ODOR OF SMOKE DETECTED AFTER TAKE OFF SHORTLY THEREAFTER THE FLIGHT DECK LOCK CIRCUIT BREAKER WAS DISCOVERED TO HAVE POPPED. CORRECTIVE ACTION: REMOVED AND REPLACED FLIGHT DECK DOOR ELECTRIC STRIKE PER B767 AMM 52-51

LE SLAT LIGHT (EICAS) MESSAGE ILLUMINATED. CORRECTIVE ACTION: BIT FSEU, M548 OUTBOARD SLAT PDU RVDT MISRIG MESSAGE FOUND. PERFORMED PSEU SENSOR (SLATS) CHECKS, NO FAULTS FOUND. ACCESSED OUTBD SLAT PDU RVDT CONNECTORS, CLEANED THEM, SECURED THEM. RUN SLATS UP AND DOWN, CHECKED FSEU FOR FAULTS, NO FAULTS. RUN FLAPS/SLATS SEVERAL TIMES, NO DEFECTS NOTED. VERIFY FSEU FOR FAULTS, NONE FOUND. OK FOR SERVICE (REF FIM 27-51 FAULT TREE 810).
MASTER CAUTION ACCESS DOORS CARGO DOORS ENTRY DOORS L/R CABIN ALTITUDE WAS RISING FROM 8,500 FT TOWARDS 9,200 FT AT A RATE OF 500 FT / ??? WE EXECUTED AN IMMEDIATE DESCENT TO 14,000 FT AT WHICH POINT ALL EICAS MESSAGES CLEARED MAX CABIN DIFFERENCIAL REACHED 8.7 CABIN ALTITUDE MASTER WARNING LIGHT AND EICAS MESSAGE DID NOT ILLUMINATE CABIN OXYGEN MASKS DID NOT DEPLOY CABIN ALTITUDE STABILIZED BRIEFLY AT 14,000 THEN BEGAN TO CLIMB AGAIN TOWARDS 9,500 FT THUS WE INDICATED FURTHER DESCENT TO 9,000 FT WE COMPLETED THE CARGO DOORS QRH (PAGE 1.4) AS THE FWD CARGO DOOR WAS THE HIGHEST PRIORITY COMPANY DIRECTED DIVERSION TO LBBG REST OF FLIGHT WAS COMPLETED UNEVENFULLY. R ENG BLEED OVERHEAT (EICAS MSG) EARLY ON CLIMB OUT ~ REMOVED AND ATTACHED WIRES IAW WDM 36-11 AND UNLOCKED FAMV FOR OPS CHECK IAW AMM 36-10. OPS CHECK OKAY.
AFTER T.O. WITH NORMAL GEAR RETRACTION STARTED AFTER T.O. CHK LIST. WHILE MOVING LANDING GEAR LEVER TO THE OFF POSITION, HEARD UNUSUAL NOISE WITH AMBER GEAR AND DOOR LIGHTS WITH GREEN NOSE GEAR DOWN AND LOCKED INDICATION. FOLLOWING QRH PROCEDURES MOVED GEAR LEVER TO THE UP POSITION. IN THE UP POSITION ALL LIGHTS PREVIOUSLY METIONED EXTINGUISHED. AFTER DUMPING FUEL AND MAX LANDING WEIGHT EXTENDED GEAR HANDLE TO THE DN POSITION. NORMAL EXTENSION WITH 3 GEAR DOWN AND LOCKED, NO AMBER INDICATIONS. PROCEEDED TO LAND WITH A NORMAL LANDING. JACKED AIRCRAFT PER B767 AMM 07-11-01. PERFORMED MULTIPLE LANDING GEAR EXTENSIONS AND RETRACTIONS IAW B767 AMM 32-32-00. NO DEFECTS NOTED AT THIS TIME. COULD NOT DUPLICATE MALFUNCTION.
AT 25000 FT STILL CLIMBING RIGHT ENGINE GAVE OUT A LOUD BANG FOLLOWED BY HIGH VIBRATION , LOW N1 AND HIGH N2. EGT APPEARED TO BE NORMAL. CORRECTIVE ACTION:RIGHT ENGINE REMOVED & REPLACED IAW OAI WC 71-R07-R
AFTER PUSHBACK AND BOTH ENGINES STARTED, RECIEVED NOTIFICATION FROM FROM FA LOCATED IN AFT CABIN THAT THERE WAS THE ODOR OF SMOKE. DURING THIS TIME A HIGH PITCH NOISE WAS HEARD THROUGHOUT CABIN. ENGINE SHUTDOWN AND NOISE ENDED. RAN BOTH ENGINES AND OPERATED HYDRALIC SYSTEM AND OPERATED AC PACK SYSTEM INSPECTED ALL SYSTEMS FOR SIGNS OF SMOKE OR ODOR. NO DEFECTES NOTED AT THIS TIME.
DURING FLIGHT, STATUS MESSAGE, RT ENGINE FUEL FILTER. DURING POWER REDUCTION AT TOP OF DECENT, EICAS MESSAGE WENT AWAY. R & R RT ENGINE FUEL FILTER LEAK AND OPS CK IAW AMM 73-11. OPS CK GOOD. PREFORMED ENGINE RUN IAW AMM 71-00. OPS CK GOOD. MSG'S CLEARED.
RIGHT OVERWING ESCAPE SLIDE AFT UPPER CORNER JAMBING AND BINDING. EVALUATION: SLIDE ON DOOR UPPER AFT CORNER IS GETTING STUCK ON DOOR LATCH ACTUATOR. R & R SLIDE PACK ASSEMBLY DUE TO BEING IMPROPERLY PACKED.
