PERFORMED GPS-4000 ANT 1 TEST PROCEDURE IAW COLLINS SYSTEMS MANUAL 523-0775811, 5.11.2.48 OPS CHECK GOOD. THIS RELEASES AIRCRAFT FOR OPERATIONAL CHECK FLIGHT ONLY. OPERATIONAL CHECK FLIGHT PASSED IN ALL PHASES OF FLIGHT.
NDT X-RAY INSPECTION OF HORIZONTAL STABILIZER HAS INDICATIONS OF CRACKS AT RIBS RR2S1, RR2S2, RR3S1, RR3S2, LR2S1, LR2S2, AND LR3S1. REMOVED HORIZONTAL STABILIZER, REINSTALLED SAME STABILIZER AFTER REPAIRS BY TEXTRON AVIATION. TAMPA FL, OPERATIONAL & FUNCTIONAL CHECKS GOOD, WORK PERFORMED IAW BEECH 400XP AMM 27-30-00 & 27-40-00.
WHEEL BRAKES ANTI-SKID FAILED ON LANDING ROLLOUT AFTER SPEEDBRAKE SWITCH WAS PUT IN RETRACT POSITION. INSPECTED ALL BRAKE SYSTEM PLUMBING WITH NO DEFECTS NOTED. INSPECTED BRAKE PEDALS ON PILOTS AND CO-PILOTS SIDE FOR ANY FADING WITH PRESSURE ON HYDRAULIC SYSTEM. NO DEFECTS NOTED. TAXIED AIRCRAFT AND PERFORMED OPS CHECK OF PILOTS AND CO-PILOTS BRAKES WHILE EXTENDING AND RETRACTING SPEED BRAKES. NO DEFECTS NOTED. OPERATIONAL CHECK OF BRAKE SYSTEM SATISFACTORY AT THIS TIME. ALL WORK IAW MM CH 32-40-00.
RT UPPER THRUST REVERSER FAIRING IS CRACKED. REMOVED DAMAGED LT THRUST REVERSER P/N:225-0012-513 S/N:B4549-2 AS REQUIRED REF AMM 78-30-00. INSTALLED CUSTOMER SUPPLIED REPAIRED RT THRUST REVERSER P/N 225-0012-509 S/N B2736 IAW AMM 78-30-00. REMOVED FWD INBD AND OUTBD STANG ATTACHED BRACKETS, AFT UPPER AND LOWER ANTI DRAG FAIRING STANG ATTACHED BRACKET FROM T/R AND INSTALLED THEM ON REPAIRED T/R AS REQUIRED REF CMM 78-30-07. REPAIRED LT T/R AFT INBD AND OUTBD STANG ATTACHED BRACKETS. WORK WAS DONE IN REF CMM 78-30-07 REPAIR NR 11. INSTALLED RT ENGINE T/R INBD AND OUTBDE STANG FAIRINGS AS REQUIRED WITH NO DEFECTS NOTED. PERFORMED MULTIPLE SATISFACTORY OPERATIONAL CHECKS OF THE LT ENGINE T/R WITH NO DEFECTS NOTED AT THIS TIME. WORK WAS DONE IN REF WITH AMM 78-80-00 AND THRUST REVERSER CMM 78-30-07.
LT UPPER THRUST REVERSER FAIRING IS CRACKED. REMOVED DAMAGED RIGHT THRUST REVERSER P/N: 225-0012-515 S/N: B4540-2 AS REQUIRED REF AMM 78-30-00. INSTALLED CUSTOMER SUPPLIED RT THRUST REVERSER P/N 225-0012-515 S/N B4514-2 IAW AMM 78-30-00. REMOVED FWD INBD AND OUTBD STANG ATTACHED BRACKETS, AFT INBD AND OUTBD STANG ATTACHED BRACKET FROM T/R AND INSTALLED THEM ON REPAIRED T/R AS REQUIRED REF CMM 78-30-07. INSTALLED RT ENGINE T/R INBD AND OUTBD STANG FAIRINGS AS REQUIRED WITH NO DEFECTS NOTED. PERFORMED MULTIPLE SATISFACTORY OPERATIONAL CHECKS OF RIGHT T/R WITH NO DEFECTS NOTED AT THIS TIME. WORK WAS DONE IN REF WITH AMM 78-30-00 AND THRUST REVERSER CMM 78-30-07.
RIGHT ENGINE THRUST REVERSER LOWER DOOR HAD A CRACK RUNNING BETWEEN TWO RIVETS ON THE LOWER DOOR INBOARD FIN. REPAIRED IAW CMM 78-30-00 REPAIR 17.
ON APPROACH, PF CALLED FOR GEAR DOWN BEFORE LANDING CHECK AND AFTER GEAR PLACED IN DOWN POSITION, THE RIGHT HALF LANDING GEAR LIGHT DID NOT ILLUMINATE. PM CALLED FOR CHECKLIST, CONSULTED MAINTENANCE AND OPS. APPROACH WAS BROKEN OFF AND DELAY VECTORS REQUESTED WHILE GOING THROUGH CHECKLIST. AFTER ALL CHECKLIST COMPLETED, DECLARED EMERGENCY. LANDING UNEVENTFUL. MAINTENANCE REPLACED THE BULB IN THE INDICATOR, AND OPS CHECKED SATISFACTORY.
DURING CLIMB OUT AT 17000 FT, WITH ENGINE AND WING ANTI-ICE ON, RECEIVED A BL DCT FAIL ANNUNCIATOR. PERFORMED BLEED AIR DUCT FAILURE ABNORMAL CHECKLIST. ANNUNCIATOR EXTINGUISHED AFTER 90 SECONDS. DID NOT RETURN. .MAINTENANCE FOUND AN AIR LEAK AT THE PNEUMATIC DUCT JOINT FWD OF LEFT PRSOV. REPLACED JOINT GASKET. RAN ENGINES , NO LEAKS NOTED.
INVESTIGATED FLIGHT CREW REPORT OF HUMMING/WHISTLING NOISES COMING FROM AFT CABIN CEILING AREA. FOUND SIGNIFICANT LEAKAGE FROM BETWEEN ADF ANTENNA ASSY AND FUSELAGE SKIN. UPON ADF ANTENNA REMOVAL NOTED AFT SPACER INSTALLED INCORRECTLY AND GASKET CUT TO FIT INCORRECT INSTALLATION. RE-ORIENTED SPACER IAW SERIES IPC 34-51-15-01 AND REINSTALLED ADF ANTENNA ASSY WITH NEW GASKET. LEAK CHECKS OK, NO ABNORMAL NOISES NOTED WITH CABIN PRESSURIZED. TRANSIENT ACFT, RECORDS NOT AVAILABLE, BUT SUSPECT ANTENNA INSTALLED INCORRECTLY SOME TIME PREVIOUSLY, AND TOOK REPEATED PRESSUIZATION CYCLES FOR SEALANT TO FAIL. RECOMMEND MFG INVESTIGATE FURTHER OR ISSUE A SB TO DETERMINE IF INCORRECT ADF ANTENNA INSTALLATION PROCEDURES ARE BEING PERFORMED AT MFG, OR IF THIS WAS A 1-TIME OCCURENCE.
HUMMING/WHISTLING NOISE IN AFT CABIN CEILING FROM CLUB SEATS BACK TO LAV AREA. PRESSURIZED ACFT FOUND ADF ANTENNA LEAKING DUE TO IMPROPERLY INSTALLED GASKET. REMOVED AND REPLACED GASKET WITH NEW, INSTALLED ANTENNA AND SEALED.
