[SUP 06-06-07] ALSO CHECKED NR 123 AND NR 5 BEARING C-D'S AND FOUND EXCESSIVE MATERIAL. FILTER AND C'D'S REINSTALLED. REPLACED ENGINE. ATTRIBUTED THE PROBLEM TO FAILED BEARINGS.
DURING ENGINE START SMOKE AND SPARKS WERE SEEN COMING FROM TAILPIPE OF NR 1 ENGINE. ON ENGINE START NOTIFIED BY GND CREW THAT SMOKE AND SPARKS WERE COMING FROM TAILPIPE OF NR 1 ENGINE. SPARKS CAUSED BY FAULTY STARTER. CHECKED INLET EXHAUST AND CHIP DETECTORS PRECAUTIONARY. ALSO ACCOMPLISHED GENERAL VISUAL OF ENGINE. NO FAULTS FOUND. RE-STARTED ENGINE THREE TIMES AND VERIFIED NORMAL OPERATION WITH NO FURTHER SPARKING. REPLACED STARTER AND START VALVE.
RETURNED TO THE FIELD DUE TO SMOKE IN THE COCKPIT. CAUSE OF INCIDENT WAS A FAILED THRUST DISK & BEARING IN LT ACM. COMPONENT FINDINGS WERE A FAILED THRUST DISK & A FAILED BEARING WHICH ALSO DAMAGED TURBINE & COMPRESSOR SECTIONS. THRUST DISK, BEARING TURBINE ASSY & COMPRESSOR WERE ALL REPLACED & UNIT MADE SERVICEABLE. THIS UNIT INSTALLED ON 06 JUN 2000 & ACCUMULATED A TOTAL OF 15,397 HRS & 5,718 CYCLES PRIOR TO FAILURE. CURRENT MX PROGRAM IS ON CONDITION FOR ACM. A HEALTH CHECK IS PERFORMED ON SYS & TEMP SENSORS CHECKED & CALIBRATED AT 1C. NO CORRECTIVE ACTIONS OR MX PROGRAM CHANGES REQUIRED AT THIS TIME.
DOOR 3L SLIDE LIGHT WOULD NOT GO OFF WITH DOOR DISARMED. ALTHOUGH UNABLE TO DUPLICATE INCIDENT, IT IS SPECULATED CONDITION WAS CAUSED BY DIRT OR DEBRIS ON PROXIMITY SENSORS WHICH AFFECTS THE INDUCTANCE NECESSARY FOR SYSTEM OPERATION.
LOW BOTTLE PRESSURE. RESERVOIR/VALVE ASSEMBLY SHAFT O-RING LEAKING. REPLACED SHAFT O-RING AND BACK-UP O-RING.
