NTSB accidents & incidents
12 records, spanning 1999–2023.
Operated by HAVERFIELD INTERNATIONAL INC (per NTSB report)
The pilot’s failure to maintain clearance from a power line structure arm, which resulted in main rotor blade contact.
Operated by Haverfield International (per NTSB report)
The pilot's failure to maintain adequate clearance from the power transmission tower structure during powerline maintenance work, which resulted in the helicopter's main rotor striking the lifeline support bracket for the tower catwalk and the subsequent loss of control and collision with terrain.
Operated by Haverfield International Incorporated (per NTSB report)
The loss of engine power due to fuel starvation as a result of unporting of the fuel tank supply pickup while the helicopter was maneuvered to move a conductor wire. Contributing to the accident was the helicopter’s inappropriate configuration for the type of operation being conducted, which impeded the pilot’s ability to release the long line and perform a successful emergency landing.
Operated by HAVERFIELD INTERNATIONAL INC (per NTSB report)
The pilot’s failure to maintain adequate clearance from transmission power wires, which resulted in a tail rotor strike.
Operated by HAVERFIELD INTERNATIONAL INC (per NTSB report)
The unporting of fuel due to the helicopter's nose-up attitude during long line operations, which resulted in fuel starvation and a total loss of engine power.
Operated by HAVERFIELD INTERNATIONAL INC (per NTSB report)
The pilot's excessive descent rate while flaring during a practice autorotation with a power recovery, which resulted in an impact with terrain.
Operated by HAVERFIELD INTERNATIONAL INC (per NTSB report)
The pilot's spatial disorientation during a low-level river crossing in rainy, gray, reduced visibility meteorological conditions, which resulted in the helicopter's subsequent descent and impact with the water. Contributing to the accident was the pilot's decision to attempt the low-level crossing under those meteorological conditions.
Operated by HAVERFIELD INTERNATIONAL INC (per NTSB report)
An undetermined pilot's failure to maintain clearance from an electrical transmission tower resulting in substantial damage to a main rotor blade. Postaccident investigation could not determine which of two pilots was flying the helicopter at the time the damage occurred.
Operated by Haverfield International Inc. (per NTSB report)
The pilot’s failure to maintain adequate altitude while conducting a power line aerial observation flight, which resulted in an in-flight collision with wires. Contributing to the accident was the pilot’s lack of total experience in the type of operation.
Operated by HAVERFIELD INTERNATIONAL INC (per NTSB report)
The fatigue failure of a blade in the second-stage turbine wheel, which resulted in a total loss of engine power.
The loss of tail rotor effectiveness and the pilot's failure to regain aircraft control. Contributing to the accident was the pilot's decision to fly in known adverse weather conditions and the gusty winds generated from convective outflow. Contributing to the severity of the injuries was the pilot's failure to provide a safety briefing to his passengers in accordance with Federal Aviation Regulations.
During a practice autorotation, the pilot flared the helicopter too low, which resulted in the tailboom contacting the ground. A factor was the dark night light conditions.

Maintenance disclosure history
Service Difficulty Reports are self-reported maintenance findings. The NUMBER of reports is not a measure of reliability — diligent operators file more, not fewer. These are shown for transparency, not as a negative signal. To verify a report, search its control number at the FAA SDRS site.
Reports span 2010–2014. Most-reported systems: Engine fuel & control (73) — 2; Landing gear (32) — 1; Engine (72) — 1. Most-common natures of condition, as filed: OTHER — 2; NO TEST — 1; WARNING INDICATION — 1.

Part: CHIP DETECTOR — CONTAMINATED (ENGINE) · airframe 3,284 hrs
DURING A RTB FLIGHT AT 700 AGL, ACFT MADE A PRECAUTIONARY LANDING ON A FOOTBALL FIELD DURING A NONSTOP FLIGHT DUE TO MASTER CAUTION LIGHT WITH A CORRESPONDING ENGINE CHIP INDICATION. NO ABNORMAL ENGINE PERFORMANCE WAS NOTED AND ENGINE OIL TEMPERATURES AND PRESSURES WERE IN NORMAL OPERATING LIMITATIONS. PILOT TROUBLESHOT THE INDICATING SYSTEM AND FOUND THAT THE LOWER ENGINE CHIP DETECTOR WAS THE LOCATION OF THE LIGHT. REMOVED THE CHIP DETECTOR AND FOUND A SMALL METALLIC CHIPS ON THE DETECTOR ALLOWABLE IAW MM 72-00-00, PARA 11 (G). CLEANED THE DETECTOR, PERFORMED A 30 MINUTE GROUND RUN IAW MM REQUIREMENTS, WITH NO OTHER INDICATION OCCURRING. PERFORMED A CHECK FLIGHT WITH NO FURTHER INDICATION. A RETURN TO SERVICE WAS VERIFIED, CONTINUED WITH THE FLIGHT WITH NO FURTHER ISSUES.
Part: SHAFT — SEIZED (GOVERNOR) · airframe 10,925 hrs
GOVERNOR SHAFT SEIZED. OVERHAUL TEARDOWN REVEALED MOST INTERNAL PARTS WORN EXCESSIVELY.
Part: GOVERNOR — MALFUNCTIONED (ENGINE)
PILOT WAS HOVERING, WHEN SUDDENLY THE RPM STARTED TO OVERSPEED. RPM CONTROL WAS MINIMIZED WITH THROTTLE INPUT UNTIL AN UNEVENTFUL LANDING WAS AFFECTED IN A NEARBY BALLFIELD. REPLACED GOVERNOR WITH A SERVICEABLE UNIT AND RECTIFIED THE PROBLEM.
Part: SKID — CRACKED (MLG)
DURING A 100 HR INSP, A CRACK WAS FOUND ON THE RT FWD LANDING GEAR FOOT. THE CRACK LOOKS TO HAVE ORIGINATED AT THE LOWER END, OTBD SIDE AT THE TOP OF THE BOSS AREA JUST ABOVE THE SKID TUBE MOUNT HOLES. THE CRACK EXTENDS UP ABUT 1" AND SPIDERS FWD AND AFT. THE AFT CRACK EXTENDS FOR 1 " AFT AND UPWARDS. THE FWD CRACK EXTENDS UP, CIRCLING THE TUBE FOR APPROX 12". THE ACFT IS ON EXTENDED LANDING GEAR.
