SHORTLY AFTER TAKEOFF, LOUD HUMMING SOUND COULD BE HEARD IN CABIN AREA. FLIGHT WAS ABORTED AND RETURNED TO CTNAZ. DURING APPROACH FROM BASE TO FINAL, THE HUMMING SOUND STOPPED. INVESTAGATION REVEALED THAT NEWLY INSTALLED HONEYWELL CONNECT TRACKER STC SR00812DE ANTENNA COAXES WERE CHAFFING ON THE INNER LEFT SIDE OF THE OIL COOLER COWLING. IT WAS DETERMINED THAT CLAMPS TO PROVIDE SUPPORT FOR THE COAXES ROUTED OVER THE OIL TANK AS SHOWN ON DRAWING CW0620-1074, SHEET 2 OF 5, WERE NOT INSTALLED. CORRECTIVE ACTION WAS TAKEN BY INSTALLING CLAMPS AS SHOWN ON DRAWING. PERFORMED MAINTENANCE CHECK FLIGHT, HUMMING NOISE WAS NOT DUPLICATED. AIRCRAFT WAS RETURNED FOR SERVICE.

ON TAKEOFF PILOT HEARD A VERY LOUD "BANG" INSIDE THE CABIN. PILOT LOWERED POWER AND SET THE AIRCRAFT BACK DOWN WHERE IT STARTED FROM. DURING INSPECTION BY MAINTENANCE IT WAS FOUND THAT UNDER THE CORRECT CONDITIONS, ROTATING THE TAIL ROTOR CAUSING ROTATION OF THE INPUT SHAFT INTO THE TRANSMISSION WITH NO ROTATION OUT OF THE MAIN ROTOR TRANSMISSION. THERE WAS A "RATCHETING" FEEL WITH SOME DRAG BUT THERE IS DEFINITELY SLIPPING INSIDE THE TRANSMISSION. ***NOTE THIS AIRCRAFT REMAINS OUT OF SERVICE AT THIS TIME AS REPAIRS ARE STILL IN WORK***

PILOT NOTED NO TRANSMISSION OIL PRESSURE INDICATION ON THE TRANSMISSION OIL PRESSURE GAUGE. THE CAWS PANEL TRANSMISSION WARNING LIGHT WAS NOT ILLUMINATED INDICATING THAT THERE WAS INDEED TRANSMISSION OIL PRESSURE. DURING TROUBLESHOOTING TRANSMISSION PRESSURE WAS FOUND TO BE NORMAL AND THEN THE TRANSDUCER WAS REPLACED. THE MALFUNCTION WAS FOUND TO BE THE INDICATOR. PN 90090006-01 S/N 0550.

WHILE ENROUTE TO THE HOSPITAL FROM A SCENE CALL, THE XMSN CHIP LIGHT ILLUMINATED. IMMEDIATE PRECAUTIONARY LANDING WAS CONDUCTED WITHOUT INCIDENT. MAINTINENCE FOUND SMALL METALLIC PARTICLES ON THE LOWER CHIP DETECTOR. GEARBOX OIL AND FILTER WERE DRAINED AND REPLACED, AFTER A GROUND RUN WAS PERFORMED THE CHIP DETECTORS WERE INSPECTED AND FOUND TO BE CLEAN. AIRCRAFT WAS PLACED BACK IN SERVICE.

DURING CRUISE FLIGHT WITH NG ABOVE 94%, OIL PRESSURE DROPPED TO 50 PSI. APPROACH WAS INITIATED AND PRECAUTIONARY LANDING WAS MADE WITH A NORMAL SHUTDOWN OF AIRCRAFT. OIL FILTER HOUSING/REGULATOR WAS CHANGED AND OIL PRESSURE RETURNED TO WORKING PROPERLY.

4 MINUTES AFTER TAKEOFF ALL CREW MENBERS NOTICED A VERY STRONG SMWLL OF HOT ELECTRICAL FUMES THROUGHOUT THE CABIN. NO VISIBLE SMOLE WAS SEEN. CREW RETURNED TO BASE AND ABORTED THE FLIGHT. MAINTENANCE FOUND THE AFT AIRCONDITIONER EVAPORATOR BLOWER MOTOR TO BE INOP. REPLACED THE BLOWER MOTOR WITH A NEW UNIT AND THE SYSTEM WORKED NORMALLY. MOTOR P/N ES61142-1 FOR THE AIR COMM AIRCONDITIONING SYSTEM
