ACFT WAS FOUND TO HAVE A 1.25" LONG CRACK ON THE TRANSMISSION DECK AT THE AFT RIGHT CORNER OF THE OPENING. THIS AIRCRAFT IS OUT OF SERVICE.
DURING A 600 HOUR INSPECTION, THE MECHANIC FOUND 4 CRACKS UNDER THE DEVORE LIGHT DOUBLER ON THE TOP RIGHT HAND SIDE OF THE HORIZONTAL STABILIZER. *** THIS AIRCRAFT REMAINS OUT OF SERVICE AT THE TIME OF THIS SUBMISSION ***
DURING 600 HOUR INSPECTION, FOUND TORQUE LOSS ON UPPER VERTICAL FIN BOLTS WHERE IT ATTACHES TO THE TAILBOOM. REMOVED UPPER AND LOWER FIN ASSY.FOUND THAT DOUBLER HOLDING NUT PLATES IS WORKING AGAINST TAILBOOM RING FRAME. HEARD A POPPING CRACKLING SOUND FROM UPPER UPPER DOUBLER DURING FIN REMOVAL. SUPPECTING CRACK UNDER DOUBLER AMC ENGINEERING CONTACTED FOUND DURING 600 HR INSPECTION. *** THIS AIRCRAFT REMAINS OUT OF SERVICE AT THE TIME OF THIS SUBMISSION ***
DETECTED AND SAW SMOKE IN COCKPIT. SMOKE OBSERVED COMING FROM LOWER RT SIDE OF INSTRUMENT PANEL. TURNED OFF BATTERY SWITCH. DIVERTED FOR UNEVENTFULL UNSCHEDULED LANDING. DETERMINED THAT A DIODE TO THE ACFT CLOCK BACK LIGHTING SYS HAD OVERHEATED ANF MELTED ITSELF AND A TY-WRAP THAT SECURED THE WIRE HARNESS TO A STANDOFF ASSY. DISABLED CLOCK BACK LIGHTING SYS AND DEFFERED.
WITHIN (1) MINUTE AFTER TAKEOFF ON A LOCAL HOSPITAL TRANSFER FLIGHT THE PILOT AND AIRCREW SMELLED BURNING ELECTRICAL EQUIPMENT. THE INSTRUMENT PANEL LIGHTS WENT OUT AT THE SAME TIME. NO FIRE WAS SEEN AND NO VISIBLE SMOKE WAS PRESENT. PILOT TURNED THE ACFT FOR THE HOME BASE HOSPITAL AND PICKED AN ALTERNATE LANDING AREA CLOSER IN CASE THE SMOKE OR FIRE BECAME VISIBLE. ACFT WAS LANDED SAFELY AND WITHOUT FURTHER INCIDENT AT THE HOSPITAL HELIPAD. NO VISIBLE DAMAGE WAS NOTICED DURING POSTFLIGHT INSP. WIRE WAS CHAFING AGAINST THE UPPER RT SIDE OF THE VEMD CASING. UPON WEARING THROUGH THE INSULATION THE WIRE SHORTED BLOWING THE INSTL LTS 2 FUSE. THE WIRE WAS REPAIRED AND REROUTED TO PREVENT FURTHER CHAFING.
MAIN ROTOR BLADE HAS DELAMINATION ON TOP TRAILING EDGE APPROXIMATELY 320 MM FROM TIP. REPLACED WITH NEW UNIT, CORRECTED PROBLEM.
NEUTRAL SWITCH INOPERATIVE WHEN HOT( DURING ENGINE OPERATION AND UP TO APPROX 20 MIN AFTER SHUTDOWN). REPLACED WITH NEW UNIT, CORRECTED PROBLEM.
TRANSPONDER INOPERATIVE, GIVES ERRATIC READINGS. REPLACED WITH NEW UNITS, CORRECTED PROBLEM.
UHF/VHF/LOW/HI BAND TRANSCEIVER - UNIT INOPERATIVE. WILL NOT TRANSMIT ON 467.975 TONE 21 (136.5) WITH GENERATOR ON-LINE. REPLACED WITH LOANER UNIT PN 971083-1 CORRECTED PROBLEM.
ELECTRONIC CONTROL UNIT HAS A FAULT CODE. 135 FLT AMG GOV/LEFT MODULE FADEC. FAIL 2 0200 HEX. REPLACED WITH USED UNIT CORRECTED PROBLEM. NO LOG CARD IN ENGINE LOG BOOK. (OUR UNIT=40 HOURS. REPLACEMENT UNIT=1240 HOURS. DIFFERENCE 1200 HOURS. PLEASE CREDIT THE DIFFERENCE.)
