A CAUTION MESSAGE OF OVERLIMIT DETECTED SHOWING ON BOTTOM OF VEMD. LOWERED POWER & DIVERTED TO KRVS, SUCCESSFUL LANDING MADE AT MAINT FACILITY WITHOUT INCIDENT. ON FURTHER INSPECT, VEMD MAINT PAGE INDICATED AN OVERLIMIT ON NG OF 139.9% FOR 8 SECONDS. NO AUDIBLE TONE INDICATING AN EXCEEDANCE HEARD AT ANY TIME. A SUBSEQUENT GROUND RUN DONE WITH A MAXIMUM NG VALUE OF 88% OBSERVED ON THE FLI, BUT THE MAINT PAGE CAPTURED A MAX VALUE OF 129.6 NG. ERRONEOUS NR SENSOR READING, REPLACED VEMD. ORIG SUBMISSION DATE: 10/4/2025.
STARTER GENERATOR INSPECTION OVERFLOWN
ON START UP THE ENGINE CHIP LIGHT ILLUMINATED AND REMAINED ON.
FOUND HORIZONTAL STABILIZER UPPER MOUNT BRACKETS CRACKED ON PRE-FLIGHT INSP.
STAR GEN FAILED ON APPROACH. WOULD NOT RESET.
25 MINUTES INTO THE FLIGHT ON A REQUEST, PILOT/MED-CREW STARTING SMELLING A FOREIGN ODOR RESEMBLY SOMETHING BURNING; NO VISUAL SMOKE OBSERVED. DIVERTED TO THE NEARESET LOCATION KRVS. CHECKED OUT BY MX AT THE MX HANGAR AND NOTHING WAS FOUND AT THIS TIME.
ACFT MADE A PRECAUTIONARY LANDING IN A FIELD DUE TO MRGB OIL PRESSURE WARING LIGHT. PULLED AND CLEANED SWITCH. NO DEFECTS NOTED. ACFT WAS RUN UP AND OPS CHECK WAS NORMAL.
ENGINE EXHAUST PIPE HAS 2.5" CRACK RUNNING LATERALLY AROUND FORWARD WELD AT THE 1 TO 3 O'CLOCK POSITION.
MECHANIC DISCOVERED CRACKS IN THE MAIN ROTOR VIBRATION ABSORBER BOXES THAT ENCASE THE SPRINGS & LEAD WEIGHTS. ALL THREE BOXES HAD CRACKS IN THEM ON THE TRAILING UPPER EDGE. THESE ORIGINATED FROM THE INBD PORTION & EXTRUDING OUTWARD ON ALL THREE BOXES.
UPON PREFLIGHT OF ACFT, RUBBER & METAL SHARDS WERE OBSERVED PROTRUDING FROM M/R SWASH PLATE. ACFT WAS REMOVED FROM SERVICE & TRAILERED TO MX. UPON FURTHER INSPECTION OF SWASH PLATE & REMOVAL OF GREASE COVER FROM SWASH PLATE, METAL SHARDS WERE CAUSED BY THE METAL RETAINING RING UNDER THE SWASH PLATE GREASE COVE & GUIDE BEING SHAVEN BY THE UNI-BALL. THE RUBBER SHARDS WERE CAUSED BY THE METAL SHARDS AS THEY EXITED THE SWASH PLATE GUIDE.
RT CABIN VIBRATION DAMPER ARM (BLADE) BROKE APPROX 3.5 INCHES INBD FROM THE WEIGHT. FAULT WAS FOUND DURING A SCHEDULED MX INSPECTION.
PILOT REPORTED FUEL ODOR IN CABIN/COCKPIT DURING FLIGHT. INSPECTED THE ENGINE, TRANSMISSION AREA, FUEL SYSTEM INCLUDING TANK AREA, UNDERSIDE OF ACFT, BAGGAGE AREA'S, AND COCKPIT/CABIN AREA FOR SIGNS OF FUEL AND/OR FUEL ODOR, LOOSE LINES, OR DAMAGE. NONE FOUND. TEST FLEW ACFT WITHOUT DUPLICATING ODOR. RE-INSPECTED ACFT FOR SIGNS OF FUEL. NONE FOUND. RETURNED ACFT TO SERVICE.
