NTSB accidents & incidents
37 records, spanning 1992–2020.
Operated by Hageland Aviation Services (per NTSB report)
The improper installation of a hydraulic line at the right main landing gear (MLG) unlock actuator, which resulted in a total loss of system hydraulic pressure and failure of the landing gear to extend. Contributing to the accident was the failure of the right MLG shuttle valve due to corrosion, which prevented the right MLG from extending during the emergency blowdown activation.
Operated by Hageland Aviation (per NTSB report)
The pilot’s unstabilized approach and improper landing flare, which resulted in a hard landing.
Operated by HAGELAND AVIATION SERVICES INC (per NTSB report)
The pilot's decision to continue a visual flight rules flight into an area of instrument meteorological conditions and his subsequent failure to level the airplane after the autopilot disconnected, which resulted in a collision with terrain.
Operated by HAGELAND AVIATION SERVICES INC (per NTSB report)
The flight crew's decision to continue the visual flight rules flight into deteriorating visibility and their failure to perform an immediate escape maneuver after entry into instrument meteorological conditions, which resulted in controlled flight into terrain (CFIT). Contributing to the accident were (1) Hageland's allowance of routine use of the terrain inhibit switch for inhibiting the terrain awareness and warning system alerts and inadequate guidance for uninhibiting the alerts, which reduced the margin of safety, particularly in deteriorating visibility; (2) Hageland's inadequate crew resource management (CRM) training; (3) the Federal Aviation Administration's failure to ensure that Hageland's approved CRM training contained all the required elements of Title 14 Code of Federal Regulations 135.330; and (4) Hageland's CFIT avoidance ground training, which was not tailored to the company's operations and did not address current CFIT-avoidance technologies.
Operated by Hageland Aviation Services, Inc. (per NTSB report)
The failure of both pilots to see and avoid each other while in level cruise flight, which resulted in a midair collision.
Operated by HAGELAND AVIATION SERVICES INC (per NTSB report)
The pilot's failure to maintain clearance from a forklift while taxiing.
Operated by Hageland Aviation (per NTSB report)
The check airman's delayed remedial action and initiation of a recovery procedure after a simulated pitch trim excursion, which resulted in a loss of airplane control.
Operated by Hageland Aviation (per NTSB report)
The pilot's decision to initiate a visual flight rules approach into an area of instrument meteorological conditions at night and the flight coordinators' release of the flight without discussing the risks with the pilot, which resulted in the pilot experiencing a loss of situational awareness and subsequent controlled flight into terrain. Contributing to the accident were the operator's inadequate procedures for operational control and flight release and its inadequate training and oversight of operational control personnel. Also contributing to the accident was the Federal Aviation Administration's failure to hold the operator accountable for correcting known operational deficiencies and ensuring compliance with its operational control procedures.
Operated by Hageland Aviation Services, Inc (per NTSB report)
The captain’s decision to initiate a visual flight rules approach and attempted landing into an area of instrument meteorological conditions, which resulted in the airplane touching down short of the runway. Contributing to the accident was the operator’s inadequate procedures for operational control and flight release and its inadequate training and oversight of operational control personnel. Also contributing to the accident was the Federal Aviation Administration’s failure to hold the operator accountable for correcting known operational deficiencies and ensure compliance with its operational control procedures.
Operated by HAGELAND AVIATION (per NTSB report)
The pilot's continued flight into adverse weather and his failure to maintain clearance from terrain while on approach in flat light conditions. Contributing to the accident was the pilot's delayed decision to initiate a go-around.
Operated by Hageland Aviation Services (per NTSB report)
The total loss of engine power as a result of a fractured first-stage compressor blade due to fatigue cracking. The source of the fatigue crack could not be determined due to secondary damage sustained to the fracture surface.
Operated by Hageland Aviation Services (per NTSB report)
The pilot's failure to abort the takeoff when he realized the airplane could not attain sufficient takeoff and climb performance and his improper decision to depart from an airport where night operations were prohibited.
Operated by Hageland Aviation (per NTSB report)
The pilot’s decision to land long on the icy snow-covered runway, resulting in a runway excursion and collision with terrain.
Operated by HAGELAND AVIATION (per NTSB report)
The pilot's decision to attempt to takeoff on a short, wet runway with a heavily loaded airplane, and his failure to abort the takeoff before it became impracticable.
Operated by Hageland Aviation Services, Inc. (per NTSB report)
The pilot's inadequate compensation for crosswind conditions, which resulted in the left wing striking the ground while taxiing. A factor in the accident was a crosswind.
Operated by Hageland Aviation (per NTSB report)
The in-flight collision with a bird while on final approach to land.
Operated by HAGELAND AVIATION SERVICES INC (per NTSB report)
An in-flight collision with ocean waters while maneuvering for an undetermined reason.
Operated by Hageland Aviation Inc. (per NTSB report)
A complete loss of engine power during cruise flight due to the shifting of a main crankshaft bearing that produced a fatigue failure of the crankshaft. A factor contributing to the accident was unsuitable terrain for a forced landing, which resulted in a nose over .
Operated by HAGELAND AVIATION SERVICES INC (per NTSB report)
The airplane encountered unforecast turbulence, which resulted in serious injury to the pilot.
Operated by HAGELAND AVIATION INC. (per NTSB report)
The pilot's improper fuel management, and subsequent fuel starvation during cruise flight. A factor contributing to the accident was unsuitable terrain for a forced landing.
Operated by HAGELAND AVIATION SERVICES INC. (per NTSB report)
The pilot's failure to maintain directional control of the airplane during the landing roll resulting in the right wing striking the runway.
Operated by HAGELAND AVIATION SERVICES INC. (per NTSB report)
The failure of the pilot-in-command to maintain the proper glide path to the runway during the final approach for a VFR landing.
Operated by HAGELAND AVIATION SERVICES (per NTSB report)
The pilot's inadequate compensation for wind conditions during the takeoff roll. A factor associated with the accident was a crosswind.
Operated by HAGELAND AVIATION SERVICES (per NTSB report)

Operated by HAGELAND AVIATION SERVICES INC (per NTSB report)
The pilot's inadequate preflight inspection to remove all contaminated fuel from the fuel system. Factors in the accident were the pilot's inadequate planning/decision to continue a second takeoff following an engine surge and aborted takeoff, and unsuitable terrain for a forced landing.
Operated by HAGELAND AVIATION SERVICES INC (per NTSB report)
The pilot's continued VFR flight into instrument meteorological conditions. Factors in the accident were low ceilings, and whiteout conditions.
Operated by HAGELAND AVIATION SERVICES INC (per NTSB report)
The pilot's disregard for lateral fuel loading limits, his improper removal of frost prior to takeoff, and the resulting inadvertent stall/spin. Factors involved in this accident were the improper asymmetrical fuel loading which reduced lateral aircraft control, the self-induced pressure to takeoff on time by the pilot, and inadequate surveillance of the company operations by company management.
