PILOT REPORTED THAT DURING APPROACH TO LANDING, HE GOT A AMBER GOV LIGHT. HE ABORTED LANDING AND DIVERTED TO EL PASO FBO. VEMD FAULT CODES WHERE SW ENG CTL (HEX 4204 AND HEX 000C) EMM REFERRED TO START STOP SWITCH, AIRCRAFT JUST CAME OUT OF HEAVY MAINTENANCE FROM LRU M FACILITY AND HAD HEADLINER TAKEN DOWN DURING MAINTENANCE. AT ELP I REMOVED THE START SWITCH HEADLINER AND COVER INSPECTED WIRING AND TERMINALS. AMBER GOV LIGHT OPS CHECKED GOOD ON GROUND TESTS. PERFORMED OPERATIONAL GROUND RUN WITH NO FAULTS OR GOV LIGHTS. COULD NOT DUPLICATE AMBER GOV LIGHT. AIRCRAFT RETURNED TO SERVICE. PILOT FLEW BACK TO DEMING NM BASE AND REPORTED BACK TO MECHANIC NO ISSUES AND NO GOV LIGHTS.

ATA 6510

DURING A VISUAL INSPECTION OF THE TAIL ROTOR DRIVE BEARINGS IT WAS DISCOVERED THAT THE INNER SLEEVE OF THE #3 BEARING HAD SPLIT AND COME OFF OF THE DRIVESHAFT AND THE BEARING AND WAS FOUND SITTING BELOW THE BEARING ASSEMBLY. THIS CAUSED NO VISUAL DAMAGE TO THE DRIVESHAFT ASSEMBLY BUT WE HAVE NEVER SEEN A SLEEVE SEPARATE LIKE THAT BEFORE.

PILOT REPORTED DURING CLIMB OUT FROM BASE AT KDMN EN ROUTE TO SENDING FACILITY HE TURNED ON THE CABIN VENTILATION AND WATER STARTED POURING OUT OF UPPER A/C VENTS. HE THEN TURNED OFF THE A/C VENT SYSTEM AND RETURNED TO BASE AND SHUT THE AIRCRAFT DOWN WITHOUT ISSUE, WROTE THE AIRCRAFT UP AND CONTACTED ON CALL MECHANIC. MECHANIC DISCOVERED AFT EVAPORATOR VENT DUCTING WAS CRACKED AND HAD ALLOWED RAIN WATER TO COLLECT AND ONCE FAN WAS TURNED ON IT PUSHED WATER THRU THE VENT SYSTEM.

HAD BEEN AN ISSUE ON A PRIOR FLIGHT, TROUBLESHOT AND PARTS REPLACED WHICH WERE THOUGHT TO CORRECT THE PROBLEM. REPLACED THREE ENGINE FIRE DETECTORS AND THE FIRE TEST PUSHBUTTON ON THE 30A PANEL.

WHILE PERFORMING AN AIRWORTHINESS CHECK, FOUND A 6" CRACK ON THE LT SIDE IF THE TAIL BOOM AT THE AFT T/R G/B COVER ATTACHMENT POINT. THE CRACK STARTS AT A RIVET SECURING THE ATTACHMENT POINT AND MOVED VERTICALLY BOTH DIRECTIONS. THE PREVIOUS EVENING, PERFORMED AN AIRWORTHINESS CHECK AND DID NOT SEE ANY CRACKS. CHECKED THIS AREA BECAUSE OF THE COMMONLY CRACKED AREA ON THE UPPER VERSICLE STABILIZER LOWER RIB. THE MORNING FOUND CRACK, HAD PREVIOUSLY PERFORMED 2 GROUND RUNS. 1 FOR 15 MINUTES TO CHECK THE FACET BYPASS INDICATOR, AND ANOTHER FOR 5 MINUTES TO PERFORM A LEAK CHECK. AFTER THE GROUND RUNS, PERFORMED A AIRWORTHINESS CHECK AND FOUND THE CRACK. THE CRACK IS VERY HARD TO SEE WHEN LOOKING FWD AND WITH THE T/R G/B COVER IS ON. THE AREA IS ALSO OVER 6` HIGH DUE TO THE HELIPAD NOT BEING LEVEL.
