YAW ACTUATOR LIGHT ILLUMINATED ON AUTO PILOT TEST. INSPECTED, CLEANED, AND RESEATED THE YAW SEMA CANNON PLUG PROBLEM WENT AWAY.
DURING START UP PILOT REPORTED POSSIBLE OVER TEMP OF ENGINE. AFTER FURTHER INSPECTION FOUND A BAD TEMP PROBE.
DURING DECENT THE VARTOMS SYSTEM DISENGAGED. CAUSING THE NUMBER 1 ENGINE TORQUE TO DECREASE. THE PILOT WAS UNABLE TO GET THE TORQUES MATCHED MANUALLY. ABORTED THE LANDING AND PROCEEDED TO AN AIRPORT TO PERFORM A PRECAUTIONARY LANDING. AFTER EXTENSIVE TROUBLESHOOTING AND CONTACT WITH TECH SUPPORT, WE COULD NOT DUPLICATE THE ISSUE OR FIND ANY FAULTS WITH THE SYSTEM.
#2 ENGINE CHIP LIGHT AFTER START. BURNT CHIP AND IT CAME BACK. SHUTDOWN AND CALLED MX
DURING CRUISE #1 ENGINE WARNING. AFTER 2 ATTEMPTS TO BURN THE LIGHT REMAINED ON. POWER WAS REDUCED AND PLANNING FOR A PRECAUTIONARY LANDING BEGAN. THE ENGINE WAS THEN SHUT DOWN AND A RUN-ON LANDING WAS PERFORMED WITH NO FURTHER INCIDENT. AFTER INSPECTION THE ENGINE WAS DETERMINED UNAIRWORTHY AND WAS REMOVED AND REPLACED.
ENGINE OVER TEMP ON START. T4 CIRCUIT BOARD TT UNKNOWN
1&2 NP AND ROTOR RPM INDICATOR (TRIPLE TAC) WAS INOP DURING THE START SEQUENCE.
DURING FLIGHT, ACTUATOR CAUTION APPEARED ON CAD. PILOT DECIDED TO LAND AT BMI AIRPORT INSTEAD OF HIS DESTINATION AT HOSPITAL. COULD NOT DUPLICATE FAULT AFTER A POWER CYCLE.
ENROUTE TO HOSPITAL, THERE WAS A BRIGHT FLASH IN COCKPIT AND A BURNED ELECTRICAL ODOR. AIRCRAFT TURNED BACK TO BASE AND CONTACTED FLIGHT COM. AFTER A FEW MOMENTS THE SMELL CEASED SO THEY CONTINUED BACK TO BASE. BOUT I MINUTE FROM LANDING ANOTHER FLASH APPEARED IN THE COCKPIT FROM THE OVERHEAD CIRCUIT BREAKER PANEL AND THEY LANDED AT THE AIRPORT AND CONTACTED MAINTENANCE. MAINTENANCE FOUND THE WIRING FOR THE CIRCUIT PANEL BACKLIGHTING WAS PINCHED BETWEEN THE PANEL AND THE AIRFRAME CAUSED BY WORK BEING DONE IN THAT AREA EARLIER THAT DAY. THE WIRING WAS REPAIRED AND REROUTED TO PREVENT THIS FROM HAPPENING AGAIN. THE AIRCRAFT WAS RETURNED TO SERVICE.
HYDRAULIC SYSTEM WARNING ON CWP SYSTEM PRESSURE INDICATED ZERO AND LANDING GEAR INDICATED UNSAFE. REPLACED THE HYDRAULIC SYSTEM PUMP.
NOSE GEAR ASSIST SPRING BROKEN. REPLACED NOSE UPLOCK HOOK RETRACT SPRING.
LEFT PROPELLER, BLADE EROSION GUARDS, ARE CRACKED AND DEBONDING. REPLACED PROPELLER.
RT PROPELLER RPM DROPPED TO 1340 DURING LANDING. REPLACED THE PROPELLER PISTON DOME O-RING.
RT TAIL PIPE HOT WARNING ILLUMINATED DURING TAKEOFF. DRIED MOISTURE ON THE TAIL PIPE HEAT SENSORS. ACFT RETURNED TO SERVICE.
LEFT ENGINE SHUT DOWN IN-FLIGHT. TORQUE WAS ERRATIC FOR 3 SECONDS AND THEN SHUT DOWN BY ITSELF. PROBLEM COULD NOT BE DUPLICATED. ENGINE WAS INSPECTED AND NO DEFECTS NOTED. THE ELECTRONIC CONTROL UNIT WAS REPLACED AS A PRECAUTIONARY MEASURE. PART TOTAL CYCLES, 15,108.
RIGHT ENGINE TAIL PIPE HOT WARNING SHORTLY AFTER TAKEOFF. COULD NOT DUPLICATE DISCREPANCY. GROUND RUNS WERE NORMAL AND ACFT RETURNED TO SERVICE.
WHEN THE RIGHT ENGINE POWER LEVER WAS ADVANCED DURING CRUISE, THE ENGINE SURGED TO 95 PERCENT. REPLACED BINDING RT ENGINE POWER LEVER CABLE.
RT ENGINE ITT INDICATION IS ERRATIC. REPLACED THE ITT THERMOCOUPLE HARNESS ASSY.
DURING ENGINE START PILOT'S EFIS TUBES WENT BLANK AND SMOKE WAS NOTICED FROM GLARESHIELD AREA. REPLACED THE PILOT'S EAD2-EFD-86.
THE LEFT ENGINE WOULD NOT ROTATE DURING START CYCLE. REPLACED THE LEFT ENGINE STARTER/GENERATOR.
NOSE LANDING GEAR DOWN AND LOCKED INDICATOR IS INOP. REPLACED THE LANDING GEAR CONTROL HANDLE ASSY.
WITH CTOT ENGAGED, THE RT ENGINE TORQUE ADVANCED TO 120 PERCENT. PLACED CTOT SYSTEM ON DEFERRED MAINTENANCE LIST AND AIRCRAFT CONTINUED SERVICE.
NOSE WHEELS TURNED 180 DEGREES TO THE LEFT. REPOSITIONED NOSE WHEELS, INSPECTED SYSTEM AND FOUND TO BE SERVICEABLE. AIRCRAFT CONTINUED SERVICE.
MECHANIC REMOVED WHEEL ASSEMBLY FROM A/C DUE TO TIRE WORN TO LIMITS. DURING INSPECTION, FOUND INNER RACE HAD SPUN. SUBASSEMBLY HAS BEEN REMOVED FROM SERVICE AND SENT TO ABS FOR BUSHING REPAIR.
WHEN RT POWER LEVER IS ADVANCED ABOVE FLIGHT IDLE, ENGINE OVERSPEEDS. ADJUSTED POWER LEVER CONTROLS AIRFRAME RIGGING TO M/N SPECS. SYSTEM OPERATES NORMAL.
MECHANIC REMOVED WHEEL ASSY FROM A/C DUE TO TIRE WORN TO LIMITS. DURING VISUAL INSPECTION FOUND INNER RACE HAD SPUN. SUBASSY HAS BEEN REMOVED FROM SERVICE AND SENT TO ABS FOR REPAIR.
MECHANIC REMOVED WHEEL ASSY FROM A/C DUE TO TIRE WORN TO LIMITS. DURING VISUAL INSPECTION FOUND INNER RACE HAD SPUN. SUBASSY HAS BEEN SENT TO ABS FOR REPAIR.
RUDDER LIMITER WARNING ACTIVATED. REPLACED DEFECTIVE WIRING CONNECTOR ON RUDDER LIMITER OVERRIDE ACTUATOR.
RT ENGINE FAILED DURING CLIMB. REPLACED THE ENGINE ASSY.
