VERY LOUD HISSING NOISE AT FL400, CABIN VSI WAS SHOWING A RAPID CLIMB AND BEGAN LOSE PRESSURIZATION. REPLACED PRESSURIZATION CONTROLLER PERFORMED PRESSURIZATION CHECKS NO DEFECTS FOUND HAWKER 400A CH 21-30-00 AND 52-10-00.
MX REPORTED THE RT UPPER THRUST REVERSER OUTBOARD STIFFENER CRACKED
MX REPORTED THE RH UPPER TR OUTBOARD STIFFENER CRACKED. MX REMOVED AND REPLACED THE UPPER THRUST REVERSER OUTBOARD STIFFENER WITH A NEW UNIT. IAW NORDAM CMM 78-30-07 REPAIR NO 1
FOUND WIRE TERMINALS LOOSE ON RIGHT ENG STARTER/GENERATOR. CLOSER EXAMINATION FOUND THE WIRE TERMINAL ADAPTER THREADS STRIPPED. THERE IS A COMMUNIQUE, KA-2014-02, ADDRESSING THIS DEFECT ON KING AIRS WHERE THERE ARE REPLACING THE STEEL LOCK NUTS WITH NYLON LOCK NUTS, THE STEEL LOCK NUT PN 130909N25 AND THE NYLON LOCK NUT IS MS21083N6. CONTACTED TEXTRON STATING THAT THE ADAPTER PN WAS THE SAME ON BOTH STARTER/GENERATORS, KING AIR AND BEECH JET. THERE IS NO DOCUMENTATION TO PUT THE NYLON LOCK NUTS ON THE BEECH JET S/G. I WAS GIVEN THE STORY THAT THE BEECH JET STARTER/GENERATORS WERE IN A HOTTER LOCATION THAN THE KING AIR, WHICH IS FAR FROM THE TRUTH. THIS SEEMS TO BE AN ONGOING PROBLEM WITH THE BEECH JETS WHERE STARTER/GENERATORS ARE DROPPING OFF LINE AND FINDING LOOSE TERMINALS. BILL AT THE BEECH SAID THE ANSWER MAYBE TO SUBMIT A FORM 337 AND MAKE THE CHANGE. I DON`T THINK THIS IS THE ANSWER.
DURING THE CLIMB THE MASTER WARNING LIGHT ILLUMINATED ALONG WITH THE RT ENG THRUST REVERSER ARM & UNLOCK LIGHT. FAULTY RT THRUST REVERSER SWITCH TO BE THE CAUSE.
FLIGHT CREW REPORTED ACFT STARTED "PORPOISING" +/- 300 FT AT 800 FPM WHILE IN CRUISE FLIGHT AT FL400. COPILOT'S SIDE DISPLAY SIMULTANEOUSLY DISPLAYED "PITCH TRIM FAIL" INDICATION. ACFT DIVERTED. FOUND ELEVATOR CABLES AND ELEVATOR SERVO BRIDLE CABLE TENSIONS BELOW SERVICE LIMITS AND READJUSTED. CHECKED AVIONICS FAULT LOG AND NOTED PITCH TRIM FAIL INDICATED ON NR 2 FCC-4007 FLIGHT CONTROL COMPUTER CARD. REPLACED NR 2 FCC-4007 CARD WITH SERVICEABLE FCC-4007 CARD. NO FAULTS INDICATED DURING POST-MX CHECKS. RECOMMEND AIRFRAME MFG INVESTIGATED WHETHER CABLE TENSION CHECKS NEED TO BE PERFORMED MORE FREQUENTLY OR SERVICE LIMITS ADJUSTED. RECOMMEND MFG INVESTIGATED FAILED FCC-4007 CARD FAIL MODE, APPEARS TO BE POSSIBLY COLD-SOAK RELATED.
FLIGHT CREW REPORTED NORMAL HORIZ STAB NOSE DOWN PITCH TRIM WOULD NOT OPERATE AT AIRSPEEDS ABOVE 250 KIAS. REQUIRED EXTENSIVE TROUBLESHOOTING AND FLIGHT CHECKS WITH MFG TECH SUPPORT TO CORRECT. REPLACED AND SWAPPED NUMEROUS ELECTRICAL COMPONENTS, PITCH TRIM ACTUATORS. REPLACED PN 132409-13, ZENER DIODES A120CR110 AND A120CR111 SEVERAL TIMES, ALSO REPLACED ADJACENT PN M81824/1-2 SPLICE A120SP1807 AFTER INSULATION RESISTANCE CHECKS SHOWED POSSIBLY SUSPECT. ADJUSTED PITCH TRIM ACTUATOR JACK SCREW TO STABILIZER MOUNTING BOLT GAP TO SERVICE LIMIT MAXIMUM OF .008 IN. IAW MM 27-40-00. UNABLE TO DETERMINE EXACT FAILURE MODE, MFG INVESTIGATING. SUSPECT ZENER DIODES A120CR110 AND A120CR111 MAY BE FAILING OPEN AFTER WAR
