL BE AUGMENTED ONCE TEARDOWN IS COMPLETE & ROOT CAUSE IS ISOLATED.
STABILIZER PITCH TRIM ACTUATOR FAILED IN FLIGHT. NO MOVEMENT OF STABILIZER WAS POSSIBLE. PILOT MADE PRECAUTIONARY LANDING. OPEN
DURING FLIGHT FROM ISN TO MOT, PILOT NOTICED FLUCTUATING OIL PRESSURE, THEN OIL PRESSURE DROPPED BELOW THE YELLOW ARC. PILOT SHUT DOWN THE ENGINE, LANDING WITHOUT INCIDENT. FOUND OIL LINE FROM NTS TEE TO GEARBOX HAD CHAFED THROUGH ON ANOTHER OIL LINE. SHOULD HAVE BEEN DISCOVERED DURING PHASE INSPECTION.
NR 1 ENGINE VARYING IN TORQUE AND TEMPRATURE. SWITCHING OFF SRL COMPUTER FAILED TO CORRECT PROBLEM. NR 1 ENGINE SHUTDOWN IN FLIGHT. AIRCRAFT LANDED IN PDX WITHOUT FURTHER PROBLEM. NR 1 FCU AND FUEL PUMP REPLACED. P2 SENSOR LINE CHECKED FOR OBSTRUCTION, NONE NOTED. ENGINE OPS CHECKED GOOD ON GROUND RUN.
