RT WING INBD FLAP L/E PUNCTURED REMOVED DAMAGED SKIN AND CORE, FABRICATED INNER SKIN AND BONDED, FABRICATED CORE AND BONDED, FABRICATED REPAIR PLIES AND BONDED, SANDED SMOOTH TO CONTOUR, SEALED EXPOSED FIBERS, CLEANED AREA, APPLIED PRIMER AND PAINT, WORK PERFORMED IAW SRM 57-52-00 PARA 8 A FIG 201. REQUIRED INSPECTION COMPLETED BY SAN NR 489916.

AFTER TAKEOFF AT 25,000FT AFT LAV SMOKE MASTER WARNING ILLUMINATED. FLIGHT ATTENDENT INVESTIGATION REVEALED NO SMOKE OR FIRE BUT ABNORMAL ODOR WAS DETECTED. FLIGHT CREW ELECTED TO RETURN TO DEPARTURE AIRPORT. MAINTENANCE FOUND THE APU INLET SOAKED WITH OIL AND OIL LEAK AT APU OIL COOLER. APU PLACED ON MEL. Â
DURING DESCENT INTO MEM, EXPERIENCED BURNING ODOR AND OBSERVED WHITE SMOKE IN COCKPIT. DECLARED AN EMERGENCY AND PROCEEDED TO MEM AND LANDED UNEVENTFULLY. INSPECTED ALL AVIONICS BAYS AND BEHIND CIRCUIT BREAKER PANELS, OVENS, AND COFFEEMAKERS WITH NO PROBLEMS FOUND AND NO ODOR PRESENT. RAN BOTH PACKS WITH NO SMOKE NOTED. PERFORMED TEST FLIGHT WITH NO SMOKE OR ODOR PRESENT. AIRCRAFT RETURNED TO SERVICE.
DURING FLIGHT, FUMES WERE REPORTED IN THE CABIN AND COCKPIT. NO SMOKE OR HEAT SOURCE WAS IDENTIFIED. THE OZONE FILTER WAS REPLACED, THEN ROW 11F EMERGENCY LIGHTS POWER SUPPLY SHORTED. REPLACED THE POWER SUPPLY AND TESTED OK.
DURING L-CHECK FOUND CRACK ON RIGHT HAND MAIN LANDING GEAR DOOR ACTUATOR FITTING. REPAIRED PER EA 32-181770.
DURING CRUISE, CREW NOTICED GREEN HYDRAULIC FLUID QUANTITY DECREASING. DIVERTED TO MSP AND LANDED WITHOUT INCIDENT. MAINTENANCE REPLACED THE NR 1 YAW DAMPER ACTUATOR, OPERATIONAL CHECK OK.
DURING INSPECTION OF PASSENGER DOOR EVACUATION SLIDE PACK RELEASE CABLE, FOUND DEFECTIVE CABLES ON SLIDES AT FORWARD RIGHT, AFT RIGHT, AND LEFT DOORS. REPLACED CABLES ON SLIDES AT AFT DOORS AND REPLACED SLIDE ASSEMBLY AT FORWARD DOOR.
DURING CRUISE, CREW NOTED GREEN HYDRAULIC SYSTEM QUANTITY DECREASING AND RECEIVED HYD G RSVR LO LVL MESSAGE. CREW FOLLOWED COCKPIT OPERATING MANUAL PROCEDURE, DECLARED EMERGENCY, DIVERTED TO DTW AND LANDED WITHOUT INCIDENT. MAINTENANCE REPLACED LEAKING O-RING SEAL ON POWER CONTROL UNIT VALVE BLOCK FILTER (24CV) AND RESERVICED HYDRAULIC SYSTEM. LEAK CHECK OK.
DURING LINE MAINTENANCE, THE EMERGENCY ESCAPE PATH LIGHTS AT SEAT ROWS 14, 15, 16, 19, 20, 21,25, 26, AND 27 WERE FOUND INOPERATIVE. REPAIRED SHORTED WIRE IN SEAT TRACK AT ROW 26 AND REPLACED EPSU (110WL2), OPERATIONAL CHECK OK.
DURING THRU-FLIGHT CHECK, THE EMERGENCY LIGHTS SYSTEM FAILED TO TEST. REPLACED EPSU (13WL) AT DOOR 3L, OPERATIONAL CHECK OK.
SHORTLY AFTER LIFT OFF, RECEIVED F/CTL SPLR 3 FAULT MESSAGE FOLLOWED BY THE BLUE HYDRAULIC SYSTEM QUANTITY AND PRESSURE DECREASING TO ZERO. AIRCRAFT RETURNED TO MSP AND LANDED WITHOUT INCIDENT. MAINTENANCE FOUND THE LEFT HAND NR 3 SPOILER ACTUATOR RUPTURED. REPLACED ACTUATOR. LEAK AND OPERATIONAL CHECK OK.
DURING CLIMBOUT AFTER LANDING GEAR RETRACTION, LANDING GEAR DOOR INDICATED UNLOCKED. CYCLED LANDING GEAR HANDLE AND ALL INDICATIONS WERE NORMAL. AIRCRAFT CONTINUED TO ORIGINAL DESTINATION AND LANDED WITHOUT INCIDENT. MAINTENANCE REPLACED THE RIGHT MAIN LANDING GEAR DOOR PROXIMITY SENSOR AND OPERATIONAL CHECK OK.
WHILE CLIMBING THROUGH 15,000 FEET MSL, RECEIVED F/CTL ADR DISAGREE AND F/CTL ALTN LAW (PROT LOST) MESSAGES. FOLLOWED COCKPIT OPERATING MANUAL PROCEDURES AND CONTINUED ON TO ORIGINAL DESTINATION WHERE AN EMERGENCY WAS DECLARED AND AN UNEVENTFUL LANDING ACCOMPLISHED. MAINTENANCE REPLACED THE NR 1 AIR DATA COMPUTER. OPERATIONAL CHECK OK.
DURING C-CHECK, THE NR 1 ENGINE FORWARD ENGINE MOUNT THRUST LINKS WERE FOUND WITH WEAR MARKS IN THE CRITICAL AREA AS DEFINED BY ROHR CMM 71-22-07. THE MAXIMUM DEPTH OF THE DAMAGED AREAS WAS 0.002 INCH. REPAIRED PER NWA EA 32-96336.
DURING C-CHECK, THE NR 1 ENGINE FORWARD ENGINE MOUNT THRUST LINKS AND THE NR 2 ENGINE FORWARD INBOARD THRUST LINK WERE FOUND WITH WEAR MARKS IN THE CRITICAL AREAS AS DEFINED BY ROHR CMM 71-21-06, FIG 803. THE DAMAGED AREAS RANGED IN DEPTH FROM 0.001 INCH TO 0.002 INCH. REPAIRS ACCOMPLISHED PER NWA EA 32-96336 AND EA 32-96337.
DURING 'C' CHECK, FOUND THE LEFT FUSELAGE BELLY FAIRING CONNECTING ANGLE FOR PANEL 193B CRACKED. THE CRACK, EMINATES FROM THE FORWARD MOST EDGE TO THE 3RD FASTENER HOLE APPROXIMATELY 6 INCHES AFT OF THE FORWARD EDGE. THE ANGLE IS ATTACHED TO THE LEFT OVERWING EMERGENCY SLIDE PACK. REPAIRED PER NWA EA 32-095314.
