SHORTLY AFTER TAKEOFF FROM JAX BAPTIST HOSPITAL HELIPAD, CREW NOTIFIED PIC, THERE WAS A STRONG SMELL OF JET FUEL IN THE CABIN. WINDS AT THE HELIPAD WERE 35 KNOTS AND COMING ACROSS MAIN HOSPITAL BUILDING CAUSING VERY STRONG TURBULENCE. I SUSPECTED THE FUEL CAP MAY HAVE BEEN LEFT OPEN AND ELECTED TO CONTINUE TO HURLONG AIRPORT 3 MILE STRAIGHT AHEAD , AT REDUCED POWER. UPON LANDING ON A TAXIWAY ADJACENT TO THE KIDSCARE BASE, A CREW MEMBER IN THE RIGHT REAR SEAT REPORTED THE FUEL CAP WAS ON AND THE LOCKING TAB WAS SLIGHTLY ELEVATED APROX.1/4 FROM IT NORMAL POSITION WITH EVIDENCE OF FUEL AROUND THE FUEL CAP AND SURROUNDING AREA. PIC ASKED HIM TO PUSH THE TAB FULLY CLOSED AND WE WOULD CONTINUE OUR REPOSITION BACK TO OUR BASE. UPON TAKEOFF THE CREW REPORTED THE FUEL SMELL REMAINED. I RETURNED TO THE HURLONG AIRPORT TO HAVE MAINTENANCE CHECK THE CAP. A NEW FUEL CAP WAS ORDERED AND INSTALLED.
AFTER MAIN GEARBOX TIME CHANGE. AIRCRAFT HAD CHIP LIGHT INDICATION AND DID A PRECAUTIONARY LANDING. PULLED, 3 EACH, CHIP DETECTORS PER MAINTENANCE MANUAL. INSPECTION REVEALED, 1 EACH, SMALL SLIVER PARTICLE. CLEANED AND PERFORMED GROUND RUN. AIRCRAFT RETURNED TO SERVICE.
LT ENGINE IGNITION LIGHT CAME ON AND TORQUE INDICATION WENT TO ZERO. REMOVED AND REPLACED IAW CMM 74-00-00. OPERATIONAL CHECK GOOD, RETURNED AIRCRAFT TO SERVICE.
LT TORQUE GAUGE WENT TO ZERO, RETURNED TO DEPARTURE. REMOVED AND REPLACED TORQUE INDICATOR IAW AMM 77-13-05. OPERATIONAL CHECKED GOOD, RETURNED AIRCRAFT TO SERVICE
FLIGHT ATTENDANT REPORTED THE EMERGENCY LIGHT ARMING BUTTON TO BE INOPERATIVE. MAINTENANCE REPLACED THE COCKPIT EMERGENCY LIGHT SWITCH IAW MFG MM. OPS CHECK WAS GOOD AND AIRCRAFT RETURNED TO SERVICE. (SW21200407466)

THE PREVIOUS REPAIR AT THE LOCATION WAS FOUND INCORRECT. REMOVED PREVIOUS DOUBLER AND FABRICATED A NEW DOUBLER. REPAIR DONE IAW SRM.

THE PREVIOUS REPAIR AT THIS LOCATION WAS FOUND TO BE INCORRECT. FABRICATED A NEW REPAIR DOUBLER. REPAIR DONE IAW MFG STATEMENT.

DURING AN MBV INSPECTION AND REPAIR OF HAIL DAMAGE. ON REPAIR STATEMENT DENT, FWD OF FRAME 512 AT SL 1603 WAS FOUND BEYOND SRM LIMITS. THE PREVIOUS REPAIR AT THIS LOCATION WAS DETERMINED TO BE INCORRECT. REMOVED PREVIOUS REPAIR DOUBLER AND FABRICATED A NEW DOUBLER REPAIR DONE IAW REPAIR STATEMENT.
AIRCRAFT SUSTAINED HAIL DAMAGE 8 IN FORWARD OF FRAME 583.0 AT SL 1606 THAT REQUIRED A REPAIR, THE PREVIOUS REPAIR AT THIS LOCATION SWAS FOUND TO BE INCORRECT. DENT WAS INSPECTED BY ULTRASONIC BOND TEST WITH NO DEFECTS NOTED. DENT RECONTOURED IAW MFG SRM. FABRICATED A REPAIR DOUBLER AND TRIPLER.
DURING AN MBV INSPECTION, THE GALLEY FRAME 258.0 WAS FOUND CORRODED BEYOND SRM LIMITS. PERFORMED A 23.0 CUTOUT OF THE DAMAGED AREA OF FRAME 258 AND TREATED THE BARE METAL EDGES. FABRICATED A DOUBLER, SHIMS AND TRIPLER FOR REPAIR. REPAIR DONE IAW MFG SRM.
DFW - FLT 3549 - CREW REPORTED AN OVERHEAD EMERGENCY LIGHT TO BE INOPERATIVE. LIGHT PUT ON MEL AT THIS TIME. MAINTENANCE REPLACED THE LIGHT ASSEMBLY AT COVE 4A. OPS CHECK WAS GOOD AND AIRCRAFT WAS RETURNED TO SERVICE. (M)
DFW - FLT 3290 - ON FINAL APPROACH, HYDRAULIC CENTRAL WARNING PANEL LIGHT ILLUMINATED. MAIN LANDING GEAR NOT INDICATING DOWN. PERFORMED CHECKLIST. ALL GER SUBSEQUENTLY INDICATED DOWN AND LOCKED. HYDRAULIC QUANTITY NORMAL AT ALL TIMES. MAINTENANCE REMOVED AND REPLACED HYDRAULIC PUMP IAW SAAB MM. OPERATIONAL AND LEAK WERE GOOD AND AIRCRAFT WAS RETURNED TO SERVICE. (M)
DFW - FLT 5050 - EXTERIOR EMERGENCY LIGHTS AT CABIN DOOR DO NOT ILLUMINATE. SECURED LOOSE PUSH BUTTON SWITCH TO THE FLIGHT ATTENDANT PANEL. OPERATIONAL CHECK GOOD. (X)
