OUTER COMBUSTION CASE CRACKED IN RT ARMPIT AREA ALONG SPLITLINE WELD. THIS AREA HAD THE BRAZED REINFORCEMENT.
PILOT COMPLAINED OF NOISE AND VIBRATION ON SHUT DOWN. INSPECTED MAIN ROTOR HUB, FOUND ROLL PINS ON DROOP STOP FOLLOWER ASSEMBLIES HAD BACKED OUT AND WERE CONTACTING THE LOWER SHOE. THE INTERFERENCE WAS CAUSING THE FOLLOWERS TO BE FROZEN IN PLACE. WHEN THE AIRCRAFT WAS SHUT DOWN AND THE ROTOR BLADES CONTACTED THE ROLLERS. THEY ONLY APPLIED A FORCE TO A FROZEN ITEM. THIS MADE A VERY LOUD NOISE, ALL FOLLOWERS AND BUSHINGS WERE REPLACED AND MFG WAS NOTIFIED.
INSTRUMENT PANEL LIGHT DIMMER CONTROL FAILED.
TAIL ROTOR GEARBOX IS MAKING METAL.
EXHAUST PIPE IS CRACKED APPROXIMATELY 4 INCHES, AFT, INBOARD CENTER.
BLEED VALVE STICKING CAUSING COMPRESSOR STALLS.
EPO BOX HAS ENGINE OUT LIGHT AND WARNING HORN ACTIVATED DURING FLIGHT. REMOVED AND REPLACED.
ENGINE HAD LOW POWER DETERMINED THAT THE BLEED VALVE WAS NOT FULLY CLOSING. REMOVED AND REPLACED.

DURING ROUTINE INSPECTION, MECHANIC FOUND METAL AND EXCESSIVELY DARK OIL IN GEARBOX. REMOVED FOR REPAIR AND REPLACED WITH SERVICEABLE UNIT. CONTROL NR 98-4-8.
PILOT LANDED AIRCRAFT AFTER T/R CHIP LIGHT CAUTION LIGHT ILLUMINATED. TAIL ROTOR CHIP DETECTOR WAS INSPECTED. A LARGE AMOUNT OF METAL ACCUMULATION WAS EVIDENT. TAIL ROTOR GEARBOX WAS REMOVED AND REPLACED AND SENT FOR EVALUATION. UPON TEARDOWN INSPECTION, ONE TOOTH ON INPUT DRIVE GEAR WAS FOUND BROKEN. FURTHER INVESTIGATION IS IN PROGRESS.
COMBUSTION TURBINE LINER CHAFED, FUEL NOZZLE SHROUD DAMAGED. FOUND DURING 100-HOUR INSPECTION. FUEL NOZZLE WAS REPLACED AND SENT FOR REPAIR. S/N AG55376. CONTROL NR 98-3-5.
PHASING OF ENGINE OUT-SWITCHING ON N1 NOT OPERATING PROPERLY.
