SMOKE DET IN LAVATORY SOUNDS INTERMITTENTLY. REPLACED DETECTOR AND BATTERY. OPERATIONAL CHECK GOOD.
FLOOR LEVEL EXIT SIGN AT 6C IS MISSING. INSTALLED SERVICEABLE EMER EXIT PANEL AT SEAT 6C. OPERATIONAL CHECK GOOD.
WHILE IN A DESCENT, AC PITCHED UP AND STARTED CLIMBING. BOTH FLIGHT DIRECTORS PITCHED UP AND DESC BUTTON WOULD NOT STAY ILLUM. REPLACED ELEVATOR AUTOPILOT SERVO IAW MM 22-10-05. OPERATIONAL CHECK GOOD.
STALL WARNING SYSTEM 2 INOP. REPLACED STALL WARNING TEST PANEL. OPERATIONAL CHECK GOOD.
LAST EMERGENCY LIGHT IN BACK OF CABIN IS OUT. INSTALLED BULB INTO SOCKET OF EMERGENCY LIGHT IN THE BACK OF THE CABIN. OPS CHECKS GOOD.
ROW 10 FLOOR LEVEL EXIT ROW WHITE LIGHT OUT. REPLACED BULBS AT ROW 10 EMERGENCY LIGHTS. OPERATIONAL CHECK GOOD.
WHEN CAPTAIN'S STALL WARNING SYSTEM IS TESTED THE SHAKER DOES NOT ACTIVATE. REPLACED NR 1 STICK SHAKER SERVO. OPERATIONAL CHECK GOOD.

CA'S AP DOES NOT HOLD ALTITUDE. REMOVED AND REPLACED NR 1 APC-65B. OPS CHECK GOOD IAW MM 22-11-00. P/N: 622-8317-402, S/N OFF: 4147.
AFTER LANDING, BOTH NACELLE FLAPS WERE SLOW TO RETRACT, CAUSING DISAGREEMENT. NACELLE FLAPS EVENTUALLY RETRACTED ON THEIR OWN. REMOVED AND REPLACED LT NACELLE FLAP ACTUATOR WITH SERVICEABLE UNIT IAW MM 27-50-00. OPS CHECKS GOOD. PN: 343100-1001, SN ON: 0228, SN OFF: 0175
CAPTAIN'S AUTOPILOT AND TRIM FAIL. REPLACED CAPTAIN'S AUTO PILOT COMPUTER. OPS CHECK GOOD.
NR 2 STALL SHAKER INOPERATIVE. REPLACED NR 2 STALL SHAKER. SYSTEM TESTS NORMALLY IAW MM 27-34-00.
ALTITUDE HOLD ON CAPT SIDE WILL DRIFT 150' DOWN. REPLACED AUTO PILOT COMPUTER APC-65B. OPS CHECK GOOD IAW MM 22-10-00.
SMOKE IN CABIN AND ASSOCIATED RIGHT PACK FAIL. REPLACED RIGHT PACK GROUND COOLING FAN. OPS CHECKED NORMAL.
ON TAXI OUT, AFTER SELECTING FLAPS 15 DEGREES, THE HYDRAULIC QUANTITY ON THE BLUE SIDE WENT TO ZERO WITH ASSOCIATED LIGHTS. REMOVED AND REPLACED LEFT NACELLE FLAP ACTUATOR IAW RII 27-307 AND 27-50-08. LEAK CHECKS GOOD. NO LEAKS NOTED AT THIS TIME.
RIGHT GROUND COOLING FAN STOPS INTERMITTENTLY AND CAUSES SMOKE IN CABIN. REPLACED RIGHT GROUND COOLING FAN. OPS CHECK GOOD.
F/A SMELLED SMOKE IN CABIN AFTER PAX DEPLANED IN MFR. CABIN HAD FILLED WITH SMOKE. LAV SMOKE DETECTOR ACTIVATED. CALLED FIRE DEPT. INVESTIGATED CABIN FOR SMOKE. RAN APU ON BOTH PACKS AT SEPARATE TIMES. INSPECTED EXTERIOR OF PACK UNITS. OIL NOTED ON RT PACK. RAN RT ENGINE WITH RT PACK. DETERMINED POSSIBLE CAUSE TO BE OIL IN RT BLEED & RT PACK. INSPECTED RT ENGINE BLEED DUCT AFT OF FIRE SEAL FOR OIL ON INSIDE. NONE NOTED. RAN RT PACK WITH RT ENGINE BLEED FOR 15-20 MINUTES. NO SMOKE NOTED. DID NOTICE RT PACK MODE SELECT IN MANUAL, SWITCHED TO AUTO. OPS CHECKED GOOD, NO DISCREPANCIES NOTED.
RIGHT ENGINE SMOKES ON SHUTDOWN. FOUND ENGINE BLOWING OIL OUT FROM FRONT COMPRESSOR. REMOVED AND REPLACED RT ENGINE AS REQUIRED IAW 71-00-00. OPS CHECK GOOD. PACKS NO LONGER SMOKING.
CAPTAINS AUTOPILOT DOES NOT CAPTURE GS WITH APPR MODE. CAPTAIN'S AUTOPILOT FAILED WITH AILERON SERVO FAILURE. REPLACED PUSHED OUT PIN 44 ON J3 OF PILOT'S AUTOPILOT COMPUTER RACK. CAPTAIN`S AUTOPILOT OPS CHECK GOOD IAW MM 22-10-00. TESTED AILERON SERVO WITH BOTH CAPT'S AND F/O'S AUTOPILOT. COULD NOT DUPLICATE DISCREPANCY. BOTH AUTOPILOTS AND AILERON SERVO OPS CHECKS GOOD IAW MM 22-10-00.
EXIT SIGN ON FIRST OFFICER'S SIDE BULKHEAD DOES NOT ILLUMINATE WHEN EMERGENCY LIGHTS ARE TESTED. FOUND LOOSE CONNECTION. SECURED CONNECTION. OPS CHECK GOOD.
AFTER DEPARTURE, BOTH THE EHSI AND THE NAVIGATION SCREENS WENT BLANK ON THE FIRST OFFICER'S SIDE. REMOVED AND REPLACED CTL32 (NR 2). OPS CHECKS GOOD ON GROUND.
FIRST OFFICERS HDG FLAG AND NR 1 RMI FLAG ON IN CRUISE. RETURNED TO THE FIELD. RE-ORIENTATED BOTH NR 1 AND 2 AHRS SYSTEMS IAW 34-00-00. GROUND OPS CHECK GOOD AT THIS TIME.
ON DOWNWIND THE AILERONS FROZE FOR 12 SECONDS, WE WERE UNABLE TO MOVE YOKE. ON BASE TURN THE AILERONS FROZE FOR 5 SECONDS AND ON FINAL THE AILERONS FROZE FROM APPROX 200 FT AGL UNTIL 20 SECONDS AFTER ROLLOUT. AUTOPILOT WAS CONFIRMED DISCONNECTED BY BOTH PILOTS. WHEN AILERONS WERE FROZEN THE YOKE WAS NEUTRAL AND UNMOVEABLE BY BOTH PILOTS. REMOVED UPPER AND LOWER ACCESS PANELS TO LEFT AND RIGHT AILERONS. INSPECTED YOKE CHAIN DRIVE. NO DEFECTS NOTED. FOUND PANEL MISSING 2 SCREWS ON LT AILERON JUST FWD OF THE TRIM TAB. LEADING EDGE WAS BENT FORWARD AND IMPEDING TRAVEL ON LEFT AILERON BELLCRANK. RE-SECURED PANEL. SCREW HOLES ARE NOT ELONGATED. CLOSED ALL PANELS. CONTROL DISCONNECT LIGHTS OUT ON MAP.
ELEVATOR TRIM FROZE IN FLIGHT AND BROKE LOOSE SUDDENLY, CAUSING VIOLENT PITCH DOWN. REMOVED AND REPLACED RIGHT AND LEFT ELEVATOR TRIM TAB LINEAR ACTUATORS IAW EMB 120 MM 27-32-02 AND 27-00-01. LEFT TRIM ACTUATOR S/N OFF: 735, S/N ON: UNKNOWN. RIGHT TRIM ACTUATOR S/N OFF: 736, S/N ON: 426.
NOTICED HOT AIRCRAFT BEFORE TAKEOFF. AFTER TAKEOFF FA SMELLED SMOKE AND SMOKE DETECTOR IN LAV WENT OFF. RETURNED TO LAX. INSTALLED NEW LEFT PAC DUCT TEMP SENSOR. CHECKS GOOD.

AIRCRAFT LOST PRESSURIZATION AT 17,000 FEET. SLOW LOSS OF PRESSURIZATION, DESCENDED PER QRH. RETURNED TO LAX. INSPECTED AND FOUND SMALL SECTION OF CARGO DOOR SEAL LOOSE FROM RETAINER. REINSTALLED AND SECURED CARGO DOOR SEAL. PRESSURIZED AIRCRAFT TO 5+ PSID. NO LEAKS NOTED. PRESSURIZATION OPS CHECKS GOOD.
PASSENGER DOOR LIGHT CAME ON AFTER TAKEOFF. RETURNED TO SEA. REPAIRED BROKEN WIRE AT DOOR HINGE, OPS CHECK GOOD IAW 52.
CABIN ALTITUDE CLIMBS IN AUTO, OK IN MANUAL. RETURNED TO SFO. REPLACED AUTO CONTROLLER. OPS CHECKS GOOD.
DIVERTED TO BUR WHEN THE LEFT PACK FAILED AND FILLED CABIN WITH SMOKE. OPERATIONAL CHECKED LEFT PACK, LEFT ENGINE HIGH STAGE BLEED SWITCHING SYSTEM, RAM AIR SYSTEM AND PACK FAIL SWITCH WIRING. NO DISCREPANCIES NOTED ANY WHERE. ALL SYSTEMS OPERATE NORMALLY.
RIGHT DUCT LEAK LIGHT CAME ON AFTER TAKEOFF DURING CLIMB-OUT. WENT THROUGH PROCEDURE BUT THE LIGHT WOULD NOT EXTINGUISH UNTIL ENGINE SHUTDOWN. RETURNED TO SLC. INSPECTED ENTIRE RIGHT SIDE BLEED ON DUCT SYSTEM AND FOUND NO DISCREPANCIES. RAN AIRCRAFT ON GROUND FOR 1/2 HR AND COULD NOT DUPLICATE.
CORRODED BUTT STRAP JUST FORWARD OF AFT CENTER BUTT STRAP. REMOVED CENTER BUTT STRAP SECOND FROM AFT OF PLANE AND REPLACED WITH A NEW BUTT STRAP IAW SRM 51-40-02.

IN-FLIGHT FAILURE OF THE BLUE HYDRAULIC SYSTEM WITH LOSS OF FLUID. FOUND NR 6 HYDRAULIC LINE CHAFING ON ENGINE OIL COOLER AIR INLET. REPLACED NR 6 LINE AND RIGHT ENGINE DRIVEN HYDRAULIC PUMP. RAN RIGHT ENGINE, OPS CHECK AND LEAK CHECK GOOD.
