BURNING SMELL WAS NOTED INSIDE AIRCRAFT IN FLIGHT. TROUBLE SHOOTING LEAD TO #2 STARTER/GENERATOR CAUSING SMOKING ELECTRICAL SMELL.
STRINGER CRACKED @ FS 5315 BL 145 AND CHAFE HOLE IN DECK COVER @ FS 4820 BL 375. FOUND DURING HEAVY MAINTENANCE, AND AIRCRAFT IS STILL CURRENTLY OUT OF SERVICE.
CRACKS FOUND IN 5 PIECES OF SUPPORTING STRUCTURE OF # 1 TAILROTOR DRIVESHAFT BEARING MOUNT. AM SERVICE ENGINEERING WORKING WITH AGUSTA ON REPAIR. *** THIS AIRCRAFT REMAINS OUT OF SERVICE AT THE TIME OF THIS SUBMISSION PENDING AN APPROVED REPAIR FOR THESE CRACKS. ***
FOUND SEVERAL CRACKS AT AFT BULKHEAD STA 3525 ON BOTH THE LT AND RT SIDES ALONG WITH THE INNER MAIN GEARBOX TRANSMISSION MOUNT LT CRACKED THAT MOUNTS TO THE AFT BULKHEAD AT STA 3525.
ON TAKE OFF PILOT FELT UNCONTROLLED ROLL. ABORTED TAKE OFF AND RETURNED TO BASE.
THE NUMBER TWO ENGINE FLAMED OUT ON CLIMB-OUT AND SHOWED NO FUEL PRESSURE SHORTLY THEREAFTER. THE FUEL PRESSURE REAPPEARED AND THE ENGINE RESTARTED WITH NO FURTHER PROBLEMS. THE PILOT RETURNED TO BASE. ONCE SAFELY ON THE GROUND, MAINTENANCE WAS CALLED IN TO TROUBLESHOOT THE ISSUE AND FOUND A BROKEN WIRE ON THE FUEL PUMP. THE WIRE WAS REPAIRED, TEST FLIGHT COMPLETED WITHOUT ANY FURTHER DEFECTS NOTED. THE AIRCRAFT WAS RETURNED TO SERVICE AT THAT TIME.
NR 1 ENGINE, OIL CHIP WARNING LIGHT ILLUMINATED IN FLIGHT.
FIRE DETECTION LIGHT ILLUMINATED DURING FLIGHT, UPON AIRCRAFT LANDING MECHANIC CHECKED PART AND DID TROUBLESHOOTING PROCEDURES TO DETERMINE THAT THE FIRE LOOP ITSELF WAS BAD.
DURING NORMAL CRUISE FLIGHT, THE PILOT RECEIVED A MAIN GEAR BOX CHIP LIGHT, PROCEEDED TO THE NEAREST AIRPORT WITH OUT INCIDENT. AFTER LANDING, THE MECH FOUND A FOUND A SMALL PIECE OF METAL ON THE CHIP DETECTOR, REMOVED THE PIECE OF METAL AND REINSTALLED THE CHIP DETECTOR AND RAN THE ACFT, AND NO CHIP LIGHT. ALL WORK WAS PERFORMED IAW MM. THE AIRCRAFT WAS THAN RETURNED TO SERVICE.
