DURING A VISUAL INSPECTION OF THE ACFT FOUND AN AREA ON THE TAILBOOM THAT LOOK LIKE CORROSION ON THE TAIL BOOM AT LT SIDE WL 2800 STA 6800. AFTER REMOVING THE TAIL BOOM AND INSPECTING THE WHOLE TAIL BOOM ASSEMBLY A CRACK WAS FOUND AT THE 10L FRAME BULKHEAD 1/8 IN LENGTH AT STA 9800 WL 3000. REPLACED TAIL BOOM ASSY.
DURING A VISUAL INSPECTION OF THE ACFT FOUND AN AREA ON THE TAIL BOOM THAT LOOK LIKE CORROSION ON THE TAIL BOOM AT LT SIDE BL 300 STA 7600 WL 2800. AFTER INSPECTING THE WHOLE TAIL BOOM ASSEMBLY A CRACK WAS FOUND AT BL 50 WL 3000 STA 9700 RT SIDE 1 INCH IN LENGTH. REPLACED THE TAIL BOOM ASSEMBLY.

DURING A VISUAL INSPECTION OF THE ACFT FOUND AN AREA ON THE TAIL BOOM THAT LOOKED LIKE CORROSION AT LT SIDE BL 300 STA 7600 WL 2800. AFTER INSPECTING THE WHOLE TAIL BOOM ASSEMBLY, A CRACK WAS FOUND AT BL 50 WL 3000 STA 9400 RT SIDE 1 INCH IN LENGTH. REPLACED THE TAIL BOOM ASSEMBLY.
DURING VISUAL INSPECTION OF THE ACFT FOUND A HOLE IN THE BELLY OF THE ACFT CENTERLINE JUST FORWARD OF THE REAR CROSSTUBE AREA. HOLE IS 18MM IN DIAMETER.
MAIN ROTOR MAST WAS REMOVED FOR OTHER INSPECTIONS AND WAS FOUND TO HAVE FRETTING IN ZONE "G". AREA OF THE SEALING RING RUNNER FOR THE UPPER MAST SEAL.
ON SHORT FINAL FOR LANDING, NR2 ENGINE NP NEEDLE ON TRIPLE TACK STARTED FLUXUATING THEN DROPPED TO 40 PERCENT AND THEN WENT TO 115 PERCENT WITHOUT POWER ADJUSTMENT NG, TORQUE, TOT VALUES UNKNOWN DUE TO CLOSE PROXIMITY TO GROUND. POWER LEVER ON NR2 ENGINE REDUCED TO IDLE. ESTIMATED TIME OF OVERSPEED TO TOUCHDOWN 10 TO 15 SECONDS EMERGENCY ENGINE SHUTDOWN WAS PREFORMED ON BOTH ENGINES AFTER LANDING. A STRONG ODOR OF BURNED OR HOT METAL WAS NOTICED BY MYSELF AND COMMUNICATIONS SPECIALIST. POST FLIGHT WAS PREFORMED, NO VISUAL DAMAGED WAS NOTICED. MX, CHAIN OF COMMAND , OPS CONTROL CENTER NOTIFIED. ACFT WAS WRITTEN UP AND TAKEN OUT OF SERVICE. DON'T KNOW IF THERE WAS AN INDICATOR FAILURE OR A 2 ENGINE FAILURE. THE PILOT DOESN'T KNOW WHAT THE TOT OR NG OR TORQUE VALUES WERE ON THE NR 1 ENGINE BECAUSE THE HE WAS ON SHORT FINAL. WE ARE REPLACING BOTH ENGINES.
APROX 3 MINUTES AFTER TAKEOFF, DURING CRUISE FLIGHT AT APROX 120 KNTS AT 1500 MSL, 1000 FT AGL, THE CO PILOT DOOR SLIDING WINDOW DEPARTED THE ACFT DURING FLIGHT AND MADE CONTACT WITH THE REAR SLIDING DOOR AND THE FUSELAGE. NO CHANGE IN ACFT CONTROL WAS DETECTED AND THE ACFT RETURNED TO BASE.
NR 2 ENGINE DECK HAS 12MM X 35MM CHAFED AREA CAUSED BY FIRE EXTINGUISHER TUBE RUBBING. THIS BEING FILED ON BEHALF OF CRS QML2.
DAMAGE FOUND ON BELLY OF ACFT THAT WILL REQUIRE A MAJOR REPAIR.
DURING INSP FOUND SPIDER CRACKS AROUND 1 RIVET ON THE DECK UNDER THE TANDEM HYD PACK. FURTHER INSP FOUND 2 FRAMES CRACKED UNDER THE HYD DECK.
THE AIRCRAFT ROLLED VERY SLIGHTLY TO THE RT AND THEN TO THE LT. THE ROLL RATE WAS CASUAL, AND SINCE WE WERE FLYING IN SLIGHTLY GUSTY CONDITIONS I TOOK ALMOST NO NOTICE OF IT. THEN THE AIRCRAFT DID IT AGAIN. THIS TIME THE ROLLS WERE ACCOMPANIED BY THE STRONG ODOR OF HOT ELECTRICAL INSULATION. PUNCHED OFF THE CSAS SYSTEM, THEN RE-ENGAGED THE PITCH AND YAW CHANNELS AND COMPLETED THE FLIGHT. ROLL CHANNEL WAS MEL'D AND PLACARDED INOP.
PATIENT FLIGHT, DURING THE FLIGHT THE MASTER CAUTION ILLUMINATED WITH ACCOMPANYING ROLL SAS CAUTION LIGHT, IMMEDIATELY FOLLOWED BY A STRONG ACRID BURNING ODOR IN THE COCKPIT. NO OTHER ADVERSE INDICATIONS WERE NOTED. SWITCHED OFF ALL SAS CHANNELS AND THE ODOR DECREASED. NOTIFIED DISPATCH OF THE INCIDENT AND ASKED FOR A MECHANIC TO BE INFORMED. FLIGHT CONTINUED TO DESTINATION WITHOUT FURTHER INCIDENT. SAS COMPUTER WAS CHANGED AND TESTED SATISFACTORILY AND RETURNED TO SERVICE. DEFECTIVE SAS COMPUTER HAD A VERY STRONG BURNED SMELL COMING FROM IT.
