CRACK FOUND AT FRAME 5A RIGHT TOP MOST HOLE EMANATING FROM RIVET HOLE TO EDGE OF DOUBLER APPROX .25 IN LENGTH. WILL REPAIR CRACK AT FRAME 5A IAW EA 23-5310-EA-1480 REV. DATED 06/23/2016 FRAME PROCEDURE ON FAA FORM 337.
DURING A 1000 HOUR AIRFRAME INSPECTION, THE MECHANIC FOUND A CRACK OF APPROXIMATELY 2.25 INCHES IN LENGTH ON THE RT FORWARD MAIN TRANSMISSION DECK AT THE TRANSITION TO THE CABIN ROOF ABOVE THE NR 4 BULKHEAD. THE CRACK IS LOCATED IN THE RADIUS OF THE DOUBLER THAT MAKES UP THE TRANSITION. REPAIRED IAW SRM 51-71-53, 8-7.
THE PILOT REPORTED THAT ON FINAL APPROACH, THE LT TAIL ROTOR PEDAL BECAME STUCK, AND THE ACFT BEGAN FLYING TO THE RIGHT. DECIDED TO ABORT APPROACH, COULD NOT GAIN CONTROL TO LAND AT DESTINATION. DECLARED AN EMERGENCY, AND AFTER 20 SECONDS, PEDAL MOVEMENT RESTORED. WAS CLEARED AND LANDED SUCCESSFULLY WITH NO FURTHER ISSUES. AFTER INSPECTION, IDENTIFIED THAT THE BALL JOINT (REAR OF FLEX BALL CABLE) WAS STIFF AND BINDING. NOTICED A LARGE AMOUNT OF CORROSION AND PITTING ON THE BALL SURFACE.
DURING TAKEOFF, THE PILOT AND MEDICAL CREW ALL DETECTED AN ODOR OF BURNING METAL AND RUBBER. PILOT IMMEDIATELY RETURNED TO THE AIRPORT AND SHUTDOWN AIRCRAFT. FOUND THAT THE Z300 CARD IN THE BATTERY MASTER BOX HAD FAILED. OPEN
