ON DECENT IN TO ELP THE LANDING GEAR WAS EXTENDED, THE RH M/L/G DOWN AND LOCKED GREEN LIGHT DID NOT ILLUMINATE. THE ALTERNATE LANDING GEAR EXTENSION SYSTEM WAS USED WITH THE GREEN LIGHT STILL WAS NOT ILLUMINATED. THE CREW DECLARED AN EMERGENCY AND FLEW BY THE TOWER FOR A VISUAL CHECK OF THE GEAR, ATC REPORTED ALL THREE LANDING GEAR WERE DOWN. A NORMAL LANDING WAS COMPLETED.
AIRCRAFT DEPARTED LYH-SHV, AT CRUISE ALTITUDE OF FL280 THE LEFT ENGINE EXPERENCED A LOSS OF POWER/FLAMEOUT, THE CREW DECENDED TO FL240 AND PERFORMED A SUCCESSFUL AIR START. AFTER LANDING IN SHV THE ENGINE WAS INSPECTED BY AMERISTAR MAINT AND THEY FOUND FOD DAMAGE TO THE 1ST STAGE COMPRESSOR BLADES AND THE FAN BLADES.
ON APPROACH IN TO KYIP THE LANDING GEAR WAS SELECTED DOWN WITH ONLY 3 RED LIGHTS ILLUMINATED, PILOT REPORTED IT DID NOT SOUND LIKE THE GEAR HAD EXTENDED AND CONTACTED TOWER TO DO A FLYBY AND WAS TOLD THEY DID NOT SEE THE GEAR EXTENDED. LANDING GEAR EMER EXTENSION PROCEDURE 'A' WAS PERFORMED PER THE AFM WITH THE GEAR EXTENDED AND ALL INDICATIONS NORMAL. AIRCRAFT LANDED WITHOUT INCIDENT.
TOOK OFF FROM RUNWAY 30, TAKEOFF NORMAL AT 400 FT, V2 + 20 SELECTED FLAPS ZERO. FLAPS STAYED AT 15 DEGREES. CLIMBED TO 4000 FT MSL. ATTEMPTED TO IDENTIFY PROBLEM AND FOUND FLAPS WOULD NOT EXTEND PAST 15 DEGREES. CONTACT TOWER AND ASKED FOR LONGEST RUNWAY. TOWER DECLARED EMERGENCY UNREQUESTED BY FLT CREW. LANDED WITHOUT INCIDENT. MAINTENANCE DISCOVERED INTERMITTENT FAILURE OF THE FLAP DRIVE MOTOR. FLAP DRIVE MOTOR/GEAR REDUCTION UNIT REPLACED IAW MM 27. OPERATIONS CHECK GOOD. AIRCRAFT RETURNED TO SERVICE.
CLIMBOUT GTR GEAR UNSAFE LIGHT UPON GEAR RETRACTION. CREW PERFORMED PROCEDURES PER MANUAL. LANDING UNEVENTFUL IN GTR. AIRCRAFT MOVED TO ADS UNDER FAA SPECIAL FLIGHT PERMIT FOR REPAIR. MAINTENANCE FOUND FAULTY SWITCH IN NOSE GEAR UP LOCK. REPLACED FAULTY SWITCH IN UP LOCK ASSY. NO FURTHER ACTION REQUIRED.
DURING TANK AND PLANK INSPECTION THE LT AND RT WING TO FUSELAGE ATTACH FITTINGS WERE DISCOVERED TO HAVE MINOR CORROSION ON THE LOWER OB SURFACE. THE WINGS WERE DEMATED TO INSURE THAT CORROSION HAD NOT MIGRATED TO BETWEEN THE WING TO FUSELAGE ATTACH FITTINGS. CORROSION WAS EVALUATED BY ENGINEERING AND AN ENGINEERING INSTRUCTION WAS ISSUED. CORROSION WAS WORKED IAW THE ENGINEERING INSTRUCTION. NO FURTHER ACTION REQUIRED. [CLOSED]

FOUND CHANNEL CRACKED & REPAIRED. AFTER REPAIR PERFORMED GROUND PRESSURIZATION CHK & FOUND SAT. A REVIEW OF EVENTS PRIOR TO DOOR OPENING REVEALED CHANNEL CRACK COULD HAVE BEEN PRE-EXISTING ALLOWING DOOR TO FLEX UNDER CABIN PRESSURE ENOUGH TO DISENGAGE FROM FORWARD LOCK & DISENGAGE ALL LOCKS. ACFT RTS. [CLOSED]
