APU FAULT LT ON AFTER 3 START ATTEMPTS NO START. FOUND NO OIL IN APU. FOUND CRACK ON TOP OF GEN AT 1200 POSITION APU GENERATOR SHARES THE OIL SYSTEM AND RESERVIOR WITH THE APU. THE LINE AND APU SHOP HAS HISTORY OF GENERATOR HOUSINGS CRACKING IN THE SAME FASHION AS THIS EXAMPLE. OIL LEAKS FROM THE GENERATOR CRACK, DEPLETING THE SUPPLY OF OIL OF THE APU. THE APU WILL HAVE A "LOW OIL PRESSURE" PROTECTIVE SHUTDOWN. SUBSEQUENT START ATTEMPTS WILL BE UNSUCCESSFUL UNTIL THE OIL SYSTEM IS SERVICED AND THE ECU (ELECTRONIC CONTROL UNIT) FAULTS ARE CLEARED. THEREFORE, WITHOUT OIL IN THE SYSTEM, ANY ATTEMPT TO START THE APU AT ALTITUDE WILL BE UNSUCCESSFUL.
EES WAS TEST FIRED AND PRESSURE RECORDED GOOD. HOWEVER, THE BOTTOM TUBE WENT FLAT WITHIN 5 MINUTES OF INFLATION PRESSURE RELIFE VALVE FOUND LEAKING. PRESSURE RELIEF VALVE ASSEMBLY WAS REPLACED.

PCA WAS FOUND TO HAVE WORN PISTON ROD SEALS CAUSING THE LEAKAGE. OVERHAULED ACTUATOR WAS TESTED AND RETURNED TO SERVICEABLE CONDITION.
DIVERTED TO DEN UNDER AMBER ALERT WITH NR 2 ENGINE SHUTDOWN DUE TO OIL QUANTITY LOSS. OIL LEAK CAUSED BY FAULTY FUEL PUMP ALLOWING OIL TO LEAK THROUGH THE WET SPLINE AREA.
EXPERIENCED NR 2 ENGINE FIRE. SECURED ENGINE AND ACTIVATED FIRE EXTINGUISHERS. FOUND THE HPT CASE COOLING VALVE FUEL RETURN LINE CROSS THREADED AT THE 2.5 BLEED VALVE FUEL PRESSURE RETURN LINE.
DURING APPROACH THE FLIGHT CREW WAS UNABLE TO EXTEND FLAPS FULLY. OP CHECKED FLAPS, BITE CHECKED FLAP SLAT ELECTRONIC UNIT, AND VISUALLY INSPECTED FLAPS. NO DEFECTS FOUND.
