BLEED AIR R RED ANNUNCIATOR ILLUMINATED AT FL 220.
THE NR 2 ENGINE BEGAN TO COMPRESSOR STALL. IT FLUCTUATED BETWEEN 400 AND 700 DEGREES C. RIGHT THROTTLE WAS RETARDED TO IDLE AND COMPRESSOR STALL STOPPED. COMPRESSOR STALL RESUMED AS THROTTLE WAS ADVANCED ABOVE IDLE.
TAKEOFF WARNING HORN SOUNDED WITH FLAPS 20 DEGREES DURING TAKEOFF ROLL AT 135 KIAS. STAB ACTUATOR WAS REPLACED
FLASHING RED ENRY DOOR LIGHT ILLUMINATED AND REMAINED ON. INSPECTION OF DOOR GUIDE PINS FOR ENGAGEMENT WAS SATISFACTORY. LUBRICATED MAIN ENTRY DOOR. NO FURTHER ISSUES WITH THIS WARNING.
#2 ENGINE TR AUTO STOWED AFTER CREW COMMANDED DEPLOYMENT DURING LANDING ROLLOUT. MAINTENANCE PERFORMED ENGINE RUN OPERATIONAL CHECK OF THRUST REVERSERS AND PERFORMED OPERATIONAL CHECKS OF ANTI SKID SYSTEM. ALL SYSTEMS CHECKED GOOD AT THIS TIME. WILL CONTINUE TO MONITOR.
STRONG FUEL SMELL DURING INITIAL CLIMB AFTER TAKE OFF. MAINTENANCE NOTED NO FUEL LEAKS AT THIS TIME. MAINTENANCE RAN MOTORS TO TAKE OFF POWER WITH NO FUEL SMELL NOTED AT THIS TIME. WILL CONTINUE TO MONITOR.
RIGHT ENGINE CHIP ANNUNCIATOR ILLUMINATED IN FLIGHT. ENG WAS REPLACED.
AFTER ARRIVAL A FUEL LEAK ON THE NR 1 ENG WAS NOTED. A CRACK IN THE PRIMARY FUEL LINE FROM THE FCU TO THE SOV WAS DISCOVERED.
PILOTS AIRSPEED INDICATOR FAILED DURING TAKEOFF. DEBRIS WAS FOUND & CLEARED FROM INSIDE NR 1 PITOT PROBE.
ON CLIMBOUT FOLLOWING A LOW APPROACH, LANDING GEAR DID NOT RETRACT WHEN GEAR HANDLE WAS RAISED. GEAR HANDLE RETURNED TO THE DOWN POSITION. LANDING GEAR REMAINED DOWN AND LOCKED. AIRCRAFT WAS FLOWN WITH LANDING GEAR EXTENDED AND LANDED UNEVENTFULLY.
SELECTED GEAR DOWN AND OBSERVED FLASHING HYDRAULIC PRESSURE LOW LIGHTS. LANDING GEAR EXTENDED DOWN WITH NORMAL INDICATIONS (TIME TO EXTEND WAS LONGER THAN NORMAL). HYDRAULIC LOW PRESSURE LIGHTS EXTINGUISHED. HYDRAULIC PRESSURE GAUGE SHOWED NORMAL. ON BASE TO FINAL TURN, HYDRAULIC LIGHTS ILLUMINATED INTERMITTENTLY AGAIN THEN STAYED ON. HYDRAULIC PRESSURE GAUGE STILL SHOWING NORMAL. ELECTED TO EXECUTE GO-AROUND TO RUN ABNORMAL CHECKLIST. CONFIGURATION REMAINED FLAPS 20 DEGREES WITH GEAR DOWN. REQUESTED DELAY VECTORS FROM ATC AND DECLARED EMERGENCY. RAN APPROPRIATE ABNORMAL CHECKLISTS. BRIEFED FOR POSSIBLE LOSS OF BRAKES, SPOILERS, AND TR'S ON LANDING. PREPARED FOR USE OF EMERGENCY BRAKE SYSTEM. CONTINUED FOR LANDING AND SELECTED AND OBTAINED FULL FLAPS ON FINAL. HYDRAULIC LOW PRESSURE LIGHTS REMAINED ILLUMINATED BUT HYDRAULIC PRESSURE GAUGE STILLED SHOWED NORMAL. CONTINUED TO LANDING AND THEN FOUND NORMAL BRAKES FAILED SHORTLY AFTER TOUCHDOWN. HYDRAULIC PRESSURE GAUGE DROPPED INTO LOW END OF YELLOW RANGE AT THIS POINT. CONDUCTED UNEVENTFUL STOP WELL SHORT OF RUNWAY END USING EMERGENCY BRAKING. SPOILERS AND TR'S WERE UNAVAILABLE. MAINTENANCE INSPECTION AFTER LANDING FOUND FAILED HYDRAULIC LINE IN WHEEL WELL.
AFTER CONDUCTING LOW APPROACH, LANDING GEAR HANDLE WAS REPOSITIONED TO THE UP POSITION. THE GEAR DID NOT RETRACT. CIRCUIT BREAKERS WERE CHECKED AND RESET, THE GEAR STAYED DOWN AND GEAR INDICATORS REMAINED GREEN. CLIMBED TO ALTITUDE AND FLEW AND LANDED WITHOUT INCIDENT. TROUBLESHOT AND FOUND RT GEAR UP TRUNNION SWITCH BAD AND RT GEAR DOWN SWITCH WAS SATURATED IN WATER. REPLACED SWITCHES, OPS CK GOOD.
CREW REPORTED SMELL OF FUEL IN AFT CABIN AT ALTITUDES OF 2500 FT AND BELOW IN LEVEL FLIGHT. IT WAS ALSO NOTICEABLE ON GROUND. IT WAS LESS NOTICEABLE AT HIGHER ALTITUDES AND UNDER PRESSURIZATION. MAINT LOCATED SOURCE OF SMELL. FWD WING CENTERLINE SPLICE PLATE (PN 602240111) WAS SEEPING FUEL PAST RIVETS. SPLICE IS LOCATED JUST AFT OF CABIN PRESS POPPET VALVES, WHICH ARE FOUND AT END OF CABIN AISLE WALKWAY. WHEN AC WAS AT ALTITUDE, POPPET VALVES WERE HELD CLOSED BY CABIN PRESSURE, BUT AS AC DESCENDED (2500 FT AND BELOW) CABIN PRESSURE DECREASED ALLOWING POPPET VALVES TO OPEN AND PERMIT JET FUEL FUMES TO ENTER CABIN. TECH RECOMMENDS TO DEMATE WING FROM FUSELAGE, REPAIR LEAKS AND REMATE WING. AC IN REPAIR.

PILOTS RIGHT BRAKE PEDAL WENT DOWN FULL TRAVEL FOLLOWED BY BOTH HYD PRESS LOW LIGHTS ILLUMINATING. HYDRAULIC FLUID DRIPPING FROM WEEP HOLES IN BELLY OF AIRCRAFT. FOUND POWER BRAKE VALVE ADJUSTING SCREW BROKEN AND LAYING IN BELLY OF AIRCRAFT. (M)
