FUEL FILTER ASSY DEVELOPED A LEAK. WAS CHANGED OUT THEN THE SERVICEABLE ONE INSTALLED. OPS CHECK GOOD FOR MAINTENANCE, BUT THE NEXT DAY ON PREFLIGHT INSPECTION BY THE PILOT THE FILTER "BYPASS" WARNING LIGHT BUTTON ON THE ASSY WOULD NOT OPERATE CORRECTLY. PART WAS CHANGED AGAIN AND IS NOW OPERATING CORRECTLY AND NOT LEAKING.
PILOT LANDED AND COMPLAINED OF STRONG ODOR IN AIRCRAFT. FOUND BARRIER FILTER OIL POOLED IN INTAKE PLENUM. CLEANED POOOLED OIL AND RAN UP AIRCRAFT. FUNCTION CHECK GOOD AT THIS TIME
PILOT HAD A TRANSMISSION CHIP LIGHT DURING CRUISE AND LANDED. MAINTENANCE INVESTIGATED THE CHIP DETECTOR AND FOUND 3 TYPES OF PARTICLES. IAW MM 63-00-00, PAGE 29 THE FIRST TYPE OF PARTICLES WERE TINY WHISKER LIKE PARTICLES INSIGNIFICANT, THE SECOND WERE THE IRREGULAR SHAPED FERROUS METAL PARTICLES ONE ALSO INSIGNIFICANT. THE THIRD TYPE WERE THE NEARLY FLAT PIECES, THREE FLAKES , WHICH IS SIGNIFICANT DEBRIS IAW MM. THE MAINTENANCE INSTRUCTIONS FOR THE THIRD TYPE OF DEBRIS IS TO REPAIR OR REPLACE THE COMPONENT.
THROTTLE CABLE SHEARED INTERNALLY AT APPROX 10" FROM COLLECTIVE TWIST GRIP CONNECTION POINT, WHERE CABLE ROUTE IS NAVIGATING A CORNER, DIRECTION CHANGE. THROTTLE CABLE MFG DATE IS 1998, ACFT DOM IS 1999, THIS CABLE HAS NEVER BEEN CHANGED. THIS WAS DISCOVERED DURING ENG SHUT DOWN AFTER A FLIGHT.
DURING FLIGHT, PILOT NOTICED AND SMELLED SMOKE IN COCKPIT WHICH APPEARED TO BE COMING FROM THE CLOCK AREA. PILOT PERFORMED A SAFE PRECAUTIONARY OFF SITE LANDING. PILOT ALSO MENTIONED THAT THE NVG LIGHTING ON OTHER INSTRUMENTS WENT OUT THEN RETURNED AFTER LANDING. WHEN MX ARRIVED THEY FOUND THE CLOCK BACK LIGHTING INOP AND AFTER REMOVING CLOCK FROM INSTRUMENT PANEL AN ACRID BURNED ODOR WAS NOTICED FROM THE CLOCK ASSY. MX NOTICED THE CLOCK WAS PLACARD FOR 5 VOLT LIGHTING AND INSTALLED IN A 24 VOLT LIGHTING SYSTEM. THE AREA AOUND CLOCK WAS INVESTIGATED FOR ADDITIONAL DAMAGE AND NONE FOUND. A USED SERVICEABLE 24 VOLT CLOCK WAS INSTALLED AND SYS CHECKED OK. ACFT WAS RETURNED TO SERVICE.
AIRCRAFT WAS IN ROUTE TO A HOSPITAL. WITHIN 10 MINUTES OF FLIGHT, SMOKE WAS COMING OUT OF THE FWD AIR CONDITIONER VENT. PIC EXECUTED A PRECAUTIONARY LANDING WITHOUT ANY PROBLEMS. FOUND THAT THE FWD RT AIR CONDITIONER BLOWER MOTOR PN ES61060-2 HAD FAILED, CAUSED BINDING WITHIN THE ELECTRIC MOTOR CAUSING SMOKE TO EMIT FROM IT. NO OTHER DAMAGE WAS NOTED AT THIS TIME. AIR CONDITIONER SYS MEL 21-3 CAT D. RETURN ACFT IN AN AIRWORTHY CONDITION.
ON TAKEOFF, PEDAL STOP FAILED TO ENGAGE AND PEDAL STOP CAUTION 1 LIGHT ILLUMINATED. ACFT WAS APPROX 4 NM FROM AIRPORT. PERFORMED EMERGENCY PROCEDURES AND DECIDED TO FLY ACFT TO AIRPORT. NORMAL LANDING WAS MADE AND MECHANIC WAS NOTIFIED. THE PEDAL RESTRICTOR CONTROL UNIT (PRCU) WOULD NOT RELEASE DURING SLOWER SPEEDS OF 55 +- 5 KNOTS, WHICH CAUSED THE PILOT TO PULL MANUAL RELEASE. THE PEDALS THEN RETURN TO FULL RANGE OF CONTROL. TROUBLESHOOTING GUIDES IN THE BHT407MM8 CHAPTER 67. FOUND THAT THE PRCU HAD FAILED. INSTALLED NEW PRCU P/N 407-375-024-101 SN 1026. FUNCTION CHECK FOUND TO BE SERVICEABLE.
