LOSS OF CABIN PRESSURE. CABIN PRESSURE LOW LIGHT ON.
RIGHT LANDING LIGHT DOOR HINGE BROKEN. REMOVED AND REPLACED UPPER DOOR HINGE AND HINGE PIN.
L/H UPPER THRUST REVERSER BUCKET INBD AND OUTBOARD STIFFNER FOUND CRACKED
L/H UPPER THRUST REVERSER BUCKET INBD AND OUTBOARD STIFFNER FOUND CRACKED. MX REMOVED AND REPLACED LEFT INBD AND OUTBOARD UPPER TR STIFFNERS WITH NEW UNITS. IAW NORDAN CMM 78-30-07
IN MAINTENANCE DURING ENGINE INSPECTION AND THE FUEL BOOST PUMP ON, A VERY TINY STREAM OF FUEL (ABOUT THE SIZE OF A FINE HAIR) WAS FOUND SPRAYING FROM THIS LINE ONTO THE SIDE OF THE ENGINE DRIVEN FUEL PUMP. WITH THE ENGINE RUNNING THE LEAK IS VIRTUALLY UNNOTICEABLE. WITH LINE REMOVED AND UNDER A 10X MAGNIFYING GLASS THE PIN HOLE IS VISIBLE, ALSO NOTED WHAT LOOKS LIKE POSSIBLE TOOL MARKS NEARBY. SUSPECT A MANUFACTURING DEFECT THAT COULD RESULT IN AN ENGINE FIRE.THIS IS THE FOURTH OCCURRENCE OF THIS P/N FUEL LINE WITH A PIN HOLE IN THE EXACT SAME LOCATION THAT WE HAVE SEEN IN THIS SHOP RECENTLY. 4 DIFFERENT AIRCRAFT OF THE SAME MODEL, WITH THE SAME ENGINE MODEL.
AFTER TAKEOFF & SELECTING LG UP, THE LG IN TRANSIT LIGHT REMAINED ILLUMINATED, & THE FLIGHT CREW COULD HEAR THAT THE GEAR WAS NOT UP & LOCKED. CREW DETERMINED THAT DUE TO THE PRESENT ICE ACCUMULATION THAT IT WAS AN EMER & ELECTED TO RETURN TO THE AIRPORT INSTEAD OF REMAINING IN THE MODERATE ICING CONDITIONS IN THAT CONFIGURATION. CREW SELECTED LG DOWN, OBTAINED ALL THREE GREEN GEAR DOWN INDICATIONS, & IN TRANSIT LIGHT EXTINGUISHED. ACFT LANDED WITHOUT FURTHER INCIDENT.
