FOLLOWING AN INVESTIGATION FOR A MISSING GROMMET ON A ELEVATOR CABLE FAIRLEAD AT BS999 IT WAS IDENTIFIED THAT THE FAIRLEAD WAS REWORKED AND EXTREMELY CLOSE TO TURNBUCKLES, AIRCRAFT AIPC SHOWED NO FAIRLEAD P/NO: INSTALLED AT THIS LOCATION. AIRCRAFT CARGO CONVERSION COMPANY ASKED FOR MORE INFORMATION ON FAIRLEAD. CARGO CONVERSION COMPANY RESPONSE GAVE LOCATIONS OF FAIRLEAD AND IDLER PULLEYS FOR ELEVATOR AND RUDDER CABLES AT MULTIPLE BS LOCATIONS FAIRLEADS MISSING, FAIRLEADS ALSO IDENTIFIED INSTALLED AT INCORRECT LOCATIONS PER CONVERSION DRAWINGS SUPPLIED. A NUMBER OF MISSING IDLER PULLEYS BETWEEN BS654 AN BS1219 ALSO IDENTIFIED FOLLOWING INSPECTION. ELEVATOR CABLE DAMAGE EVIDENT FROM INSPECTION.
DURING INSPECTION OF THE CARGO FIRE BOTTLE IT WAS IDENTIFIED THE PRESURE SENSING SWITCHES WERE NOT INSTALLED AS PER AMM INSTRUCTION. THE PRESSURE SENSING SWITCHES WERE FOUND DISCONNECTED FROM THEIR CORRECT INSTALLED POSIITONS.
DURING A FWD ENTRY DOOR CLOSING OPERATION TASK, THE VIEWING WINDOW/PRISM WAS IDENTIFIED NOT INSTALLED CORRECTLY PER AMM. THE VIEWING WINDOW/PRISM WAS DISLODGED FROM ITS CORRECT INSTALLED POSITION AND THE RETAINING HARDWARE IDENTIFIED AS NOT WITHIN AMM REQUIRED FASTENER TORQUE TOLERANCES.
R/H WING, OVERWING EXTERIOR EMERGENCY LIGHTS INOP. REMOVED AND REPLACED POWER SUPPLY M731 IAW MM 33-51-07. OPS CHECK GOOD. IAW MM 33-51-00.

AIRCRAFT WAS GROUNDED: PILOT REPORTS WHILE THROTTLE BOTH CLOSED, DURING DECENT LEFT ENGINE DID NOT GO TO IDLE LIKE RIGHT ENGINE (WHICH WAS NORMAL) POWER WAS CONFIRMED AS HIGH. CYCLED ENGINE ANTI ICE SEEMED TO CLEAR THE ISSUE. MAINTENANCE REMOVED AND REPLACED LEFT ENGINE TAI T/D CONTROL RELAY IAW FIM 30-21-00-815-806. ACCOMPLISHED FLT/GRD IDLE SOLENOID TEST IAW AMM 73-21-04-725-004 AND APPROACH IDLE CHECK IAW AMM 71-00-00 TEST #3 OK FOR SERVICE.
R1 DOOR AND OVERWING EXTERIOR LIGHTS WERE INOPERATIVE DURING TEST. REPLACED THE CHARGER MODULE AND BATTERY IAW AMM 33-51-07.
AIRCRAFT IN BASE MAINTENANCE: RIGHT HAND DOOR 1 AND OVERWING EXTERIOR LIGHTS WERE INOPERABLE DURING TEST. REPLACED CHARGER MODULE (SEE AALA20190211002) AND BATTERY WITH SERVICEABLE UNITS PER AMM 33-51-07.

AIRCRAFT WAS GROUNDED. PHL-BUD DIVERTING JFK ACCOUNT COMPRESSOR STALL DUE TO A POSSIBLE BIRD INGESTION. AN EMERGENCY WAS DECLARED AND THEY LANDED OVERWEIGHT. DURING CLIMB THE "LEFT BLEED OVHT" LIGHT ILLUMINATED. FOLLOWED QRH PROCEDURES AT CRUISE LEVELLING OFF COMPRESSOR STALL TO LEFT ENGINE. EGT NORMAL. #1 ENGINE REPLACED PER MCM 71-93-01.

THE AIRCRAFT WAS GROUNDED. DURING TROUBLESHOOTING, FOUND A BAD WIRE SPLICE SM7, AND SENSOR S277. REPLACED THE SENSOR , AND ACCOMPLISHED LEADING EDGE OPS CHECK IAW MM 27-88-00. ALL OPS NORMAL.
AFTER TAKE OFF, AND DURING CLIMB OUT, EICAS MESSAGE LEADING EDGE SLAT ASSEMTRY MESSAGE ILLUMINATED. EMERGENCY DELCARED AND AC RETURNED TO THE FIELD, FLIGHT LANDED WITHOUT ISSUE. THE AIRCRAFT WAS GROUNDED. DURING TROUBLESHOOTING, FOUND A BAD SPLICE SM7 AND SENSOR S277. REPLACED THE SPLICE SM7 IAW WDM 27-88-12, AND ACCOMPLISHED LEADING EDGE OPS CHECKED IAW MM 27-88-00. ALL OPS NORMAL.
LEADING EDGE SLAT A59M DISPLAYED INTERMITTENTLY AND AT SAME TIME FLAP INDICATOR WENT BETWEEN O AND 1. DECLARED AN EMERGENCY AND DIVERTED WITHOUT FURTHER INCIDENT. THE AIRCRAFT WAS GROUNDED. MAINTENANCE REPLACED LE SLAT SENSOR SWITCH IAW AMM 27-88-00. ACCOMPLISHED ALL REQUIRED TEST.

THE ACFT WAS GROUNED. MAINTENANCE FOUND THE LEFT ENGINE SPINNER AND ALL FAN BLADES DAMAGED. REPLACED THE FAN BLADES AND SPINNER IAW MM 72-31-02, AND 72-31-01. ACCOMPLISHED ENGINE FAN TRIM BALANCE IAW MM 72-31-00.
