AFTER PASSING 10,000 FT., INFORMED FROM CABIN OF SLIGHT FUEL LEAK IN OVERWING FUEL CAP. RT WING LEAK INCREASED AT 310. DECLARED EMERGENCY, DUMPED 24.5K FUEL, RETURN TO PGUA.
R HPSOV EICAS AND STATUS MESSAGE IN FLIGHT
AT CRUISE AUTOTHROTTLE AND AUTOPILOT DISCONNECTED. AUTO THROTTLE WOULD NOT RE-ENGAGE. AUTOPILOT L AND C INOP. R AUTOPILOT WORKED. LEFT AND FO FLIGHT DIRECTORS INOP. CAPT HEADING BUG INOP. CAPT SPEED BUG INOP. CAPT ALTITUDE SEL INOP.
PILOTS REPORTED MAIN BAT CHARGER HAD A STRONG BURNING SMELL.
AFTER TAKEOFF AT APROX 100' AGL HEARD A LOUD BANG AND AN ASSOC LEFT MOMENTARY YAW.
ON CLIMB AT 5000 FEET R RECIRC FAN EICAS. AT 6500 FEET LOUD BAND AND YAW AND R RECIRC FAN EICAS EXTINGUISHED. WE HAD A BURNING SMELL, ENGINE INDICATIONS NORMAL. VIBRATION ON DECENT BUT ENGINE INDICATIONS NORMAL. OIL SMELL BACK OF CABIN AFTER SHUTDOWN
REASSESS DENT 20 RH FUSELAGE STATION 368+6INCHES STRINGER 26+4INCHES. FOUND TO BE OUTSIDE OF ALLOWABLE DAMAGE LIMITS IAW B767-300 SRM 53-00-01-1A-1 REV 128.
GOUGES ON LH H-STAB LOWER INSPAR SKIN AT S-2 AND STA 226.605 (RIB 12 CENTERLINE).
DISBOND AROUND HOIST POINT ATTACH POINT L/H WING UPPER SURFACE.
DENT #13, REPAIRED ON STRINGER 19L BETWEEN STATION 368-388 IAW B767-300 SRM 53-00-03-2R-1 REV128.
DURING C CHECK, CORROSION WAS FOUND ON THE NR 2 ENGINE FWD AND AFT CASE AROUND THE EXTERIOR AND INTERIOR SKIN. IT WAS FOUND TO BE OUT OF REPAIRABLE LIMITS FOR THE FWD FAN CASE PER AMM 72-32-00 AND OUT OF FLYABLE LIMITS PER AMM 72-00-00. THE ENGINE WAS REMOVED AND SENT FOR REPAIR. WHILE AT GA TELESIS IT WAS FOUND BY THE ENGINE SHOP THAT THE AFT FAN CASE WAS ALSO BEYOND REPAIR. THE FWD AND AFT FAN CASE WERE CHANGED AT GA TELESIS ON WO#1155. AFTER REPAIR THE ENGINE WAS SENT BACK AND INSTALLED BACK ON N36NE AT THE #2 POSITION.
DENT NR 12 LT FUSELAGE STATION 368 +8 INCHES, STRINGER 17L +4 INCHES. DURING SCHEDULED MAINTENANCE, TWO DENT DAMAGES WERE FOUND COMMON TO THE LOWER FUSELAGE SKIN BETWEEN STA 368 - STA 390 AND STGR 17L - STGR 19L..
DURING C-CHECK AT YHM, THE RH HORIZONTAL STABILIZER AUXILIARY SPAR WEB NEAR OUTBOARD CLOSURE RIB AT HSBL 357.442 WAS FOUND TO HAVE TOOLING MARKS, AFTER BLENDING OUT OF THE DAMAGED AREAS, TWO CRACKS OF 0.300" AND 0.250" LONG WERE FOUND.
DURING ACCOMPLISHMENT OF INSPECTION 76AD2012-15-12-G, A CRACK WAS FOUND IN THE LH UNDERWING LONGERON. PER SB 767-57A0126 INSTRUCTIONS, THIS REQUIRES FURTHER INSPECTION OF THE STRUCTURE AND AT MINIMUM REPLACEMENT OF THE UNDERWING LONGERON.
AT 10,000 MSL, CREW O2 QTY DROPPED TO 70 PSI

ON APPROACH FLIGHT CONTROLS FELT AS THOUGH THEY WERE BINDING. FLIGHT CONTROLS ASYMETRY IS INDICATED ON INDICATOR.
DURING CRUISE FLIGHT AT FL370 LEFT ENGINE N1 ROLLED BACK WITH CORRESPONDING VIBRATIONS
RIGHT ENGINE OIL PRESSURE IN CAUTION RANGE AT TOP OF CLIMB. RIGHT ENGINE OIL QUANTITY ZERO. SHUT DOWN ENGINE AT OIL PRESSURE IN THE RED.
L CDU FAILURE, L FMC FAILURE: DURING FLIGHT CREW REPORT THE L FMC FAILED. DISCUSSIONS WITH MOC AND DO. ALL PARTIES AGREED TO CONTINUE. CREW CALLED BACK 1 HOUR LATER AND STATED THE L CDU FAILED AND WENT BLANK. FIGHT DIVERTING TO ANC DUE TO ETOPS AND FUEL RESTRICTIONS.

CORRECTIVE ACTION: REPAIRED DAMAGE ON H-STAB BOX UPPER SKIN FROM FRONT SPAR TO S-11 AT RBL 13.833 IAW STS EA 79-2022-141 REV IR AND OMNI AIR INTERNATIONAL EO B7655-10 INITIAL ISSUE REFERENCE Q1 AVIATION W/O 139617-2, 139617-3 AND 139617-16 FOR ALL NDT HFEC, RHEC AND UT REQUIREMENTS NIL FINDINGS THIS IS A CAT B REPAIR WITH FIRST STAGE APPROVAL WHICH APPROVES THE STATIC STRENGTH DATA AND WHICH PROVIDES THE SCHEDULE FOR SUBMITTAL OF THE DT DATA WHICH MUST BE SUBMITTED NO LATER THAN 24 MONTHS FROM THE DATE OF THIS FORM.

DURING CRUISE FLIGHT, AUTOTHROTTLES DISCONNECTED FOLLOWED BY ALL AUTOPILOTS DISENGAGED AND LEFT FLIGHT DIRECTOR FAILED. REPLACED THE CAPTAIN'S ASA IAW AMM 22-14 AND LEFT TPM IAW AMM 24-32. AUTOPILOTS & AUTOTHROTTLE CHECKED IAW MCDP, GROUND TESTED, NO FAULTS AT THIS TIME ALSO REPLACED CB.
STATUS - APU OIL QTY. CORRECTIVE ACTION:UPON VISUAL INSPECTION FOUND APU OIL SIGHT GAUGE READING LOW OIL QUANTITY. RAN THE APU AND OIL QUANTITY RETURNED TO CORRECT LEVELS IAW B767 AMM 49-11. NO LEAKS NOTED
