Part: ENGINE — FAILED
ENGINE FAILED IN FLIGHT. NR 5 AND NR 6 CONNECTING ROD (CRANK END) PROTRUDING THROUGH TOP OF ENGINE CASE. HOLES IN BOTH SIDES OF CASE BELOW NR 5 AND NR 6 CYLINDERS ABOVE OIL PAN FLANGE. THE ENGINE HAS BEEN IN SERVICE SINCE 07/2016. IT IS A FACTORY REBUILT ENGINE.

FOUND BOTH LEFT AND RIGHT T CHANNELS CRACKED APPROX 8 INCHES. THESE CHANNELS ARE LOCATED ON THE FORWARD CORNERS OF THE NOSE WHEEL WELL BOX ASSY AND ACCESSED FROM THE NOSE BAGGAGE AREA. POSSIBLE CAUSE FOR CRACKING DUE TO EXCESSIVE SIDE LOADING WHILE TAXIING. (M)
Part: INTAKE TUBE — CRACKED (FLANGE)
FUEL STAINS WERE NOTED AROUND SEVERAL INTAKE TUBES. IN REMOVING THE COLLARS FROM THE CYLINDERS TO GAIN ACCESS TO THE INJECTORS, TWO OF THE INTAKE TUBES WERE FOUND CRACKED AROUND THE RADIUS OF THE FLANGE.
