NR 2 ENGINE HIGH ITT, 765 AT 85 PERCENT AND FUEL FLOW HIGHER THAN NR 1 ENGINE. FOUND Y-PIPE JUST AFT OF NR 2 HP VALVE BROKEN. REMOVED AND REPLACED Y-PIPE USING ORIGINAL HARDWARE AND CLAMPS, OPS & LEAK CHECK GOOD IN ACCORDANCE WITH 21-11-00 AND ALL ENGINE PARAMETERS WITHIN LIMITS.
DURING C-CHECK INSP, A CRACK WAS FOUND IN THE FWD, INBD CORNER OF THE WING BOX LADDER PLATE UNDER STRESS PANEL 622AT. AN ENGINEERING ORDER WAS ISSUED REFERENCING RD8-57-4107 TO INCORPORATE RD8-57-2387 TO REPAIR THE LADDER PLATE. REPAIR WORK WAS THEN COMPLETED.
DURING C-CHECK INSPECTIONS, A CRACK WAS FOUND IN THE AFT OTBD CORNER OF THE WING BOX LADDER PLATE UNDER STRESS PANEL. AN EO WAS ISSUED REF RD8-57-4106 TO INCORPORATE RD8-57-2375 TO REPAIR THE LADDER PLATE. REPAIR WORK WAS THEN COMPLETED. (K)

DURING C-CHECK INSPECTIONS, CORROSION WAS FOUND ON THE LOWER DOOR SILL FOR THE AFT RT SERVICE DOOR. AREA OF CORROSION WAS CLEANED, ALODINE AND PRIMER APPLIED IAW DEHAVILLAND STRUCTURAL REPAIR MANUAL.
IN PREPARATION FOR LANDING AT PHL THE FLIGHT CREW SELECTED THE GEAR DOWN AND THE NOSE GEAR INDICATOR WAS RED. THE MAIN GEAR INDICTED NORMAL. THE FLIGHT CREW DID THE ALTERNATE EXTENSION AND THE NOSE GEAR DID NOT COME DOWN. THE AIRCRAFT LANDED WITH THE NOSE STILL RETRACTED, , THERE WERE NO INJURES REPORTED. UPON INSPECTION, LINK ON THE STEERING ACTUATOR FOUND BROKEN. STEERING ACTUATOR AND LINKS REPLACED. ACFT FERRIED TO SBY. DURING INSPECTION AFTER A NOSE GEAR UP LANDING, THE FRAME MEMBER AT X167.5 WAS BUCKLED BETWEEN STRINGERS 32S AND 32P.FRAME MEMBER WAS REPLACED IAW DEHAVILLAND STRUCTURAL REPAIR MANUAL PROCEDURES. AIRCRAFT HAS NOT BEEN RELEASED YET DUE TO OTHER ONGOING WORK.
STRONG ELECTRICAL ODOR IN CABIN OVERHEAD ISLE BETWEEN ROWS 2 AND 4, OVERHEAD FLUORESCENT LIGHT FAILED. AIRCRAFT WAS ON GROUND. INSPECTED AREA AND FOUND OVERHEAD LIGHT AT 2DF SOCKET BURNED. INSTALLED NEW BULB, NO FIX. INSPECTED SOCKETS, REMOVED AND REPLACED BALLAST INSTALLED NEW BULB OPERATIONAL CHECK GOOD. REMOVED BALLAST, BR9000-21, CONFORMED WITH REQUIREMENTS OF AD 2006-17-10, AND SHUT POWER OFF TO THE LIGHT FIXTURE. (K)

AFTER LANDING FOUND OIL DRIPPING FROM BOTTOM OF NR 1 ENG. 00 ESP HISTORY: NONE. FACT 19AUG08 05:05 CHS, FOUND PCU AND ADAPTER LEAKING. REPLACED ALL O-RINGS AND REINSTALLED AND RIGGED PCU IAW WORKCARDS 61-1-0-10R AND 76-00-02Q AND COMPLIED WITH MM 05-50-00 (EXCLUDING PATCH CHECK). NO DEBRIS OR DEFECTS FOUND. GROUND RAN, ENG LEAK AND OPS CHECK GOOD. (S)

WHILE TAXIING IN EWN, NR 2 SIDE DC BUSS FAULTED AND THERE WAS SOME SMOKE ON NR 2 SIDE. THE SMOKE WAS FOUND TO BE COMING FROM THE APU. FOUND THE STARTER GENERATOR OVERHEATED AND APU START CONTACTOR WELDED. NO DAMAGE FOUND IN THE APU AREA. REPLACED THE START CONTACTOR K-62 AND INSPECTED WIRING, NO DEFECTS NOTED.

SMOKE COMING FROM F/O WINDSHIELD NEAR HEATER TERMINAL. MAINTENANCE INSPECTED WIRING AND FOUND A LOOSE WIRE ON THE FIRST OFFICERS WINDOW AT THE HEATER TERMINAL L2. WIRING INSPECTED FOR DAMAGE, NO DEFECTS NOTED. TIGHTENED CONNECTOR, OPERATIONAL CHECK GOOD.
F/A REPORTS EMERGENCY LIGHTS DO NOT WORK. REMOVED AND REPLACED EMERGENCY BATTERY PACKS.
LOST NUMBER 2 HYDRAULIC FLUID AFTER GEAR RETRACTION, SYSTEM WENT INTO ISOLATION, ONE QUART LEFT IN SYSTEM, PRESSURE NORMAL. REMOVED AND REPLACED THE BLUE FLEXIBLE UP LINE TO NOSE GEAR DRAG STRUT ACTUATOR, SERVICED AND BLED SYSTEM.
TWO INBOARD COWL LATCHES CAME LOOSE. INSPECTED AND SECURED LATCHES AS REQUIRED.
IN FLIGHT NR 2 HYDRAULIC SYSTEM WENT TO 1 QT AND NR 2 ISO VALVE CAUTION LIGHT ILLUMINATED. NO OTHER CAUTION LIGHTS AND QUANTITY REMAINED ABOVE 1QT. FLIGHT DIVERTED TO MDT WHERE IT WAS FOUND HYDRAULIC FLUID WAS LEAKING FROM BEHIND THE NR 2 NACELLE GRAVEL GUARD. LINES WERE CAPPED OFF AND AIRCRAFT WAS FERRIED TO SBY. INSTALLED HYDRAULIC LINE IN NR 2 NACELLE FOR GEAR DOOR DOWN PRESSURE AND REPLACED HYDRAULIC LINE IN NR 1 NACELLE FOR STAY STRUT ACTUATOR.
ON CLIMB-OUT, BETA WARNING HORN SOUNDED. ALL ENGINE PARAMETERS NORMAL. DEACTIVATED SYSTEM. R&R MICRO SWITCH ACTUATOR AT M/S 3151-S9 IN CENTRAL COLLUM. OPS CHECK GOOD.
WITH WEATHER RADAR ON, GRAPHICS AND WEATHER COMPRESS'S TOWARD CENTER OF RADAR SCREEN. RETURNED TO FIELD. REMOVED AND REPLACED INDICATOR, OPS CHECKED GOOD.
