DAMAGE WAS DISCOVERED ON THE L/H BEAM UNDER THE ENGINE DECK (P/N: 355A21-2110.25) IN THE FORM OF AN ELONGATED HOLE IN THE LOWER FLANGE NEAR THE AFT END OF THE PART
THE RH TRANSMISSION DECK EDGING (PN 350A21-1041-22) HAS A GOUGE THAT IS UP TO 0.200â IN DIAMETER AND ALMOST THROUGH THE PART.
RT LOWER MAIN GEARBOX COWLING DEPARTED AIRCRAFT IN FLIGHT AND STRUCK THE TAIL ROTOR OF THE AIRCRAFT AFTER THE IMPACT THE AIRCRAFT HAD TO MAKE A PRECAUTIONARY LANDING. THIS AIRCRAFT REMAINS OUT OF SERVICE AT THIS TIME WHILE ALL OF THE REQUIRED INSPECTIONS ARE COMPLETED, AND A REPLACEMENT COWLING IS ACQUIRED.
CORROSION WAS DISCOVERED ON THE RIGHT HAND SIDE CABIN FLOOR EDGE SECTION (AIRBUS PN 350A21-1359) RANGING FROM STA 1120 TO 2437.
AC 4613 HAS A SMOOTH DENT ON THE TAILBOOMâS RT SKIN LOCATED AT APPROXIMATELY STA 8098, WL 1829, AND BL 70R JUST FORWARD OF THE HORIZONTAL STABILIZER OPENING. THE DENT IS .4 BY .4 AND .035 AT ITS DEEPEST.
A/C 4613 HAS SEVERAL DENTS LOCATED ON THE ENGINE DECK LOCATED AT APPROXIMATELY STA 4387, WL 1759, AND BL 650L. THE DAMAGE IS IN AN AREA 2.4 BY 1.8 WITH THE DEEPEST DENT MEASURING .045. THE DENTS ARE LOCATED ABOVE A FRAME WHICH IS UNDAMAGED.
CHAFING NOTED ON THE LEFT HAND X-FRAME P/N: 350A21-1051
PILOT & CREW REPORTED STRONG BURNING RUBBER SMELL IN COCKPIT AT ENG RUN UP TO IDLE. PILOT ABORTED FLIGHT & CALLED MECHANIC. CHECKED ACFT SYS, FINDING NO OBVIOUS DEFECTS. HAD PILOT RUN ENG AT IDLE & FULL POWER WHILE CHECKING VARIOUS SYSTEMS INCLUDING BLEED AIR DEMISTER SYS & AIR CONDITIONING SYS. NO INDICATION WARNING LIGHTS WERE ILLUMINATED & NO DEFECTS WERE NOTED. SMELL WAS NEVER REPRODUCED EVEN FAINTLY & ACFT WAS RETURNED TO SERVICE. MECHANIC BELIEVES SMELL MAY HAVE BEEN FROM PORTABLE ELEC HEATER THAT SITS ON LITTER & KEEPS CABIN WARM IN THE WINTER. SOMETIMES THE WARM AIR ENHANCES THE SMELL OF THE RUBBER & PLASTIC TYPE LITTER MATTRESS PAD COVERING, AS THIS ALSO IS NOTICEABLE IN THE SUMMERTIME, IF THE SUNSHADE IS NOT INSTALLED ON WINDSCREEN.
FOUND 10MM CRACK IN TAILBOOM MOUNTED BATTERY ACCESS DOOR, TOP RT CORNER, DURING 100HR A/F INSPECTION.
SMOKE IN CABIN FRONT LT SIDE OF INSTRUMENT PANEL, WHILE IN HOVER. PERFORMED COMPLETE INSP OF AFFECTED AND SURROUNDING AREAS. NO EVIDENCE OF SMOKE OR FAULTY WIRING FOUND. SUSPECT SMOKE WAS POSSIBLY CHAFED FROM GLARESHIELD/WINDSHIELD CENTER POST, BLOWN THRU VENTS. PERFORMED OPS FLIGHT CHECK SATISFACTORY. ACFT APPROVED RETURN TO SERVICE.
DFW EMERGENCY LIGHT INOPERATIVE AT SEAT 16D. REPLACED LIGHT HARNESS AT SEAT 16D. SYSTEM GROUND CHECKED NORMAL OPERATION.
