DURING INSPECTION IT WAS NOTED THAT THE COCKPIT VOICE RECORDER (CVR) WAS NOTED AS ONLY RECORDING VOICES FROM THE AREA MIC. THE VOICES FROM THE PILOT'S AND COPILOTS MICROPHONES WERE NOT BEING PICKED UP AND RECORDED. FURTHER INSPECTION WILL NEED TO BE DONE IN ORDER TO DETERMINE THE CAUSE OF THIS DEFECT.
RIGHT MLG DOWN ANNUNCIATOR NOT ILLUMINATING. REPLACED GEAR LIGHT ASSEMBLY PN 351-33790-007 IAW DIAGRAM 32-61-01. OPERATIONS CHECKED GEAR SWING SAT IAW BE19000C MM 32-30-00.
DEPART ANC, PAX INDICATED SMOKE IN CABIN, LEVEL OUT, TURN BLEED AIR OFF, DUMP CABIN, DECLARED EMERGENCY, ROW 2 LEFT SIDE ROW 5 BOTH SIDES INDICATE BY 3 PAX. PERFORMED HP ENGINE RUNS WITH BLEEDS ON IN HOT AND COLD MODES, COULD NOT DUPLICATE VAPOR/SMOKE SMELL DURING OPERATION. FOUND FORWARD VENT BLOWER VERY HOT TO TOUCH, FOG NOZZLE AT HEAT EXCHANGER CLEAR OF OBSTRUCTIONS, FOUND WATER COLLECTOR PIPE AT ACM CRACKED. FORWARD VENT BLOWER REMOVED AND REPLACED IAW B1900C MM 21-51-01. OPS CHECK GOOD. OK FOR CONTINUED SERVICE. REPLACED WATER COLLECTOR PIPE WITH SERVICEABLE UNIT IAW B1900C MM 21-51-01.
ANC - FLAPS STUCK AT TAKEOFF. ANC- INFLIGHT GEAR HANDLE LIGHT DOES NOT COME ON. WARNING HORN SOUNDS. ADJUSTED RIGHT FLAP SWITCH IAW B-1900 MM 27-50-06. OPS CHECKED GOOD. OPS CHECKED GEAR WARNING SYSTEM AFTER ADJUSTING SPLIT FLAP SWITCH IAW B-1900MM 32-60-06. GEAR HANDLE LIGHTS AND HORN OPS CHECK GOOD. OK FOR SERVICE.
SMOKE IN CABIN REPORTED BY PASSENGERS. PASSENGERS BELIEVE THE SOURCE MAY HAVE BEEN THE FLOOR VENTS. PILOTS COULD NOT CONFIRM SMOKE, BUT RETURNED TO FAIRBANKS IMMEDIATELY AFTER DEPARTURE. CONFIRMED AFT VENT MOTOR AS CAUSE OF SMOKE. REMOVED BAD MOTOR INSTALLED OVERHAULED MOTOR. GROUND OPERATIONAL CHECKS IN LOW AND HIGH SATISFACTORY.
NR 1 ALTIMETER INOP. REPLACED ALTIMETER AND DID 2 YEAR CERTIFICATION IAW FAR 91.411, IAW FAR 43 APPENDIX E.
INTERMITTENT AND COMPLETE FAILURE OF EHSI, COMM NR 1 AND GPS SIMULTANEOUSLY WHILE IN CRUISE FLIGHT. HAPPENED TWICE FOR APPROX 1 TO 2 MINUTES. CHECKED ALL CONNECTIONS EHSI, COMM NR 1 COMM, GPS RESEATED ALL CONNECTIONS OPS CHECK SAT. ON GROUND. AFTER START THE GPS, COMM NR 1 AND EHSI DO NOT HAVE POWER. WILL COME ON AFTER RESETTING AVIONICS' MASTER 2-3X. REPLACED K136 RELAY SYSTEM OPS OKAY.
LT FIRE PULL HANDLE FLASHED INTERMITTENT (2 SEC) ON DESCENT. ALL INDICATIONS OTHERWISE WERE NORMAL DID TEST LT LOOP AND AMP NEITHER WOULD TEST.
WHEN EXTENDING GEAR IN KSM, NONE OF THE GREEN INDICATORS FOR THE GEAR DOWN & LOCKED INDICATION ILLUMINATED. RAN THROUGH QRH. RETURNED TO ANC, FLEW BY TO VISUALLY CONFIRM GEAR DOWN. LANDED WITHOUT INCIDENT. REMOVED AND REPLACED LIGHT BULBS IN GEAR DOWN POSITION INDICATOR LIGHTS & OPS CHECKED GOOD.
SEVERAL RIVETS HOLDING THE HINGE TO THE AIRSTAIR DOOR ASSY HAD PULLED THROUGH THE LOWER CLOSURE REINFORCEMENT. THE REINFORCEMENT, A COMPONENT OF AN AIRSTAIR DOOR REINFORCEMENT KIN, 114-4023-3, HAD BEEN INSTALLED 1,374 HOURS EARLIER AS PART OF MFG DESIGNED FIELD REPAIR. A NUMBER OF RIVETS SECURING THE AIRSTAIR DOOR HINGE TO THE FUSELAGE WERE FOUND TO BE WORKING. FURTHER INVESTIGATION FOUND CRACKS IN THE DOOR FRAME BETWEEN THE RIVET HOLES IN THE UPPER LINE OF RIVETS TOWARDS THE AFT END OF THE HINGE. (AS01200503513)
WHEN TRANSFERRING FUEL FROM ONE MAIN TANK TO ANOTHER, THERE WAS A SOUND LIKE A HAMMER HITTING THE PIPES. NARROWED IT DOWN TO THE RIGHT MAIN FUEL TANK, FUEL MANIFOLD CHECK VALVE SPRING WAS BROKEN AND THE VALVE WAS CHATTERING, WHEN PRESSURE WAS APPLIED.
LOWER AFT AIRSTAIR DOOR LATCH CABLE BROKE, PREVENTING DOOR BEING OPENED. PASSENGERS HAD TO EXIT THRU CARGO DOOR. WHEN THE AIRSTAIR DOOR IS CLOSED, ANY DIRT OR MOISTURE ON THE TREADS IS FUNNELLED TOT HE LOWER PULLEY FOR THIS CABLE. THIS CABLE IS BLENDED AND BROKEN IN TWO LOCATIONS THAT CORRESPOND TO THE LOWER AFT PULLEY TRAVEL AND LOWER AFT LATCH CAM PULLEY. OPERATOR RECOMMENDS MORE FREQUENT DETAILED INSPECTION OF CABIN DOOR.
AIRSTAIR DOOR WOULD NOT OPEN. BROKEN CABLE PREVENTED LATCHES FROM ROTATING.
AIRSTAIR DOOR LATCH CABLE FRAYED.
NR 1 GENERATOR DROPPED OFF-LINE. WOULD NOT RESET. ITT BEGAN TO RISE. POWER REDUCED, BUT WOULD NOT LOWER ITT. THEN FIRE HANDLE LIGHT ACTIVATED. SHUT DOWN ENGINE. TEAR DOWN WITH MFG''''S PMI PRESENT. MASSIVE FAILURE OF ARMATURE WINDINGS. STATOR GOUGED BY ARMATURE CAUSING HEAT WHICH MELTED SOLDER HOLDING SHAFT TO ARMATURE. IT IS THOUGHT THAT COMMUTATOR RETAINING BAND SHIFTED AND HIT BRUSH BLOCK CAUSING PIECES GETTING INTO ARMATURE AND STATOR WINDINGS. (X)
PILOT'S WINDSHIELD CRACKED IN-FLIGHT. LANDED AT GAL. NOT E: WINDSHIELD HUMP SEALS HAD BEEN REPLACED ON 6-23-99.
