DURING 1000-HOUR AIRFRAME INSPECTION FOUND TWO RIVETS MISSING FROM LH FRAME 4 WEBBING APPROX. 39â FROM FLOOR AND WEB DISTORTED IN THE AREA. ON RH SIDE SAME HIGHT 39â ABOVE FLOOR A 6.5-INCH CRACK IN THE WEBBING AT FRAME 4. WHEN DISASSEMBLY FOR REPAIR ON 6.5 -INCH CRACK DISCOVERED LOWER FRAME ANGLE CRACKED. REPAIR OF FRAME 4 WEB DISTORTED SECTION WAS COMPLETED UNDER RDAS-EC135-53-2022-002B. THE REPAIR OF CRACKED LOWER ANGLE WAS INSTALLING NEW DOUBLER PN SK-0085-21 IAW ADAS AHI-135-53-001-22.
DURING A 400 HOUR INSPECTION, FOUND A CRACK APPROXIMATELY 1.5 INCHES LONG ON THE RT FORWARD MAIN GEARBOX DECK AT THE TRANSITION TO THE CABIN ROOF ABOVE THE NR 4 BULKHEAD. REPAIRED IAW SRM 51-71-53,8-7.
APPROX 10 MINUTES INTO FLIGHT (LEVEL) EXPERIENCED A FADEC MINOR CAUTION ON THE CAD. CONFIRMED THE NR2 TWIST GRIP WAS IN THE NUETRAL POSITION. THE FADEC MINOR THEN BECAME A FADEC FAIL AND RAN THROUGH THAT CHECKLIST AND PERFORMED A SHALLOW APPROACH. VEMD FAILURE CODES (214R & 214L) INDICATED FADEC FAILURE OR WIRING ISSUE. TROUBLESHOT NR2 ENGINE FADEC SYS AND DISCOVERED AIRFRAME CONNECTOR (113VV2AA) HAD LOST TORQUE. CLEANED, REINSTALLED CONNECTOR, PERFORMED 20 MINUTE GROUND RUN AND OPS CHECK FLIGHT. ALL INDICATIONS WERE NORMAL AND RETURNED ACFT TO SERVICE.
DURING FLIGHT, THE MAIN TRANSMISSION OIL PRESSURE INDICATION FLUXUATED. PILOT PERFORMED A PRECAUTIONARY LANDING SHORT OF BASE. TECH DETERMINED THE TRANSMISSION OIL PRESSURE TRANSDUCER WAS DEFECTIVE. REPLACEMENT PART ORDERED.
