FOLLOWING THE SEVERAL LEAK ISSUES WE HAD WHILE PERFORMING THE POST INSTALLATION CHECKS ON CRFS INSTALLATION KIT, AS350 PN- HM019-600-11 SN- 0025 WE WERE INSTRUCTED TO REPLACE THE TANK ASSY WITH ONE FROM KIT SN- 0023. AFTER INSTALLATION OF THE NEW TANK ASSY WE PERFORMED THE POST INSTALLATION CHECKS AGAIN. EVERYTHING WENT GOOD, ALL THE CHECKS PASSED AND THE TANK DID NOT LEAK. WE TOPPED OFF THE TANK AFTER THE CHECKS TO LET IT SIT OVERNIGHT AND LEAK CHECK AGAIN IN THE MORNING. A MECHANIC TURNED POWER ON THE SHIP AND NOTICED THEâ FUEL QUANTITYâ INDICATION WAS YELLOW AND NOT DISPLAYING PROPERLY. WE CHECKED THE FUSES AND CONNECTORS AND EVERYTHING LOOKED GOOD. FROM THERE WE STARTED TROUBLESHOOTING PER THE ICA INSTRUCTIONS WITH A FULL TANK OF FUEL. A CONTINUITY CHECK OF THE WIRING WAS PERFORMED AND PASSED. ALSO ALL FUEL QUANTITY INDICATION CHECKS PASSED WITH THE PREVIOUS TANK. THEN OPERATION OF THE FUEL QUANTITY SENDER WAS SIMULATED BY APPLYING 0-5 VOLTS TO PIN 11 OF THE FUEL CELL HARNESS TO CONFIRM THE FUEL QUANTITY INDICATION WENT FROM EMPTY TO FULL AND IT PASSED. WE VERIFIED THERE WAS 28 VOLTS COMING FROM PIN 7 OF THE FUEL CELL HARNESS AND IT PASSED (POWER TO FUEL QUANTITY SENDER) THE TROUBLESHOOTING PARA 6.2- FUEL GAUGE OUTPUT TROUBLESHOOTING- WAS PERFORMED AND FOUND THAT PIN 11 ON THE FUEL CELL WAS SHOWING 22 VOLTS WHICH IS NOT IN THE 0-5 VOLT RANGE.
INSTALLED A CRASH RESISTANT FUEL SYSTEM STC SR02492AK INTO AS350B2 S/N 4337. FUELED AIRCRAFT TO PERFORM REQUIRED LEAK AND FUEL QUANTITY CHECKS. AFTER FUELING, FOUND A LEAK FROM THE BOTTOM PLATE OF THE FUEL TANK. UPON INSPECTION FOUND THE LEAK COMING FROM THE PLUMBING MODULE DOWN THE WIRED OF THE FUEL QUANTITY WIRING HARNESS. PROCEEDED TO DEFUEL THE AIRCRAFT AND NO FUEL WOULD COME OUT OF THE MIN FUEL LINE. AFTER FURTHER INVESTIGATION FOUND THE MAIN FUEL LINE TO THE TANK QUICK DISCONNECT DEFECTIVE. REPLACED QUICK DISCONNECT AND DEFUELED AIRCRAFT.
UPON REMOVAL OF FWD TAIL ROTOR DRIVE SHAFT, FOUND THE NR 6 DRIVE SHAFT BEARING SUPPORT ATTACHMENT HOLES ELONGATED.
DURING INSPECTION FOUND THE LEFT AND RIGHT CABIN EDGING FRAMED CORRODED BEYOND LIMITS.
DURING INSPECTION FOUND THE LEFT & RIGHT KEEL BEAM INBOARD ANGLES CORRODED BEYOND LIMITS REQUIRING REPLACEMENT, AND THE RIGHT KEEL BEAM WEB CORRODED BEYOND LIMITS REQUIRING REPAIR.
ON DESCENT TO HOSPITAL, AS THE PILOT DECREASED COLLECTIVE, THE HIGH RPM AUDIO SOUNDED. HE CHECKED THE RPM INDICATION, AND IT WAS READING 410 (HIGH). HE IMMEDIATELY INCREASED POWER TO THE CRUISE SETTING AND THE RPM RETURNED TO 390 (NORMAL). HE GENTLY TRIED TWICE TO INITIATE THE DESCENT WITH THE SAME RESULTS. HE THEN NOTIFIED AIRCOMM THAT HE WOULD BE DIVERTING TO KMSV TO COMPLY WITH THE EMERGENCY PROCEDURES FOR LANDING. UPON ARRIVAL AT KMSV AND ALIGNED WITH THE RUNWAY, HE COMPLETED THE SLOW RUN-ON LANDING WITHOUT ANY FURTHER ISSUES. THE AIRCRAFT REMAINS OUT OF SERVICE AT THIS TIME AWAITING PARTS (FUEL CONTROL REPLACEMENT).
