WHILE AT CRUISE ALTITUDE, THE SLIDING DOOR BEHIND THE PILOT CAME OPEN. AFTER SLOWING DOWN THE AIRCRAFT THE CREW WAS ABLE TO CLOSE THE DOOR. THE DOOR THEN OPENED A SECOND TIME A FEW MINUTES LATER AND AFTER CLOSING THE DOOR THE CREWMEMBER DECIDED TO HOLD THE DOOR CLOSED FOR THE REST OF THE FLIGHT. THE DECISION WAS MADE TO GO TO AN AIRPORT ADJACENT TO THE INTENDED HOSPITAL DUE TO THE NEED TO HAVE A MECHANIC INSPECT THE AIRCRAFT. IT WAS DETERMINED THAT THERE WAS NOTHING WRONG WITH THE DOOR, BUT USER ERROR BECAUSE OF NOT BEING FAMILIAR WITH THE STYLE OF LATCH COMPARED TO THE CREW MEMBERS REGULAR AIRFRAME.
MULTIPLE ENGINE CHIP CAUTIONS OVER A HANDFUL OF FLIGHTS SINCE MID-APRIL. MOST RECENT EVENT (THIS INCIDENT) OCCURRED LESS THAN ONE DAY AND 0+45 FLIGHT HOURS AFTER PREVIOUS INCIDENT. MULTIPLE CHIPS (IN EXCESS OF 20) FOUND ON CHIP DETECTOR (M01) AND SEVERAL CHIPS/SLIVERS (IN EXCESS OF 5) FOUND ON, AT LEAST, ONE MAG PLUG (M05). AIRCRAFT GROUNDED AND OIL SAMPLES/CHIP CARDS PULLED AND SENT FOR ANALYSIS. REPLACED ENGINE AND HMU
INSPECTING THE CANOPY FRAME ATTACHMENT WEB OF THE 22 DEGREE FRAME, DISCOVERED A CRACK OF APPROXIMATELY 55MM. THIS IS THE VISIBLE PORTION OF THE CRACK, THERE MAY BE MORE BEHIND THE RIVETED BRACKET. WE KNOW THIS IS DEFINITELY A CRACK BECAUSE WHEN WE PUSH ON THE METAL IT SEPARATES.
ON SHORT AND FINAL TO THE HOSPITAL, PILOT AND CREW NOTICED A BURNING SMELL COMING FROM THE AC VENTS, UPON LANDING AND SHUTDOWN THE SMELL DISSIPATED. MAINTENANCE WAS CALLED AND NOTICED A SMALL RESIDUE AROUND A POWER CONNECTION TO THE AFT EVAP BLOWER MOTOR THAT SMELLED A LITTLE BURNT. UPON FURTHER INVESTIGATION THE BLOWER MOTOR WAS FOUND TO NOT BE WORKING AND HAD A REALLY HIGH RESISTANCE. ORDERED A NEW AFT EVAP BLOWER MOTOR.
C130 S/N 7174 WAS FOUND TO HAVE LUGGAGE COMPARTMENT DOOR FRAME DAMAGE AT APPROXIMATELY STA 1863, WL 1990, AND BL 280L/R WITH A MAXIMUM DEPTH OF 0.021
DURING 150 HOUR AIRFRAME INSPECTION, THE MECHANIC FOUND THE LEFT HAND PICK UP WEB CRACKED. THIS CRACK WAS REPAIRED PER APPROVED DATA RECEIVED FROM THE OEM/MANUFACTURER.
DURING SCHEDULED 600HR AIRFRAME INSPECTION FOUND LOWER R/H STABILIZER MOUNT FITTING CRACKED. INFORMATION BELOW IS FOR THE NEXT HIGHER ASSY AS THE PARTS ARE NOT SERIALIZED AND NOT PROCURABLE FROM IPC, REPAIR AND PARTS ARE ONLY ALLOWED FROM AIRBUS ONLY
A CRACK WAS FOUND DURING A 1660.22.1, 600 HR IN 24 MONTH, INSPECTION ON THE 22 DEGREE FRAME APPROX 65 MM IN LENGTH.
PILOT REPORTED GEARBOX CHIP LIGHT ON APPROACH. AFTER LANDING, REMOVED AND INSPECTED CHIP DETECTOR AND FOUND CHIP DETECTOR COVERED WITH SMALL CHIPS. AFTER LOOKING IN THE MM FOR LIMITS, FOUND THE AMOUNT OF CHIPS EXCEEDED LIMITS. GEARBOX WAS REMOVED FROM SERVICE.
