DAMAGE TO THE LH BEAM UNDER THE ENGINE DECK (P/N: 355A21-2110.25) IN THE FORM OF AN ELONGATED HOLE IN THE LOWER FLANGE NEAR THE AFT END OF THE PART. THE ORIGINAL HOLE IS 1/8â DIAMETER, AND THE ELONGATED HOLE WAS MEASURED TO BE APPROXIMATELY 0.4â X 0.2â.
CHAFING DAMAGE WAS DISCOVERED ON THE TRANSMISSION DECK. THE DAMAGE MEASURES APPROXIMATELY 0.3â LONG, 0.1 WIDE WITH A MAXIMUM DEPTH OF 0.010".
CHAFING DAMAGE TO THE AFT SIDE OF THE AFT CABIN BULKHEAD (PN 350A21-1032-86). ADDITIONALLY, THERE IS CHAFING DAMAGE TO THE VERTICAL SUPPORT (PN 350A21-1032-32).
CHAFING DAMAGE WAS DISCOVERED ON THE TRANSMISSION DECK AT APPROXIMATE STATION X 3900, Z 1759, R/H Y 210. THE DAMAGE IS APPROXIMATELY 0.014â DEEP AND 1.2â LONG BY 0.2â WIDE.
DAMAGE TO THE ENGINE DECK (PN: 350A21-1239.21) IN THE FORM OF SEVERAL DENTS. THE DAMAGE AREAS WERE DISCOVERED TO BE NEAR THE SCUPPER DRAIN HOLE AND AFT ON EITHER SIDE OF A STIFFENING FEATURE. THE LARGEST DENT WAS MEASURED TO BE APPROXIMATELY 1.20â X 0.20â AND 0.030â DEEP.
WHILE REMOVING CORROSION FROM THE UNDERSIDE OF THE LEFT SIDE OF THE COCKPIT FLOOR BELOW THE LITTER BASE MOUNT, FOUND CORROSION ON THE FRAME AT BS 1294.20.
DURING INSPECTION, FOUND CORROSION ON THE LEFT & RIGHT EDGING OF THE FUSELAGE TO THE EXTENT THAT REPLACEMENTS HAD TO BE SPLICED IN.
GENERATOR LIGHT ILLUMINATED IN CRUISE FLIGHT FOLLOWED IMMEDIATELY BY A STRONG ODOR OF BURNING AND VISIBLE SMOKE IN THE COCKPIT. ELECTED TO LAND. STARTER GENERATOR WAS ON FIRE.
DURING FLIGHT OPERATIONS FLIGHT CREW STATED THAT WHEN THEY ENGAGED THE AIR CONDITIONING THERE WAS AN ELECTRICAL TYPE SMELL AND WHEN THE AIR CONDITIONING WAS DISENGAGED THE SMELL WOULD DISIPATE . THE ACFT DID NOT HAVE ANY WARNINGS OR CAUTIONS SO THEY PROCEEDED TO HOME BASE. DURING FURTHER INVESTIGATION BY THE TECH ON DUTY IT WAS FOUND THAT THE AIR CONDITIONING COMPRESSOR CLUTCH WAS MALFUNCTIONING AND CAUSING A MOMENTARY SEIZURE OF THE BELT WHICH WAS COUSING THE BURNING SMELL. THE ITEM WAS PLACED ON MEL AND A NEW COMPRESSOR ASSY ORDERED.
