CREW SMELLED FUEL IN CABIN DURING FLIGHT. PLANNED FLIGHT WAS FROM OMAHA NE TO NORTH PLATTE NE. PILOT HAD THE AIRCRAFT TOP OFF WITH FUEL BEFORE THIS EVENT HAPPENED. WHEN CREW SMELLED FUEL IN CABIN PILOT LANDED AIRCRAFT. MECHANIC WAS CALLED IN. LOG BOOK ENTRY WAS MADE (SMELLED FUEL IN CABIN DURING FLIGHT). CORRECTIVE ACTION IS (DID VISUAL INSPECTION OF THE ENGINE BAY AND FUEL LINES IAW AMC BELL 407GX DMC-407-A-28-00-00-00A-042A-A. PERFORMED GROUND RUN COULD NOT DUPLICATE AND CABIN SMELLED CLEARED. THIS AIRCRAFT HAS BEEN RETURNED TO SERVICE.) THE AIRCRAFT FLEW BACK TO NORTH PLATTE WITH NO SIGNS OF FUEL SMELL. SUSPECTING THAT THE FUEL SMELL WAS CAUSE BY THE TANK BEING FULL OF FUEL AND A COMBINATION OF THE WIND BLOWING THE FUEL VAPORS BACK INTO THE CABIN.
INTERMITTENT TRANSMISSION CHIP LIGHT. TRANSMISSION CHANGED OUT AT ACFT TT 2605+37 ON DATE 4/21/2022. INSPECTED CHIP DECTETECTORS FOUND SMALL A SLIVER OF METAL ON LWR CHIP DETECTOR. CLEANED ALL CHIP DETECTORS. DRAINED AND RESERVICED TRANSMISSION FUILDS. REMOVED, INSPECTED TRANSMISSION FILTER AND INSTALLED NEW FILTER. SENT SLIVER METAL OUT FOR LAB ANALYSIS.
JUST AFTER TAKE OFF FROM THE HOSPITAL HELIPAD, DURING THE INITIAL CLIMB TO CRUISE ALTITUDE, THE XSMN CHIP CAUTION LIGHT ILLUMINATED. ALTITUDE WAS APPROXIMATELY 700 FEET, AIRSPEED OF 90 KTS WITH CLIMB-OUT POWER APPLIED. FORTUNATELY, WE WERE JUST PASSING THE APPROACH END OF RUNWAY 33 AT GRANT MUNICIPAL AIRPORT. I ENTERED A SHALLOW LEFT DESCENDING TURN AND LANDED DIRECTLY TO THE PARKING AREA. THERE WERE NO OTHER ABNORMAL INDICATIONS AND THE AIRCRAFT WAS SHUT DOWN WITHOUT INCIDENT. TOTAL ELAPSED TIME FROM TAKE OFF TO LANDING WAS 4 MINUTES. THE PATIENT AND MEDICAL STAFF TRANSITIONED TO A GROUND VEHICLE TO CONTINUE TO THE RECEIVING HOSPITAL. MAINTENANCE INSPECTED THE CHIP PLUGS AND FOUND LARGE METAL PARTICLES ON THE LOWER CHIP PLUG. THE TRANSMISSION WILL HAVE TO BE REPLACED. *** THIS AIRCRAFT REMAINS OUT OF SERVICE AT THIS TIME AWAITING A SERVICEABLE REPLACEMENT MAIN TRANSMISSION FOR INSTALLATION. ***
