DURING 400 HOUR INSPECTION FOUND A CRACK APPROX. 1.5" LONG ON THE R/H FORWARD MAIN GEARBOX DECK AT THE TRANSITION TO THE CABIN ROOF ABOVE THE #4 BULKHEAD. THE CRACK IS LOCATED IN THE RADIUS OF THE DOUBLER THAT MAKES THE TRANSITION. REPAIR REQUIRES DOUBLERS IAW EC-135 STANDARD REPAIR MANUAL SRM 51-71-53,8-7.

PILOT REPORTED BURNING RUBBER SMELL IN COCKPIT UPON ENGINE SHUTDOWN BACK AT BASE. HE MADE A LOG BOOK ENTRY NOTATING THE DISCREPANCY AND CALLED ME IN TO INSPECT THE AIRCRAFT. I INSPECTED THE AIRCRAFT VENTILATION, HEATING AND AC SYSTEMS. ALL WERE FOUND TO BE IN AN AIRWORTHY CONDITION I THEN PERFORMED A GENERAL VISUAL INSPECTION OF ALL THE MAJOR COMPONENTS ON THE AIRCRAFT. THE PILOT AND I THEN PERFORMED A GROUND RUN OPERATIONAL CHECK OF ALL THE AIRCRAFT SYSTEMS. OPS CHECK NOTED GOOD. NO PROBLEMS FOUND.

NURSE IDENTIFIED SMALL QUANTITY OF FUEL LEAKING FROM MEDICAL CABIN CEILING AT AFT RT SIDE STATION THROUGH LIGHT FIXTURE AFTER LANDING AT HOSPITAL. FUEL WAS FOUND IN RT SIDE ENGINE COMPARTMENT INTAKE AREA ON BOTTOM DECK AS WELL AS A FEW DROPS FROM LOWER DECK DRAIN. FOUND JAM NUT ON TOP OF FUEL CONTROL HAD FUEL SEEPING AROUND THREADS. CHECKED AND TORQUED ALL FUEL LINES IN THE VICINITY OF THE LEAK. PERFORMED 15 MINUTE PENALTY GROUND RUN WITHOUT FURTHER EVIDENCE OF LEAKAGE. RETURNED ACFT TO SERVICE.

THE NR 2 ENGINE FIRE DETECTION WARNING LAMP ILLUMINATED DURING FINAL APPROACH TO THE HOME HELIPAD. THE LANDING WAS COMPLETED WITHOUT EVENT AND THE ENGINES WERE SHUTDOWN. NO FIRE OR SMOKE. TROUBLESHOOTING REVEALED A DEFECTIVE FIRE DETECTOR.
