Maintenance disclosures
DURING A JUMP RUN, WHILE IN CRUISE, THE LT ENGINE CHIP LIGHT FLICKERED MOMENTARILY & THEN THE ENGINE SHUT DOWN. RETURNED TO THE DEPARTURE & LANDED WITHOUT FURTHER INCIDENT. SUSPECT CATASTROPHIC FAILURE OF THE LT ENGINE. R & R ENGINE. ACFT RETURNED TO SERVICE. TEARDOWN OF FAILED ENGINE, DETERMINED THAT ONE OF THE GEARS ON THE TORQUE SENSOR DRIVE HAD A CATASTROPHIC FAILURE, BREAKING INTO THREE SECTIONS & DROPPED METAL TO THE BOTTOM OF THE GEARBOX. THIS CAUSED THE FUEL CONTROL DRIVE TRAIN TO STOP ROTATING, SHUTTING DOWN THE ENG. THIS PREVENTED THE SCAVENGE PUMP & OIL PUMPS FROM MOVING DEBRIS THROUGH THE REST OF THE ENGINE.
THE LEFT ENGINE FIRE WARNING LIGHT FLICKERED DURING THE TAKEOFF ROLL. THE CO-PILOT CALLED THE LIGHT AND THE PIC ABORTED THE TAKEOFF. NO SECONDARY INDICATIONS WERE OBSERVED. PIC CONFIRMED THAT THE ENGINE WAS NOT ON FIRE AND PERFORMED A NORMAL SHUTDOWN. MAINTENANCE DISCOVERED TWO WIRES THAT WERE CHAFED AND SHORTING. MAINTENANCE REPAIRED THE CHAFING WIRES AND THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING CLIMB AFTER TAKEOFF, THE PILOTS RUDDER PEDALS MOVED ALL THE WAY FORWARD. THE COPILOT TOOK OVER AND SAFELY LANDED THE AIRCRAFT. MAINTENANCE INVESTIGATED AND DISCOVED THE PILOTS RUDDER PEDAL ADJUSTMENT MOUNT BRACKET HAD BROKEN OFF IT'S MOUNT ALLOWING THE RUDDER PEDALS TO SLIDE FORWARD.
