RIGHT MAIN GEAR BRAKE CAUGHT FIRE.
RUDDER UPPER ATTACH FITTING FLANGE CRACKED.
FLAPS WOULD NOT EXTEND. TIGHTENED LOOSE FLAP SWITCH HANDLE, CLEANED & RESEATED 2 CONNECTORS.
LANDING GEAR RETRACTION DELAYED APPROX 1 MINUTE FOLLOWING SELECTION OF GEAR HANDLE UP. FOUND HYD ACCUMULATORS TO BE 700PSI. SERVICED HYD ACCUMULATORS TO 850PSI. OPERATIONS CHECKED GOOD.
DURING ROLLOUT CHECK, ACFT EXPERIENCED A TIRE FAILURE ON THE RR SIDE FOLLOWED BY THE OTHER TIRE FAILING APPROX 15 SECONDS LATER. WHEN THE FIRST TIRE FAILED, PF BEGAN SLOWING THE ACFT & TURNING OFF ON THE HIGH SPEED TAXIWAY THAT WE WERE APPROACHING. THE SECOND TIRE FAILED AS WE WERE EXITING THE RUNWAY. PF BROUGHT THE ACFT TO A STOP & THE MS NOTICED SMOKE COMING FROM THE WW ON THE RT SIDE. PNF DECLARED AN EMER WITH TOWER & CREW RAPIDLY EXITED THE ACFT. BY THE TIME FIRE CREWS ARRIVED THE SMOKE HAD DISSIPATED. EMER TERMINATED BY FIRE CREW.
DURING PRE-START CHECKS WITH ACFT BATTERIES ON AND APU RUNNING AND ALL SYSTEMS (AFIS, AVIONICS) OPERATING, DISTINCT ACRID FUMES AND FAINT GREY SMOKE OBSERVED IN CABIN. ALL SYSTEMS OPERATED NORMALLY AND NO CIRCUIT BREAKERS TRIPPED. ACFT POWERED DOWN AND MX CONTACTED FOR FURTHER INVESTIGATION. MX OBSERVED NR2 ATC RECEIVER SHOWS FAIL LIGHT. UPON INSP, DISCOVERED BURNING ELECTRICAL SMELL COMING FROM INSIDE RECEIVER. PULLED NR 2 ATC CB AND COLLARED NR 2 AFIS ATC.
SEVERE NOSEWHEEL VIBRATION AT APPROX. 130 KIAS ON TAKEOFF ROLL. STRONG VIBRATION AND LOUD NOISE DURING CLIMBOUT WITH GEAR DOWN. LASTED FOR OVER A MINUTE. GEAR REMAINED DOWN FOR REMAINDER OF FLIGHT. NORMAL LANDING MADE WITH NO ABNORMALITIES NOTED.
WHILE CONDUCTING A LOW APPROACH (ILS-3 MANEUVER) AT THE LOGAN-CACHE, LOGAN, UTAH AIRPORT, AIRCRAFT INGESTED A BIRD WITHIN THE RIGHT ENGINE AS THE AIRCRAFT PASSED THE THRESHOLD AT AN ALTITUDE OF 50 FEET. THE WEATHER AT THE TIME OF THE INCIDENT WAS CLEAR SKY'S, GOOD VISIBILITY, AND CALM WINDS. THE AIRCRAFT EXPERIENCED NO ABNORMAL ENGINE INDICATIONS. THE FIRSTINDICATION OF THE ENGINE BIRD INGESTION WAS A "FOUL" ODOR THROUGHOUT THE AIRCRAFT CABIN. THE RIGHT ENGINE BLEED AIR VALVE WAS SELECTED OFF AND THE FLIGHT WAS SUBSEQUENTLY TERMINATED. A PRECAUTIONARY, UNEVENTFUL LANDING WAS MADE AT THE SALT LAKE CITY INTL, SALT LAKE CITY, UTAH AIRPORT. THE AIRCRAFT WAS RELEASED TO MAINTENANCE FOR FURTHER EVALUATION. RT
THE A/C WAS IN THE HANGAR AND A SMALL AMOUNT WAS DETECTED ABOVE THE AIRCRAFT, ON INVESTIGATION THE LT HORIZONTAL STAB DE-ICER BOOT WAS FOUND SMOKING AND ON FIRE. POWER WAS REMOVED FROM THE AIRCRAFT AND THE FLAMES AND SMOKE QUICKLY DIED OUT. THE STAB HEAT SWITCH WAS FOUND IN THE OFF POSITION. THE AVIONICS TECH WORKING ON THE AIRCRAFT HAD EXTERNAL POWE ON FROM TIME TO TIME WHILE TROUBLESHOOTING AN UNRELATED PROBLEM. FOUND CONTACTS WELDED SHUT ON REMOTE CONTROL CIRCUIT BREAKER. REPLACED RCCB AND HORIZONTAL STAB DE-ICE BOOT. (M)
AFTER TAKEOFF, CREW FELT VIBRATION IN AIRCRAFT. SUSPECT COMPRESSOR STALLS ON NR 2 ENGINE. ADVANCED AND RETARDED THROTTLE ON NR 2 ENGINE AND NOTICED ENGINE POPPING AND VIBRATION. NR 2 ENGINE SET AT 88 PERCENT N2, WHERE IT RAN SMOOTH. RETURNED KOKC. ON LANDING, BOTH ENGINES IN REVERSE (IDLE) AT 40 KTS. ITT RISING THROUGH 900 DEG CELSIUS AND ENGINE POPPING. SECURED ENGINE. THE TEARDOWN REPORT INDICATES THE REPORTED OVER TEMPERATURE CONDITION CAN MOST LIKELY BE ATTRIBUTED TO THE FAULTY BLEED VALVES AND SOLENOID. (M)
